Prediction of the mediastinal drainage after coronary artery bypass surgery.

Using multiple correlation and linear regression approaches, we investigated the association between the amount of mediastinal drainage for the first 24 postoperative hours and clinical variables as well as multiple haematological tests performed at three time points: before anaesthesia induction, 10 minutes after protamine administration and just after skin closure, on 46 patients undergoing primary coronary artery bypass grafting. Three models from the three times were then developed to predict mediastinal drainage. The number of internal mammary grafts, the total number of grafts and plasma fibrinogen concentration were useful predictors of mediastinal drainage at all three times. The platelet count taken only after skin closure was found to provide additional predictive information. Each regression model explained approximately 60% of the variation in postoperative mediastinal drainage. The information obtained from these predictive models is useful in defining high-risk populations.

Effect of calcium or 25OH vitamin D3 dietary supplementation on bone loss at the hip in men and women over the age of 60.

Dietary supplements that prevent bone loss at the hip and that can be applied safely in the elderly are likely to reduce hip fractures. A daily dietary supplement of 750 mg calcium or 15 microg 25OH vitamin D3 on bone loss at the hip and other sites, bone turnover and calcium-regulating hormones were studied over 4 yr in elderly volunteers using a randomized, double-blind, placebo-controlled trial. Bone mineral density (BMD) was measured by dual x-ray absorptiometry and bone structure by radiographs. Calcium biochemistry and bone turnover markers were measured in blood and urine. The 316 women entering the trial had a mean age of 73.7 yr and the 122 men of 75.9 yr. Baseline median calcium intake was 546 mg/day, and median serum 25OH vitamin D3 was 59 nmol/L. On placebo, loss of BMD at total hip was 2% and femoral medulla expansion was 3% over 4 yr. Calcium reduced bone loss, secondary hyperparathyroidism, and bone turnover. 25OH vitamin D3 was intermediate between placebo and calcium. Fracture rates and drop-out rates were similar among groups, and there were no serious adverse events with either supplement. A calcium supplement of 750 mg/day prevents loss of BMD, reduces femoral medullary expansion, secondary hyperparathyroidism, and high bone turnover. A supplement of 15 microg/day 25OH vitamin D3 is less effective, and because its effects are seen only at low calcium intakes, suggests that its beneficial effect is to reverse calcium insufficiency.

Plasma endothelin-1 levels and circulating endothelial cells in patients with aortoarteritis.

To investigate the correlation between plasma endothelin-1 (ET-1), circulating endothelial cells (CECs), and the disease activity in patients with aortoarteritis. In this study, radioimmunoassay was used to measure plasma levels of ET-1 in 56 patients with aortoarteritis. Circulating endothelial cell counts were also carried out as an indicator of vessel wall lesions. The plasma levels of ET-1 and CECs in the active disease patient group were significantly higher than those in inactive patient group (p<0.001). A significant positive correlation was found between plasma ET-1 levels and erythrocyte sedimentation rates (ESRs) in patients with aortoarteritis (r=0.645, p<0.001), as well as CECs (r=0.876, p<0.001). These results suggested that the ET-1 secreted during the active stages of aortoarteritis may cause constriction and proliferation of vascular smooth muscle cells, thus contributing to the pathogenesis of luminal narrowing. The increased CECs might serve as a marker of disease activity.

Cimetidine absorption and elimination in rat small intestine.

The purpose of this study was to determine the characteristics of intestinal absorption and metabolism of cimetidine. The initial finding of the appearance of cimetidine sulfoxide in rat and human jejunum from cimetidine perfusions had prompted an isolation of mucosal membrane transport and enterocyte metabolism contributions in earlier membrane vesicle and microsomal studies, respectively. In this report, perfusion studies in rat small intestine detail regional differences in intestinal elimination. Cimetidine S-oxide appears to a significantly greater extent in the jejunum compared with the ileum. Jejunal metabolite appearance is shown to be a function of the pH-dependent intracellular uptake of cimetidine. Cimetidine permeability decreases with increasing perfusion concentration in both jejunum and ileum. Similar permeability magnitudes and concentration dependence are observed in both regions. Perfusion studies with inhibitors of cimetidine mucosal transport and inhibitors of microsomal S-oxidation provide an inhibition profile suggesting that jejunal cimetidine permeability decreases with increasing intracellular cimetidine concentration. The data support a reduction in paracellular cimetidine absorption as controlled by intracellular cimetidine. This inference is drawn on the basis of mass balance. Because significant appearance of cimetidine S-oxide was previously found in human jejunal perfusions, this region-dependent intestinal elimination process detailed in rats may be relevant to drug plasma-level double peaks observed in clinical studies. Saturation of jejunal metabolism at typical oral doses may limit paracellular absorption of cimetidine in the jejunum and contribute to the double peak phenomenon and to absorption variability.

