[Comparison of mortality rates between peritoneal dialysis and hemodialysis in diabetic nephropathy patients].

OBJECTIVE: To evaluate and compare the different survival rates and clinical outcome of continuous ambulatory peritoneal dialysis (PD) and hemodialysis (HD) treatment in diabetic patients. METHODS: 62 diabetic patients suffering from end stage renal complication were treated with PD or HD. The age, sex and residual renal function of the two groups were matching. Their survival rates and levels of blood lipid, blood sugar, serum albumin and hemoglobin were observed. RESULTS: The mortality rates of HD and PD were not significantly different in the first two years. But mortality of PD increased rapidly after two years of treatment. The levels of blood triglycerides, low density lipoprotein cholesterol and fasting sugar in PD patients were higher than that in HD patients, while the concentration of serum albumin was lower than HD patients significantly. CONCLUSIONS: In diabetic patients with renal failure PD as a choice to replace the kidney function was not superior to HD, especially after two years of PD treatment. A close watch is necessary to detect the change of parameter levels mentioned above and if is better to shift to HD. A strict long-term and extensive survey is needed to compare these two methods further.

Outcomes of pediatric pedestrian injuries by locations of event.

Data from the National Pediatric Trauma Registry October 1995-October 2000, containing medical records of children under 20 years old hospitalized for pedestrian injuries, were examined. Demographics and outcome measures (nature and severity of injury, utilization of resources, deaths, and disability at discharge) were compared by location of occurrence. Pediatric pedestrian injuries resulted in severe outcomes whether the events occurred in driveways, public places, or in the road. Off the road injuries accounted for a significant proportion (13.2%) of all serious pedestrian injuries and disproportionately affected the youngest children. Prevention should consider the child's age and the location of injury occurrence.

[Effect of manganese on the ratio of micronucleus cells in mice marrow].

The ratio of micronucleus in the bone marrow cell of mice induced by manganese (Mn2+, MnCl2) through intraperitoneal injection at doses of 0, 25, 50 and 100 mg/kg respectively was investigated. The number of micronucleus cells per 1000 polychromatic erythrocyte (PCE) from breast bone marrow was counted. The results showed that micronucleus ratio in bone marrow cells were 12.0 and 16.4 per 1000 in the medium and high dosage groups, which were higher than that of control (P < 0.01). There is no distinct difference between the low dosage group and the control group (P > 0.05), but show a rising trend with the dosage of Mn2+. It indicated that the higher the dose of MnCl2, the stronger the toxic effect to the micronucleus in bone marrow cells. MnCl2 is considered having mutation to chromosome and potential heredity toxicity.

[Expression of HLA-I antigens in primary hepatocellular carcinoma].

OBJECTIVE: To find out whether hepatocellular carcinoma (HCC) cells express HLA-I antigens. METHODS: LSAB assay of immunohistochemistry was used to detect six frozen HCC specimens. RESULTS: All the HCC cells have strong expression of HLA-I antigens. CONCLUSION: In view of these data on the expression of HLA-I antigens, the authors deem it practical to develop HCC CTL vaccine.

[Expression of type I collagen mRNA in rat molar periodontal ligament under normal bite force].

OBJECTIVE: The main component of the periodontal ligament(PDL) is collagen fiber, especially type I collagen, and collagen plays an important role in PDL. The aim of this study is to observe the expression of rat PDL mRNA under normal bite force and to investigate the molecular mechanisms of these changes in type I collagen mRNA. METHODS: 40 male Wistar rats were used, and animals were intra-cardiac perfused with a solution of 4% polyformaldehyde under anesthesia. Dissected mandibles were immersed in the same fixation for 6 hours and subsequently decalcified in EDTA. The demineralized specimens were embedded in paraffin and cut into slices with thickness 5 microns. The probe was synthesized and labeled with digoxigenin. Expression of type I collagen mRNA was measured by using in situ hybridization(ISH). RESULTS: Under the normal bite force, the mRNA expression of type I collagen was very strong on the whole, including the alveolar bone side, the root side and the area between them. Positive signals were located mainly in the cytoplasm and some in the nuclei. But the mRNA expression of type I collagen still had spatial characteristics. The signals in some fibroblasts were apparently stronger than those in other fibroblasts in the apical 1/3 fragment of the roots. The signal of type I collagen mRNA was strong near the root sides. The expression signal on the proximal alveolar walls was strong, however, on the distal alveolar wall, there was no expression. CONCLUSION: The expression of type I collagen mRNA is closely related with bite force.

[X-ray changes of temporomandibular joints after indirect trauma].

