Primary malignant lymphoma of the cervix in pregnancy. A case report.

BACKGROUND: Malignant lymphoma arising from the uterine cervix is a very rare entity. Only two such patients have been reported as pregnant at the time of diagnosis. CASE: A 35-year-old woman (negative Pap smear at antenatal clinics) was referred because of the accidental finding of a huge cervical mass during labor. The patient underwent cesarean section because of arrest of cervical dilatation and persistent floating of the fetal head. The final diagnosis of this cervical mass was malignant lymphoma, low grade B cell, after radical abdominal hysterectomy. CONCLUSION: Although labor obstructed by a tumor of the pelvic organs is a relatively rare event and the majority of cases are benign leiomyomas of the uterus or cervix, the risk of pelvic malignancies should be considered. Bimanual examination and pelvic ultrasound and/or color Doppler ultrasound should be applied without hesitancy in any uncertain situation during pregnancy or labor.

Enhanced efficacy of the combination of all-trans-retinoic acid and gamma-interferon in inhibiting proliferation of Tca8113 cells.

OBJECTIVE: To develop a more effective induced-differentiation or -apoptosis therapy for treating oral squamous cell carcinomas. METHODS: The combination of all trans retinoic acid (ATRA) and interferon-gamma (IFN-gamma) was tested for its antiproliferative activity against human tongue squamous cell carcinoma cell line (Tca8113 cells) in vitro. Clonogenic assay, MTT assay, and microscopic cell analysis were performed in this experiment. RESULTS: The results show that the combination of ATRA and IFN-gamma can produce synergistic antitumor effects, resulting from induction of apoptosis, on Tca8113 cells. CONCLUSION: The combination of ATRA and IFN-gamma is a more effective induced-apoptosis therapy for treating oral squamous cell carcinoma.

[Likelihood ratio and ROC curve in evaluation of iron parameters for diagnosing iron deficiency].

OBJECTIVE: To evaluate the value of iron parameters in diagnosing iron deficiency (ID). METHODS: Cross-sectional study was performed for diagnostic tests. Ninety consecutive patients with anemia including iron deficiency anemia without chronic diseases (36 cases), chronic diseases (54) were divided into chronic diseases without ID(ACD) (23 cases) and chronic diseases with ID(CDID) (31) by bone marrow iron staining. The exclusion criteria included hemolytic anemia, deficiency of Vitamin B12 or folic acid, blood transfusion, taking iron preparations within one month and hematological malignancies. By using absence of bone marrow iron as gold standard of ID, we compared the diagnostic powers in the diagnosis of iron deficiency for sTfR, serum iron, total iron binding capacity, serum ferritin (SF), transferrin saturation and the presence of hypochromic red cells by analyzing the likelihood ratio, the area under ROC(AUCROC). RESULTS: AUCROC for sTfR in determining ID in groups IDA + CDID vs. ACD, IDA vs. ACD and CDID vs. ACD were 0.9 (95% CI: 0.82-0.98), 0.96(0.9-0.99) and 0.84 (0.72-0.96) respectively, AUCROC for SF in above groups 0.87 (95% CI: 0.77-0.87), 0.94(0.86-0.99) and 0.77(0.63-0.91) respectively. AUCROC for other iron parameters in determining iron deficiency in chronic diseases was lower than 70%. CONCLUSION: ROC and LR are useful tools for evaluating iron parameters in diagnosing iron deficiency. TfR is a best parameter in determining iron deficiency in chronic diseases than SF and other iron parameters.

[Evaluation of clinical value of technetium-99m(V) dimercaptosuccinic acid SPECT in diagnosis of parotid lump].

OBJECTIVE: To evaluate the clinical value of single photon emission computed tomography (SPECT) on classifying parotid lump as benign or malignant before treatment. METHODS: SPECT images of the parotid region were obtained in forty-five patients with parotid lump before surgery. We used technetium-99m(V) dimercaptosuccinic acid as imaging agent. The results were compared with their pathological diagnoses, clinical examination, and ultrasonography. RESULTS: The sensitivity, specificity and accuracy of Tc-99m(V)-DMSA SPECT imaging in determining malignancy of parotid lump were 93.7%, 72.4%, 80.0%, respectively. The uptake ratios of parotid lesions were higher in malignant tumors than benign tumors, cysts, and inflammatory lumps (P < 0.05). CONCLUSION: It is suggested that Tc-99m (V)-DMSA SPECT can be a useful aided method of preoperative diagnosis of parotid lumps. The uptake ratios can help increase the accuracy of diagnoses. For the benign tumors and inflammatory masses of parotid gland, the diagnoses had some false positives which may be reduced by combined clinical examination and ultrasonography.