Block Copolymer Nanotubes.

Rigid cylindrical micelles are made from polyisoprene-block-poly(2-cinnamoylethyl methacrylate)-block-poly(tert-butyl acrylate). The PI-PCEMA-PtBA polymer assembles to micelles with PI as the core, PCEMA as the shell, and PtBA as the corona. The cylindrical structure is fixed by photocrosslinking the PCEMA shell, and PCEMA-PtBA nanotubes are obtained by degrading the PI core with ozone (see scheme).

[Application of bio-electrical impedance analysis in the compensation period of intracranial pressure increasing].

In the compensation period of intracranial pressure increasing, brain impedance rises continuously while cerebrospinal fluid volume changes. Along with the illness progressing, intracranial pressure would rise obviously, but the intracranial component relative volume would go near to constant; at the same time, brain impedance fluctuates around a baseline and there appears an inflection point on the impedance track record curve after the compensation period.

Prevention of acute allergic conjunctivitis and late-phase inflammation with immunostimulatory DNA sequences.

PURPOSE: To evaluate the therapeutic potential of immunostimulatory sequence oligodeoxynucleotides (ISS-ODN) administration in ocular allergy, using a mouse model of ragweed-specific conjunctivitis. METHODS: A murine model of allergic conjunctivitis involving SWR/J mice sensitized and challenged with short ragweed was used to test immunostimulatory DNA sequences for therapeutic potential. ISS-ODN or control ODN (0.1 mg/mouse) was administered intraperitoneally or topically to the conjunctiva 3 days before final allergen challenge. Multiple parameters of clinical symptoms evident during the acute-phase reaction and the cellular components of the late-phase reaction were evaluated in both groups of mice. RESULTS: All parameters of clinical symptoms were markedly inhibited after intraperitoneal injection of ISS-ODN, whereas topical application to the conjunctiva did not inhibit clinical symptoms significantly. Remarkably, a single topical treatment with ISS-ODN (as well as by intraperitoneal injection) completely inhibited both eosinophilia and neutrophilia in the late-phase reaction. CONCLUSIONS: Systemic or conjunctival administration of ISS-ODN was shown to significantly inhibit allergic responses in this mouse model. This indicates that ISS-ODN may be an effective form of immunotherapy for this class of allergic disease.

Evaluation of the mechanical alignment of a linear accelerator with an electronic portal imaging device (EPID).

Mechanical misalignment of a medical linear accelerator can be caused by any combination of source position displacement relative to the collimator rotational axis, collimator jaw asymmetry, or when the rotational axes of the gantry and the collimator do not intersect. A test procedure sensitive to all of these problems has been developed using an Electronic Portal Imaging Device (EPID). Each marker is placed on top of the EPID housing and on the treatment couch, then two images are acquired for gantry positions 180 degrees apart. By comparing the positions of the markers and their distances to the beam centre, mechanical alignment of the linear accelerator can be assessed. By comparing the positions of the beam centre for another two images acquired at collimator angles 180 degrees apart, the three potential sources of misalignment can be distinguished. Results with Siemens' Beam View were presented and determination accuracy of better than 0.25 mm can be achieved.

[Adaptation of diaphragm muscle strip mechanics to chronic electrical stimulation and the effect of change in extracellular Ca2+ in rabbit].

AIM: The mechanical character of diaphragm muscle after chronic electrical stimulation(CES) and effect of the extracellular Ca2+ change have not yet been explored. We wondered whether there might be great different change and effect on muscle mechanics after CES and the extracellular Ca2+ change. METHODS: The twitch tension(Pt), time to peak tension(TPT), half-relaxation time(1/2 RT), tetanic tension(Po), fatigue index (FI) and fatigue recovery index(FRI) were respectively measured in normal group and CES groups; the switch tensions of diaphragm muscle strips were observed in the standard Hank's solution and the Hank's with free Ca2+. RESULTS: There were more significant decrease in Pt, Po, FI and FRI, more significant lengthening in TPT and 1/2 RT in 10 Hz and 20 Hz groups(P < 0.01). However, there was completely opposite effect in 50 Hz and 100 Hz groups. There were more significant effect on muscle mechanics of contraction and relaxation in 10 Hz and 20 Hz groups than that in 50 Hz and 100 Hz groups when the extracellular Ca2+ changed. CONCLUSION: After CES the significantly frequency dependent were presented on mechanical character of diaphragm muscle strips, and there were more effect on diaphragm muscle mechanics in 10 Hz and 20 Hz groups when the extracellular Ca2+ was changed.

[An analysis of precision and accuracy of the phonocardiogram exercise test].