OBJECTIVE: The aim of this study is to investigate the structural changes of goat's temporomandibular joints (TMJs) after indirect trauma. METHODS: The trauma to the goat's TMJ achieved under an impact to the right and left mandibular angles using a self-made device. These goats were killed after 2 hours, 7 days, 1 month and 3 months. The goat's TMJs were examined using tomography before being killed, and also checked by using a histological method. The normal temporomandibular joints were treated as the control. RESULTS: 1. The X-ray tomograms: In the early post-trauma stage, the articular cavities were dimmed, and the joint spaces were decreased; the articular surfaces were not smooth, and the structures of TMJ were not clean. Moreover, the stations of the condyle were moved from middle of the fossa to the posterior fossa in the later stage. 2. The histological examination: The condyles exhibited eroded surfaces with osteophytes, the discs were thinned or perforated, and the temporal surfaces were thick-end and the linings were separated. CONCLUSION: The indirect trauma on TMJs of goats can cause the TMJ internal derangement and the osteoarthrosis of TMJs. The X-ray tomography is a very useful method in the long-term monitoring of bone changes in TMJs after trauma.

[Experiences of surgical treatment with cicatricial laryngotracheal atresia: report of 48 cases].

OBJECTIVE: In order to improve the treatment methods of laryngotracheal atresia, several improved surgical procedures were used. METHOD: From 1986, the 48 patients with laryngotracheal atresia accepted surgical treatment in our hospital. Laryngo-fissure with laser cauterization of the glottis, laryngotracheal reconstruction using surgical flaps with or without particles, laryngo-tracheal reconstruction using itself's cicatrices, tracheal end-to-end anastomosis, two-stage laryngotracheal reconstruction using aqueduct method, bronchotracheal reconstruction through the chest. RESULT: 46/48 cases were cured breathing without tracheal cannula. Following-up survey is 1-12 years. CONCLUSION: The treatment of cicatricial laryngotracheal atresia was difficult, but if the surgical methods were selected carefully according to the situation of each patient, we also can get a good result.

[A study on the breeding of new Ganoderma varieties by UV induced mutagenesis].

UV induced mutagenesis of Ganoderma's protoplast was carried out. By crude screen and careful screen, we obtained two mutants, 43020# and 43026$, whose dry weight and polysaccharides contents are higher than original strain. Results of continued cultivation for 10 generation on PDA slants and successive tests of shaking bottles, and successive 3 time pilot scale tests at 3 tons, showed that these two mutants are excellent strains with steady properties, high production and high content of polysaccharides. This study has provided a rapid and effective method for breeding of Ganoderma varieties which is suitable to industrial fermentation.

A new crystal form of nateglinide.

AIM: To find a new crystal structure of S-form in nateglinide [N-(trans-4-isopropylcyclohexylcarbonyl)-D-phenylalanine]. The XRD, IR patterns and data were given and the melting point was determined. METHODS: Phase analysis was performed by X-ray powder diffraction, IR and elemental analysis and the melting point was determined by differential scan calorimetry. RESULTS: S-form nateglinide is different from H-form or B-form. The melting point is 172.04 degrees C. CONCLUSION: The S-form nateglinide is a new crystal form.

[Study on hepatic-protective effect of "yinzhihuang" granula].

OBJECTIVE: To study hepatic-protective effect of "Yinzhihuang" granula. METHODS: By D-GalN, CCl4 induced acute hepatic damage model, ALT and AST in serum, pathological degree of hepar adopted as indexes, the potency of "Yinzhihuang" granula was observed and compared with "Yinzhihuang" oral solutions. Mice administrated ig 1-naphthyl-isothiocyanate, with bilirubin in serum used as index, the effect of "Yinzhihuang" granula was observed. RESULTS: 1.8 g/kg, 3.6 g/kg "Yinzhihuang" granula groups could decrease the rising of ALT, AST in mice serum induced by D-GalN.3.6 g/kg "Yinzhihuang" granula group could also improve pathological degree of hepar.1.0 g/kg, 2.0 g/kg "Yinzhihuang" granula groups could decrease the rising of ALT, AST in rats serum induced by CCl(4).2.0 g/kg "Yinzhihuang" granula group could also improve pathological degree of hepar.1.8 g/kg, 3.6 g/kg "Yinzhihuang" granula groups could decrease the rising of bilirubin in mice serum induced by 1-naphthyl-isothiocyanate. CONCLUSIONS: "Yinzhihuang" granula had an evident hepatic-protective effect on acute hepatic damage induced by D-GalN or CCl4, and could decrease the rising of bilirubin in mice serum induced by 1-naphthyl-isothiocyanate.