[An analysis of prognostic factors for acute leukemia patients with 5-year long-term survival in Shanghai].

OBJECTIVE: To sum up the long-term survival rate and prognostic factors in acute leukemia in Shanghai. METHODS: Retrospective cohort study and survival analysis. RESULTS: In the recent decade, 1,028 patients with acute leukemia were enrolled in Shanghai. One hundred fifty patients had 5-year long-term survival. The 5-year long-term survival rate was 18.6% for all the cases, being 13.3% for acute myeloid leukemia and 28.1% for acute lymphocytic leukemia. Children, lower white blood cell count, higher blood platelet count, lower percentage of blast cell in peripheral blood and no DIC and FAB-M(3) were protective factors for long-term survival. The remission induction regimens such as DA (daunorubicin and cytarabine) and HA (homoharringtonine and cytarabine) had no relation to long-term survival. There was no difference between the median survival time of the group with chemotherapy time less than 3 years and the group more than 3 years for acute myeloid leukemia. However, the median survival time was shorter in the group with chemotherapy time less than 3 years for acute lymphocytic leukemia. CONCLUSIONS: The 5-year survival rate for acute leukemia was 18.6%. L(1) and M(3) subtype had better prognosis. The following prognostic factors were highly associated with long-term survival: age, white blood cell count, blood platelet count, blast cell proportion in peripheral blood, DIC and FAB classification at the time of diagnosis. DA and HA regimen had the same effect. The total chemotherapy time was recommended to be 3 years for acute myeloid leukemia and 3 - 5 years for acute lymphocytic leukemia.

[Re-assessment of indicators for screening lead poisoning].

OBJECTIVE: To determine the accuracy of the indicators for detecting lead poisoning. METHODS: Blood and urine specimens in 157 workers exposed to lead were collected. Their blood levels of lead, free erythrocyte protoporphyrin (FEP), zinc protoporphyrin (ZPP), urine delta-aminolevulinic acid and activity of red cell pyrimidine 5'-nucleotidase were determined, with gold standards of blood lead levels of >or= 1.93 micromol/L and >or= 2.90 micromol/L. Accuracy assessment of the indicators mentioned above for detecting lead poisoning was based on the area under the receiver operative characteristic (AUC(ROC)) curve with a software specially for ROC curve analysis and software of Epi-Info, after determining the optimal cut-off points for the above mentioned indicators in detecting those with blood lead >or= 1.93 micromol/L and >or= 2.90 micromol/L. The sensitivity of screening in parallel for detecting those with blood lead >or= 1.93 micromol/L and >or= 2.90 micromol/L was calculated and compared. RESULTS: For detecting those with blood lead level >or= 1.93 micromol/L, the area under ROC curve (AUC(ROC)) of above indicators was 0.978 for ZPP, 0.973 for red cell P5'N, 0.937 for FEP, 0.890 for urine delta-ALA (significantly different from that for ZPP with P < 0.05), and 0.845 for urine lead level (significantly different from that for ZPP with P < 0.05), respectively. For detecting those with blood lead level >or= 2.90 micromol/L, the area under ROC curve (AUC(ROC)) of above indicators was 0.975 for red cell P5'N, 0.954 for ZPP, 0.906 for FEP (significantly different from that for red cell P5'N with P < 0.05), 0.900 for urine delta-ALA (significantly different from that for red cell P5'N with P < 0.05), and 0.750 for urine lead level (significantly different from that for red cell P5'N with P < 0.05). Sensitivity of screening in parallel with ZPP and P5'N was significantly higher than that with urine levels of lead and delta-ALA. CONCLUSION: Determinations of red cell P5'N and ZPP for detecting those with blood lead >or= 1.93 micromol/L and >or= 2.90 micromol/L are more accurate.

[Studies on variations of alkaloids in zhebeimu (Fritillaria thunbergii Miq.) by improved colormetric determination].