Phonocardiogram exercise testing (PCGET) is a recently developed method to evaluate cardiac contractility and the cardiac reserve of patients with heart disease and of healthy subjects. In order to test the reliability of PCGET method, the present author conducted a study on its precision and accuracy. Thirty volunteers underwent PCGET. When different examiners measured the S1 amplitude in the same cardiac cycle of the same subject, the data obtained by examiner A were: x +/- s = 5.05 +/- 0.0451; the data obtained by examiner B were: x +/- s = 4.95 +/- 0.0346, F = 1.699, P > 0.05. When different examiners measured the same cardiac cycle of the same subject, the data obtained by examiner A were: x +/- s = 0.789 +/- 0.0018; the data obtained by examiner B were: x +/- s = 0.787 +/- 0.0017, F = 1.167, P > 0.05. The results suggest that PCGET is a nonivasive, convenient, and inexpensive technique to quantitatively evaluate cardiac reserve for abnormal or normal persons.

[The effect of coxsackie virus B3 on intracellular calcium homeostasis in rat cardiomyocytes].

OBJECTIVE: To investigate the effect of coxsackievirus B3 (CVB3) on intracellular calcium, L type voltage dependent calcium channel (VDCC) and Na+/Ca2+ exchange (NCE) in adult rat cardiomyocytes. METHODS: Rat hearts were perfused with collagenase to gain single cardiomyocyte. The intracellular free calcium concentration of the cardiomyocytes were detected by laser confocal microscopy with calcium indicator, Fluo3-acetoxymethyl ester; and the currents of VDCC, Na+ and NCE were recorded by patch clamp techniques. RESULTS: The free intracellular calcium concentration increased from (51 +/- 43) nmo1/L in normal cardiomyocytes to (339 +/- 208) nmo1/L in CVB3 infected cardiomyocytes. The VDCC current increased from (4.13 +/- 0.75) pA/pF to (4.9 +/- 1.4) pA/pF after CVB3 infection, and the inward NEC current decreased from (-0.46 +/- 0.21) pA/pF to (-0.28 +/- 0.16) pA/pF; besides, the reverse potential of NCE was shifted from (29.4 +/- 7.0) mV to (41 +/- 14) mV with CVB3 infection. CONCLUSIONS: The increase of L type VDCC current and decrease of inward NCE current with shift of NCE reverse potential may partly attribute to intracellular free calcium accumulation; also, it may be the cause of cell damage and the occurrence of abnormal electric activities induced by CVB3 infection.

[Autogenous costochondral graft applied in the reconstruction of the temporomandibular joint].

OBJECTIVE: To study the surgical treatment of ankylosis of the temporomandibular joint. METHODS: Autogenous costochondral grafts were applied in the reconstruction of the temporomandibular joint in 27 cases with condyle ankylosis from 1988-1998. The follow up period was 1-10 years. The pre- and post-operative degree of mouth opening and the TMJ function was examined. RESULTS: Postoperative mouth opening was improved up to 2 cm in the 27 cases. All the results were successful without recrudescence of condyle ankylosis. CONCLUSION: The use of autogenous costochondral grafts in TMJ reconstruction is the best method in the treatment of TMJ ankylosis.

[Numerical dynamics of sediment bacteria in shrimp polycultural ecosystems].

The numerical dynamics of sediment bacteria was studied from May to September 1997, in the Shrimp Farm of Huanghai Fisheries Group Corporation with five experimental enclosures. It shows that the numbers of sediment bacteria ranged from 4.10 x 10(8) to 12.36 x 10(8) cells.g-1, averaged 8.09 x 10(8) +/- 2.17 x 10(8) cells.g-1, and increased gradually with increasing cultural time. The maximum was in 0-1 cm layer, and decreased obviously bellow 2-3 cm layer.

[Effect of cutting on vegetation composition and soil properties].

Different cutting experiments were executed on well-grown natural Imperata cylindrica vegetation in red soil region. The biomass of every 6 years cutting was the highest, and the annual cut was the lowest. Annual cutting resulted in the highest total accumulative biomass in 6 years, with a total production of 399.1 kg per plot. The species composition was also influenced by the length of cutting period. In the 6th year of experiment, the xylophyta biomass in the treatment of every 6 years cutting accounted for 41.5% of the total biomass, while the herb biomass in the treatment of annual cutting was 99.0% of the total. The length of cutting period also influenced the layer's development of soil profile and the physicochemical properties of soil. In comparing with the treatment of annual cutting, the soil bulk density in the treatment of every 6 years cutting was lower, and the contents of soil organic matter, total nitrogen, available phosphorus and available potassium were higher.

[Approximate entropy and its application in heart rate variability analysis].