[Effect of decreased bite force on the temporal and spatial expression of type I collagen mRNA in rat molar periodontal ligament].

OBJECTIVE: The aim of this study was to observe the effect of decreased bite force on type I collagen mRNA in periodontal ligament (PDL) and probe into the molecular mechanism of the change in type I collagen mRNA. METHODS: Animal models were established by extracting left maxillary molars of rats. In situ hybridization was applied. RESULTS: The results showed that under decreased bite force, the expression level of type I collagen mRNA decreased at the sixth hour (49.7 +/- 11.1) (P < 0.05), reached the nadir on the second day (23.7 +/- 8.6), began to get over on the third day (43.1 +/- 10.3), and reached a relatively high level at the second week(56.3 +/- 9.8), but it was still lower than that of the normal bite force group (91.4 +/- 16.0)(P < 0.05); the expression of type I collagen mRNA near the cementum was higher than that near the alveolar bone. CONCLUSION: These results indicate that the expression level of type I collagen mRNA is closely related to bite force.

[Study on hypermethylation of the calcitonin gene in malignant hematological disorders].

OBJECTIVE: To investigate the clinical significance of hypermethylation of the calcitonin (CT) gene in malignant hematological disorders (MHD). METHODS: Polymerase chain reaction (PCR) in combination with digestion of DNA with Hpa II was used to examine the methylation patterns of the CT gene in 73 patients with MHD, 6 healthy people and 24 patients with nonmalignant hematological disorders (NON-MHD). RESULTS: Hypermethylation of the CT gene occurred in 12 of 14 (85.7%) patients with acute lymphoblastic leukemia (ALL), 9 of 15 (60%) acute nonlymphocytic leukemia (ANLL), 8 of 10 (80%) chronic myelogenous leukemia (CML), 5 of 15 (33.3%) malignant lymphoma (ML), 2 of 5 patients with myelodysplastic syndrome (MDS), 1 of 2 malignant histiocytosis (MH), 1 of 3 chronic lymphocytic leukemia (CLL) and 1 of 9 multiple myeloma (MM). However, it did not occur in normal controls and NON-MHD. CONCLUSION: Hypermethylation of CT gene may serve as an important molecular maker for tumorous cell clone and may be useful in diagnosing MHD, monitoring minimal residual disease and predicting prognosis.

[A multicenter randomized controlled clinical trial on lipids regulating effects of domestic simvastatin].

OBJECTIVE: This clinical trial was designed to assess the lipids regulating effects of domestic simvastatin (DS, produced by Chengdu Huayu Pharmaceutical Co.) in patients with hyperlipidemia. METHODS: 160 hyperlipidemic patients were randomly divided into 3 groups (A, B and C). Groups A and B were subjected to single-blind trial; group C was for open trial. Group A took DS 10 mg q.n., group B Zocor 10 mg q.n. and group C DS 10 mg q.n. respectively for 8 weeks. All the patients were followed up at the 4th week and 8th week. 155 patients finished the trial with 59 cases in group A, 47 cases in group B and 39 cases in group C. RESULTS: At the 4th week, serum total cholesterol (TC) in group A, B and C decreased by 16.88%, 19.23% and 14.10%; serum triglycerides (TG) decreased by 19.27%, 15.66% and 17.96%; HDL-C increased by 7.69%, 7.46% and 6.69%; and LDL-C decreased by 23.02%, 27.84% and 24.43%, respectively; there was no significant difference among the three groups (P > 0.05). At the 8th week, serum TC in groups A, B and C decreased by 25.03%, 26.53% and 25.22%. TG decreased by 23.85%, 24.74% and 24.75%; HDL-C increased by 9.23%, 8.95% and 8.39%; and LDL-C decreased by 33.72%, 35.50% and 30.99%, respectively; still, no significant difference among the three groups was observed (P > 0.05). The incidence rates of side effects in the three groups were similar. The clinical effects were more significant at the 8th week than at the 4th week for Zocor and DS. CONCLUSION: These data suggest that DS is as effective and safe as Zocor in clinical use for lipids regulating serum.

[Rapid construction of directional cDNA library from human nasopharynx].