OBJECTIVE: To find out the seasonal variation of active alkaloids in Zhebeimu(F. thunbergii) collected from Zhejiang Ninbo and Jiangsu Nantong, and confirm the suitable harvesting time for the crude drug. METHOD: Determining the alkaloid contents in the samples of Zebeimu collected monthly from the above-said twoplaces by improved colormetric method. RESULT: The alkaloid accumulation patterns of the samples from two places are very similar, with the highest contents appearing in samples collected in January of February, and the absolute amounts in samples harvested in May. CONCLUSION: The optimum time for harvesting Zhebeimu is in May.

Enantiomeric inhibitors of cholesterol esterase and acetylcholinesterase.

Enantiomers of N-methyl-N,alpha-methylbenzylbutyramide (1), 1-butyl-3-methyl-3'-alpha-methylbenzylurea (2), 1,2,3, 4-tetrahydro-1-naphthyl-N-butylcarbamate (3), 1,1'-bi-2-naphthyl-2, 2'-di-N-butylcarbamate (4), 1, 1'-bi-2-naphthyl-2-ol-2'-N-butylcarbamate (5), and 1, 1'-bi-2-naphthyl-2-butyrate-2'-N-butylcarbamate (6) are inhibitors of porcine pancreatic cholesterol esterase-catalyzed hydrolysis of 4-nitrophenyl butyrate and of electric eel acetylcholinesterase-catalyzed hydrolysis of acetylthiocholine in the presence of 5,5'-dithiobis-2-nitrobenzoate. For competitive inhibitors, values of the inhibition constant (Ki) and the enantiomeric ratio (Ecomp.) are investigated. For active site-directed irreversible inhibitors, values of the inhibition constant (Ki), the carbamylation constant (k2), the bimolecular rate constant (ki), and the enantiomeric ratio (E) are investigated. Toward both enzymes, compounds 1 are poor competitive inhibitors (Ki=102-104 microM) but have good enantioselectivities (Ecomp.=10-50, the preference for R). R-2 and S-2 are competitive inhibitors of acetylcholinesterase with Ki=26 and 80 microM, respectively (the preference for R) but are active site-directed irreversible inhibitors of cholesterol esterase with ki=4 and 16 M-1 sec-1, respectively (the preference for S). For those competitive inhibitions, both leaving group hydrophilic and hydrophobic binding sites of cholesterol esterase or both anionic substrate binding site and peripheral anionic binding site of acetylcholinesterase bind to N,N-methyl-alpha-methylbenzyl disubstituted amide parts of these inhibitors and the enzyme does not catalyze the hydrolysis of these inhibitors. The opposite stereopreference (S) for the inhibition of cholesterol esterase by compounds 2 may be due to the fact that N, N-methyl-alpha-methylbenzyl disubstituted amide parts of these inhibitors bind to the alkyl chain binding site of the enzyme. Compounds 3-6 are active site-directed irreversible inhibitors of cholesterol esterase (ki=1-13000 M-1 s-1) and peripheral anionic binding site-directed irreversible inhibitors of acetylcholinesterase (ki=1.7-1300 M-1 s-1). Compounds 3 have low enantioselectivities (E=1.3-1.4) for both enzymes. The stereopreference for atropisomers 4 and 6 is S-form toward both enzymes (E=2-30) and is identical to that of cholesterol esterase-catalyzed hydrolysis of 1,1'-bi-2-naphthyl-2,2'-diacylate. This stereopreference (S) may be due to the fact that the butyryl group or one of two butylcarbamate groups of S-atropisomers binds more effectively to the leaving group hydrophobic binding site of cholesterol esterase or the peripheral anionic binding site of acetylcholinesterase than that of R-atropisomers. The opposite stereopreference (R) for atropisomers 5 toward both enzymes may be due to a favorable interaction between the hydroxyl group of the inhibitors and the leaving group hydrophilic binding site of cholesterol esterase or the peripheral anionic binding site of acetylcholinesterase.

Conformationally restricted carbamate inhibitors of horse serum butyrylcholinesterase.

