Changes in health over time in patients with symptoms allegedly caused by their dental restorative materials.

OBJECTIVES: In Sweden, many patients with symptoms allegedly caused by their dental materials have exchanged their restorations, but the effects of the exchange have been insufficiently investigated. Therefore, the aim of the study was to describe the change in health over time for these patients and the hypothesis was that the patients could be divided based on their symptoms and that the ability to recover differs between these groups. Furthermore, we also examined if other factors such as replacement of dental restorative materials and follow-up time had any impact on the perceived health status. METHODS: A questionnaire was sent to 614 patients who had been referred to the School of Dentistry, Umeå, Sweden, with symptoms allegedly caused by dental restorative materials. The response rate was 55%. RESULTS: The risk of having any further complaints was higher for patients with complex symptoms (P = 0.03) and these patients had exchanged their restorations to a significantly larger extent than the others (P = 0.03). The remaining complaints was more frequent among men (P = 0.02). Exchange of dental restorative materials had no significant impact on the ability to recover completely. However, the patients who had exchanged their restorations completely perceived a significantly larger alleviation of their symptoms than the others (P < 0.01), although the frequency of most of the symptoms had increased. CONCLUSIONS: Patients with complex symptoms had a more unfavorable long-term prognosis concerning persistent complaints than those with localized symptoms only. Furthermore, the results indicate that the patients might experience health improvements after removal of their dental restorative materials. The reason for this improvement, however, is unclear. Further analyses regarding other possible explanations than the 'odontological/medical' are needed.

Coping and self-image in patients with visual display terminal-related skin symptoms and perceived hypersensitivity to electricity.

OBJECTIVES: The aim of the present study was to measure coping resources and self-image in patients with visual display terminal (VDT)-related skin symptoms and hypersensitivity to electricity (HE). METHODS: From 1980 to 1998, 350 patients with electrical sensitivity were registered. The patients were subdivided into two groups: patients with skin symptoms evoked by VDTs, television screens, and fluorescent-light tubes and patients with so-called hypersensitivity to electricity with multiple symptoms evoked by exposure to different electrical environments. A questionnaire was sent to all patients and contained the coping resources inventory (CRI) and the structural analysis of social behaviour (SASB) in order for us to measure coping resources and self-image, respectively. The CRI and SASB scores were compared with those of control groups. Two hundred and fifty respondents (73%) returned the questionnaire, 200 (78.5% women) in the VDT group and 50 (62% women) in the HE group. RESULTS: The patient group rated high on the CRI spiritual/philosophical scale and high on the SASB spontaneous, positive and negative clusters but low on the controlled cluster. The female patients scored high on the CRI emotional scale. The VDT group rated lower than the controls on the SASB controlled cluster and higher on both the positive and negative cluster. The HE group scored higher than the control group on the SASB spontaneous and positive clusters. The women in the HE group scored higher on the CRI cognitive and CRI total scale than the VDT group and control group and higher on the CRI emotional scale than the controls. The women in the HE group rated higher than both the women in the VDT and control groups on the SASB spontaneous and positive clusters. CONCLUSIONS: The deviant self-image found in these patients, especially the female HE patients, support the view that VDT and HE symptoms can be stress related. In the clinic, a trustful alliance should be established with the patient in order for a more realistic view to be achieved of the capacity.

Terpene exposure and respiratory effects among workers in Swedish joinery shops.

OBJECTIVES: Exposure to monoterpenes (alpha-pinene, beta-pinene and delta 3-carene) in joinery shops was studied in Sweden during the processing of Scot's pine, and the acute respiratory effects among the employees were evaluated. METHODS: A cross-sectional study of 38 workers was carried out in 4 joinery shops. The investigation included personal air sampling of monoterpenes, biological monitoring of metabolites of alpha-pinene in the workers' urine, interviews following a standardized questionnaire, and dynamic spirometry. RESULTS: The personal exposure to monoterpenes in the joinery shops was 10-214 mg/m3. The correlation (correlation coefficient = 0.69) between exposure to alpha-pinene and verbenols (metabolites from alpha-pinene) in urine was relatively good. No acute effects on forced vital capacity or forced expiratory volume during 1 s were detected. The workers had significantly reduced preshift lung function values when compared with the values of a local reference group, even when smokers and ex-smokers were excluded. CONCLUSIONS: Personal exposure to the monoterpenes alpha-pinene, and delta 3-carene in joinery shops may exceed the present Swedish occupational exposure limit of 150 mg/m3 during the winter season when workroom air is commonly recirculated. The determination of metabolites of alpha-pinene (verbenols) in urine can be used as an index of exposure to fumes released during wood-treating processes. The results from the lung function tests indicate chronic rather than acute reactions in the airways. The fact that there were no major changes in lung function over a workshift indicates chronic reaction in the airways.

Human-microcomputer relations in an integrated laboratory service: the users' perspective.

The Motala Clinical Chemical Laboratory includes both central (hospital) and peripheral primary health care units. A laboratory computer system has been developed that is shared by all units. In order to illuminate the crucial human-computer interaction, the authors did a questionnaire study of the attitudes and experiences of the whole laboratory staff. The null hypothesis of the study-that computerizing is a threat to the users-was rejected. The quality of the laboratory work and reports was also felt to be improved.

A comparative study of Danish (Statens Seruminstitut), Glaxo and Behringwerke vaccines--revaccination of schoolchildren.

Three BCG vaccines were studied for their capacity to evoke tuberculin reactivity and local reactions at revaccination. The vaccines were produced by Statens Seruminstitut in Denmark (vaccine based on the Danish strain 1331), by the Glaxo Laboratories in England (vaccine based on the British strain 1077) and by the Behringwerke in the Federal Republic of Germany (vaccine based on the Danish strain 1331). The study comprised three independent trials (in Gothenburg, Umeå-Skellefteå and Stockholm) in children aged 14-15 years who had been vaccinated with BCG at birth. A total of 2997 children were vaccinated. In the Gothenburg areas tuberculin tests (2 TU PPD RT23) were performed 6-7 weeks after vaccination. Of the children vaccinated with the Danish vaccine 98% developed a tuberculin induration of greater than 6 mm. The mean size of induration was 11.4 mm. The corresponding figures for the Glaxo vaccine were 98% and 13.4 mm, and for the Behringwerke vaccine 76% and 8.0 mm. Open lesions 6-7 weeks after vaccination were reported in 4% of the children vaccinated with Danish vaccine (mean diameter of lesion 4.0 mm) and in 8% vaccinated with the Glaxo vaccine (mean diameter of lesion 4.6 mm) while none vaccinated with the Behringwerke vaccine showed open lesions. On the basis of the results obtained in our investigation as well as in other studies reported in the BCG literature, the authors concluded that the Danish and the Glaxo vaccines at present appear suitable vaccines for use in Sweden.