OBJECTIVE: To construct a directional cDNA library from human adult nasopharynx by SMART (switching mechanism at 5' end of RNA transcript) technique. METHODS: The total RNA was separated from human adult nasopharynx epithelial tissue and the first-strand cDNA was synthesized through reverse transcription by a modified oligo(dT) primer(contained sfi IB site) while the SMART oligonucleotide(contained sfi IA site) was utilized as a template so that the first-strand cDNA could be extended over the 5' end of mRNA. The double-strand cDNA was amplified by LD-PCR(long-distance PCR) with the above two primers and then digested by sfi I (IA & IB) restriction enzyme. After cDNA size fractionation through CHROMA SPIN column, the double-strand cDNA was ligated into the sfi I-digested lambda TripIEx2 vector and then the recombinant DNA was packaged in vitro. RESULTS: The unamplified human adult nasopharynx cDNA library consists of 1.5 x 10(6) independent clones in which the percentage of recombinant clones is about 100%. The titer of the amplified cDNA library is 3.8 x 10(9) pfu/ml and the average exogenous inserts of the recombinants is 1.5 kb. CONCLUSION: These results shows that the human adult nasopharynx cDNA library has an excellent quality and lays solid foundation for screening and cloning new tumor suppressor genes of nasopharyngeal carcinoma(NPC) and tissue-specific genes of human nasopharynx.

[Significance of the unbalanced expression of Th1/Th2 type cytokines in human glioma].

OBJECTIVE: To study the significance of the unbalanced expression of Th1/Th2 type cytokines in human glioma. METHODS: The gene expressions of Th1/Th2 type cytokines in 62 specimens of human glioma tissues, 4 glioma cell lines, peripheral blood mononuclear cell (PBMC) of 15 glioma patients, 5 specimens of normal adult brain tissue and 5 brain meningioma tissues were detected by semiquantitative reverse transcription polymerase chain reaction. IFN-gamma and IL-2 represent Th1 type cytokines. IL-4, IL-6, IL-10 and IL-13 represent Th2 type cytokines. RESULTS: There were obviously predominant expression of Th2 type cytokines in glioma cell lines (P < 0.01) and specimens of human glioma tissues (P < 0.01). The tendency of distinct expression of Th2 type cytokines in PBMC was also existent. There wasn't obvious discrepancy of the expression of two type cytokines in normal adult brain tissues and meningioma tissues. CONCLUSIONS: It is likely that the switching of Th1/Th2 type cytokines in gliomas as predominant expression of Th2 type cytokine genes is related to the origination of gliomas and the evasion of glioma cells from immune surveillance.

[Influence of adjustment of balance of Th1/Th2 type cytokines on proliferation of glioma cells].

OBJECTIVE: To study the influence of adjustment of balance of Th1/Th2 by external cytokines on proliferation of glioma cells. METHODS: The gene expressions of Th1/Th2 type cytokines in C6, 9L, U251 and SHG44 glioma cells were detected by semiquantitative reverse transcription polymerase chain reaction (RT-PCR). After the cells were induced with IFN-gamma + IL-4 McAb and IL-4 + IFN-gamma McAb respectively, we isolated the total RNA to proceed RT-PCR again. The evaluation of cell proliferation was proceeded by MTT assay method. RESULTS: There was obviously predominant expression of Th2 type cytokines in glioma cell lines (P < 0.01). The expression intensity of IFN-gamma was improved in IFN-gamma + IL-4 McAb groups and Th2 type cytokines were enhanced in IL-4 + IFN-gamma McAb groups. IFN-gamma and IL-4 McAb could cause the switch from Th2 to Th1, and could remarkably inhibit the proliferation of glioma cells in a dose-dependent way (P < 0.01). On the other hand, IL-4 and IFN-gamma McAb could strengthen the switch of Th2, and might stimulate the glioma cell growth, also in a dose-dependent way (P < 0.01). CONCLUSIONS: There is a Th2 preponderance in glioma cells. IFN-gamma and IL-4 McAb could regulate the switch from Th2 to Th0 or Th1, and inhibit the proliferation of glioma cells.

[Analysis on the trace element and amino acid content in xinjiang 6 series Rhodiola L. plant].

Trace elements Na, K, Mg, Ca, Mn, Fe, Cu, Zn, Se and 20 kinds of amino acid in Xinjiang 6 series of Rhodiola's root and rootstalk were measured. The result shows that contents of 9 kinds of trace elements in them are different. They contain from 8 to 18 kinds amino acid, and also contain from 3 to 7 kinds of indispensable amino acid respectively. Rhodiola rosea L. contains most kinds of amino acid among six kinds of Rhodiola L. in Xingjiang, and contents of trace elements in it are also moderate, so it is fittest to be a nutrious chinese traditional and herbal drug of six kinds of Rhodiola L. in Xingjiang.