Conformationally restricted carbamate inhibitors, exo-2-norbornyl-N-butylcarbamate (1), endo-2-norbornyl-N-butylcarbamate (2), l-adamantyl-N-butylcarbamate (3), and 2-adamantyl-N-butylcarbamate (4) as active site-directed irreversible inhibitors of horse serum butyrylcholinesterase are investigated for values of the dissociation constant (KI), the carbamylation constant (k2), and the bimolecular rate constant (ki). Compound 1 is the most potent inhibitor of the enzyme and the values of KI and ki are 20 nM and 1.1 x 10(5) M-1sec-1, respectively.

Comparison of responses of tattoos to picosecond and nanosecond Q-switched neodymium: YAG lasers.

OBJECTIVE: To test the hypothesis that picosecond laser pulses are more effective than nanosecond domain pulses in clearing of tattoos. DESIGN: Intratattoo comparison trial of 2 laser treatment modalities. SETTING: A large interdisciplinary biomedical laser laboratory on the campus of a tertiary medical center. PATIENTS: Consecutive patients with black tattoos were enrolled; all 16 patients completed the study. INTERVENTION: We treated designated parts of the same tattoo with 35-picosecond and 10-nanosecond pulses from 2 neodymium:YAG lasers. Patients received a total of 4 treatments at 4-week intervals. All laser pulse parameters were held constant except pulse duration. Radiation exposure was 0.65 J/cm2 at the skin surface. Biopsies were performed for routine microscopic and electron microscopic analysis at the initial treatment session and 4 weeks after the final treatment in 8 consenting patients. Also, ink samples were irradiated in vitro. MAIN OUTCOME MEASURES: In vivo, on the completion of treatment, a panel of dermatologists not associated with the study (and blinded to the treatment type) evaluated photographs to assess tattoo lightening. Formalin-fixed specimens were examined for qualitative epidermal and dermal changes as well as depth of pigment alteration. Electron micrographs were examined for particle electron density and size changes (in vivo and in vitro). The gross in vitro optical density changes were measured. RESULTS: In 12 of 16 tattoos, there was significant lightening in the picosecond-treated areas compared with those treated with nanosecond pulses. Mean depth of pigment alteration was greater for picosecond pulses, but the difference was not significant. In vivo biopsy specimens showed similar electron-lucent changes for both pulse durations. In vitro results were similar for both pulse durations, showing increases in particle sizes and decreased electron density as well as gross ink lightening. CONCLUSIONS: Picosecond pulses are more efficient than nanosecond pulses in clearing black tattoos. Black tattoos clear principally by laser-induced changes in the intrinsic optical properties of the ink.

Determination of hepatotoxic pyrrolizidine alkaloids by on-line high performance liquid chromatography mass spectrometry with an electrospray interface.

The present study describes the determination of two different types of hepatotoxic pyrrolizidine alkaloids (PAs) and also distinguishing the hepatotoxic PAs from non-toxic ones by both in-source collision-induced dissociation high performance liquid chromatography mass spectrometry (CID-HPLC/MS) and HPLC/MS/MS (CID in the collision cell), using electrospray ionization. The mass spectra provided molecular ions and characteristic fragment ions, which could be used readily for a rapid identification of different types of PAs. Applications of both in-source CID-HPLC/MS and HPLC/MS/MS analytical methods were successful for the determination of PAs in blood samples obtained from rats dosed with PAs and in the PA-containing plant. The results demonstrated that the developed HPLC/MS methods with two different CID techniques provided a very simple and rapid analysis for an unequivocal diagnosis of PA poisoning and for definitive identification of PAs in plants or herbal medicines.

Glutathione transferase T1 and M1 genotype polymorphism in the normal population of Shanghai.

Glutathione transferases are known to be important enzymes in the metabolism of xenobiotics. In humans genetic polymorphisms have been reported for the hGSTM1 and hGSTT1 genes leading to individual differences in susceptibility towards toxic effects, such as cancer. This study describes the distribution of the two polymorphisms of hGSTT1 and hGSTM1 in the normal Chinese population of Shanghai. Out of 219 healthy individuals having been genotyped for GSTT1 and GSTM1, 108 (49%) were identified to be homozygously deficient for the GSTT1 gene and 107 (49%) for the GSTM1 gene.

High performance liquid chromatographic analysis of a novel aminoalkylpyridine anticonvulsant 2-(4-chlorophenyl)amino-2-(4-pyridyl)ethane.