Objective. To introduce approximate entropy in the analysis of heart rate variability, and to discuss the changes of the complexity of cardiovascular system during orthostatic standing posture. Method. ECG of 8 subjects were recorded during supine and orthostatic standing postures. Approximate entropy was used to analyze HRV. Result. Compared to supine before orthostatic standing posture, R-R intervals at 0-5 min, 5-10 min, 10-15 min and 15-20 min during orthostatic standing posture were decreased significantly, while approximate entropy at 0-5 min and 15-20 min decreased significantly. It showed that the complexity of cardiovascular system was decreased and the modulation of cardiovascular system changed. Conclusion. It is feasible that approximate entropy be used for heart rate variability analysis.

Transcatheter closure of secundum atrial septal defects using Amplatzer device.

OBJECTIVE: To evaluate the feasibility and short term results of transcatheter closure of secundum atrial septal defects (TCSASDs) using Amplatzer occluder device. A new self-centering device, the Amplatzer atrial septal occluder, has recently been evaluated in an animal model with excellent results. This paper reports our clinical research with this device. METHODS: 30 patients underwent an attempt at TCSASDs at an average age of 35.6 +/- 15.9 y (range, 5-62 y) and average weight of 63.6 +/- 14.5 kg (range, 17-78 kg) using the Amplatzer atrial septal occluder. The average ASD diameter measured by transesophageal echocardiography (TEE) was 23.6 +/- 4.0 mm (range, 16-31 mm), and the average atrial septal defects (ASD) balloon stretched diameter was 25.2 +/- 5.8 mm (range, 9-34 mm). All patients had right atrial and ventricular volume overload with a Qp/Qs of 3.4 +/- 1.2 (1.4-5.2). A F8-12 catheter was used for delivery of the device in all patients. RESULTS: The device was placed correctly in all patients. There was immediate and complete closure in 30 patients. The average device waist size used to close the defect was 25.6 +/- 5.9 mm (range, 9-34 mm). There was no episode of device embolization or any other complication. Follow-up was performed using transthoracic echocardiography (TTE) 1 day, 1 month, 3 months, 6 months and 1 year after the procedure. The first day after operation; there was complete closure of the ASD in all patients; there have been no episodes of endocarditis, thromboembolism, or wire fracture. CONCLUSION: TCSASDs using the Amplatzer occluder device was an efficient nonsurgical method which had a very high technical success rate and satisfactory short term and follow-up results.

[Clinical study in treating type 2 diabetes mellitus according to liver in TCM].

OBJECTIVE: To explore the clinical effect of Danzhi Xiaoyao powder (DZXY) in treating type 2 diabetes mellitus (DM) according to Liver in TCM. METHODS: Fifty-one type 2 DM patients were randomly divided into the treated group and the control group, 31 patients of the treated group were treated with conventional and DZXY therapy, while 20 patients in the control were treated with conventional therapy alone. Before and after treatment for 3 months, the levels of fasting blood glucose (FBG), fasting insulin, glycosylated hemoglobin (HbA1c), blood cholesterol (CH) and triglycerides (TG) were examined in all the patients. RESULTS: By the end of 3 months, the effect in treating type 2 DM in the treated group was better than that in the control group (P < 0.05). Compared with those before treatment in each group and those of the control, the levels of FBG, fasting insulin, HbA1c after DZXY treatment were reduced (P < 0.05), but levels of CH and TG in all groups had no significant difference. CONCLUSION: DZXY could soothe the Liver to relieve depression, which is effective in treating type 2 DM and could improve the symptoms of DM patients.

[Clinical and experimental study on yun'an granule in treating threatened abortion].

OBJECTIVE: To observe the effect of Yun'an Granule (YAG) on threatened abortion. METHODS: Five hundred and eighty patients were divided randomly into the treatment group and the control group. The effect of YAG in treating 580 patients was observed by double-blinded method and compared with that treated with progesterone (50 cases) or Shoutai Pill (50 cases) in the control group. And experimental study was conducted also. RESULTS: Clinical results showed that the therapeutic effective rate in YAG (96.9%) was obviously higher than that in the control group (56.0% for progesterone and 86.0% for Shoutai pill), the difference between them was significant (P < 0.05). Experimental results also showed that the YAG had fetus protective effect on the acupuncture or oxytocin induced threatened abortion model in rats, which was better than that of progesterone (P < 0.05). The mean weight of baby rats and placenta delivered by the mother rats which taken YAG were heavier than those which were untreated or treated by progesterone (P < 0.05). In addition, YAG could inhibit the normal or oxytocin stimulated contraction of rat's isolated uterus. CONCLUSION: YAG has good effect in treating threatened abortion and without any adverse effect on fetus.
