[Role of integrated imaging (computerized tomography and echography) in the surgical reconstruction of the external ear].

Three-dimensional Computed Tomography (3DCT) imaging methods of the skull and ribs by axial and contiguous slices allow the selective rendering of the involved structures. The possibilities given by variable threshold ranges allow 12-bit density structures (Hounsfield Units) to be obtained within threshold limits. Imaging has been optimized by shading solids. Such methods, together with US (duplex-Doppler) techniques, offer new diagnostic possibilities relative to the treatment planning of patients with either congenital malformations or acquired abnormalities of the external ear and in the evaluation of associated abnormalities of craniofacial areas. In the reconstruction of the earlap by autograft of modelled rib cartilage, after preparing an expanded otomastoid skin flap, the above methods allowed accurate preoperative planning with consequently significant reduction of surgical time and better clinical results. Six patients with earlap abnormalities (four with microtia due to complete hemifacial microsomia syndrome, one with anotia and one with post-traumatic loss of the earlap) were studied for reconstruction. High-quality 3DCT images of the rib and auricolar cartilage were obtained in real-scale representation (1:1 ratio) the evaluation of the donor site and of the autograft size. Useful pieces of information on the development and vascularization of an expanded otomastoid flap, after gradual and constant expansion, were given by duplex-Doppler US, which allowed easy monitoring in both surgical planning and follow-up.

[Technical principles of solid tridimensional modeling with computerized tomography for the study of maxillofacial diseases].

3DCT allows the solid modeling of body structures from contiguous slices. The 3D images are free to rotate on the x, y, and z axes. It is possible to evidentiate structures with various densities by means of threshold operators, which allow a 3D model of both soft tissues and bones to be obtained. Shading operations allow image quality to be improved by varying the elementary units to surface units ratio. Implemented 3D rendering reduces the spatial edges by means of anti-aliasing functions. The 3D images allow the study of the complexity of maxillofacial bony structures, and they are especially useful in both surgical planning and in postoperative follow-up. We studied 58 patients with maxillofacial diseases (34 traumatic, 14 malformations, 4 dysplastic, and 6 neoplastic). In most cases (96.5%), we obtained high-quality images, which allowed both the site and the extension of the lesions to be evaluated, together with their relationships to adjacent structures. In 65% of traumatic cases, the 4 basic views thoroughly demonstrated lesion spread, while in the extant 35% of cases cutting operations and rotatory translations were necessary. In all malformation cases a clear visualization of somatic asymmetries was obtained. In both dysplastic and neoplastic cases, the best lesion evidence was obtained in superficial lesions with cortical bone involvement. This technique was always easy and quick to perform, with no need for supplemental dose exposure to the patient.

Duplex-Doppler and cine-Bmode echography of the temporal-mandibular joint. Clinical validation.

We study, with the recently introduced method of analysis of the behavior of the TMJ through duplex-Doppler and cine-Bmode echography, a set of 38 patients already clinically examined. Among them we have 10 normal and 28 abnormal patients with various pathological conditions, 22 clicks of different intensities, three condylomeniscal lack of coordination and three locks. We obtained from the normal patients the typical duplex-Doppler spectra showing symmetrically alternate behavior and regular profiles during opening and closing of the mouth. In the case of clicks we obtained spectra with various irregularities of the profile and discontinuities in the worst cases. For the locks we observed a drastic and systematic reduction of the spectral amplitude.