A simple, rapid and specific high performance liquid chromatographic (HPLC) method for the quantitation of 2-(4-chlorophenyl)amino-2-(4-pyridyl)ethane (AAP-Cl) and identification of its putative metabolite, 2-(4-chlorophenyl)amino-2-(4-pyridyl)ethanol (beta-AA) in rat blood and urine has been developed. AAP-Cl, beta-AA and an appropriate internal standard were extracted from rat biofluids by a solid phase extraction technique using C18 cartridges prior to the HPLC analysis. The extractibility was 92% for AAP-Cl and 98% for beta-AA. The HPLC analysis employed a symmetrical or standard reversed-phase HPLC column (Apex ODS, 5 microm, 25 cm x 0.46 cm) for blood or urine analysis, a mobile phase of water methanol acetonitrile (40:30:30) containing 20 microl 100 ml(-1) diethylamine at a flow rate of 1 ml min(-1), and UV detection at 254 nm. The limit of detection was 100 ng ml(-1) for both analytes in both blood and urine. The calibration curves for AAP-Cl in rat biofluids were shown to be linear in both low and high concentration ranges (blood: 0-1 and 1-10 microg ml(-1); urine: 0-10 and 10-100 microg ml(-1)) with intra- and inter-day coefficients of variation of no more than 18% for blood and 14% for urine. The method developed was successfully applied to a preliminary analysis of intact AAP-Cl in both blood and urine obtained from rats dosed with AAP-Cl.

Trophoblast IFN-tau differentially induces lymphopenia and neutropenia in lambs.

Type I interferons (IFN), including IFN-alpha and IFN-beta, cause severe lymphopenia, resulting from altered lymphocyte recirculation and redistribution. IFN-tau, a product of trophectoderm of ruminant conceptuses and new member of the type I IFN family has not been examined for its effect on leukocyte recirculation. Additionally, differential effects of type I IFNs on the redistribution and recirculation of subsets of T cells have not been reported. The present study determined the effects of IFN-tau on the redistribution and recirculation of ovine leukocytes and T cell subsets. Total peripheral blood leukocytes, lymphocytes, and segmented neutrophils were reduced (p < 0.05) following treatment of lambs with IFN-tau. Furthermore, administration of IFN-tau caused an acute, differential reduction in peripheral blood CD4+ T cells (p < 0.05), CD5+ cells (p < 0.05), and gammadelta TCR+ (p < 0.01) T cells but had no effect on CD8+ T cells (p > 0.05). IFN-tau reduced the percentage of gammadelta T cells by 8-fold and that of CD4+ T cells and CD5+ cells by <2-fold in peripheral blood when compared with control lambs. The reduction in leukocytes, lymphocytes, and neutrophils was observed as early as 6-12 h after administration of IFN-tau, but levels returned to control values within 48 h. These results indicate that IFN-tau, like other members of the type I IFN family, can have immediate effects on leukocyte recirculation and redistribution. The present study is the first to demonstrate that IFN-tau differentially regulates T cell recirculation with the greatest effect on gammadelta TcR+ T cells.

A decision-support system for off-site nuclear emergencies.

In the case of a nuclear emergency, quick, well-founded decisions must be made about the type of protective action, its region of application, and initiation time. These typically are tasks for computer-based systems. Even with emergency-preparedness, exercises, and training, the decision-support system is one of great importance. This paper describes a decision-support system recently developed by the China Institute of Atomic Energy; it can optimally rank actions during the early phase of an accident using multiattribute utility analysis, and for the intermediate and later phases by cost-benefit analysis. This system runs both on MICRO VAX II and PC systems.

Activity of HMR 3647 compared to those of five agents against Haemophilus influenzae and moraxella catarrhalis by MIC determination and time-kill assay.

The microdilution MICs of HMR 3647, erythromycin A, azithromycin, clarithromycin, roxithromycin, and pristinamycin against 50/90% of 249 Haemophilus influenzae and 50 Moraxella catarrhalis isolates were 2/4, 0.06/0.125; 8/16, 0.25/0.25; 2/4, 0.06/0.125; 16/16, 0.25/0.25; 32/>32, 1/2; and 2/4, 0.5/0.5 microg/ml. Azithromycin was bactericidal against all 10 H. influenzae and 3 of 5 M. catarrhalis isolates and HMR 3647, erythromycin A, clarithromycin, roxithromycin, and pristinamycin were bacteriostatic, against all 15 strains after 24 h at the MIC.
