Clinical trial of xipamide in the treatment of hypertension.

Nine patients with hypertension were given xipamide 40 mg orally as monotherapy on alternate days. An additional seventeen patients had xipamide added to an existing treatment regime of clonidine 450 micrograms per day. Xipamide alone produced a significant fall in blood pressure comparable to that obtained with clonidine alone. However, a significant further reduction was obtained when xipamide was added to the existing clonidine regime.

Human platelet aggregation by thimerosal. Functional and ultrastructural studies.

Thimerosal, a sulfhydryl group inhibitor, produces in an aggregometer a decrease in optical density of normal platelet-rich plasma over a wide range of concentrations. Ultrastructural study shows that the decrease of optical density produced by thimerosal at low doses is due to a true platelet aggregation preceded by a release reaction, whereas the aggregometric curves recorded after addition of thimerosal at high doses can be attributed to marked alterations of platelet morphology. Electron microscopic study shows the presence of electron-dense material between plasma membranes after addition of a low dose, and the early rupture of membranes after a high dose. These findings support previous conclusions that thimerosal binds to plasma membranes. Thimerosal induces a release reaction, seen in ultrastructural study and revealed by measurement of 14C-serotonin release. Moreover, thimerosal-induced aggregation is independent of released ADP and of formation of intermediates of the arachidonate pathway. Thimerosal-induced platelet aggregation is inhibited neither by ADP removal nor by aspirin addition.

Platelet heterogeneity. Relationship between buoyant density, size, lipid peroxidation and platelet age.

Human platelets were separated into 2 density populations by repeated centrifugations of platelet-rich plasma at increasing gravitational force. The heaviest platelet fraction was rich in larger platelets. The lightest platelet fraction was rich in smaller platelets. In both fractions and in the platelet button, lipid peroxidation (malonaldehyde-MDA-production after addition of thrombin) was measured at basal condition, on the 1st, 3rd, 5th, 7th and 9th day after aspirin ingestion. At basal conditions and after ingestion of aspirin, MDA production was higher in the heavy-large platelets than in light-small ones, but a parallel increase of MDA production was observed in the light and in the heavy population and in the platelet button. The data are not compatible with the hypothesis that platelet density and size are age-related. Aspirin inhibits platelet lipid peroxidation by permanently acetylating their cyclooxygenase and if the heaviest platelets were the young ones, lipid peroxidation should reappear sooner in them.

[Metaphyseal chondrodysplasia with atypical vertebral changes in a case of anorchism (author's transl)].

A case of metaphyseal chondrodysplasia with testicular dysgenesis, hair aplasia, hormonal and somatic features partially indicative of Noonan's syndrome, followed from 6 to 14 years, is described. The associated vertebral changes can be referred to same chondrodysplasic process; persistence of intermediate cartilage and precocious discal damage contribute in vertebral morphogenesis.

[Stenosis of the lumbar spinal canal and dolichophalangy in a case of dyschondrosteosis (author's transl)].

A case of dyschondrosteosis in a 13 years old boy, with multiple epiphyseal changes, atypical Madelung's deformity, stenosis of the lumbar spinal canal and dolichophalangy is described. Skeletal changes show manifold characters of resemblance with those encountered in Turner's syndrome. An osteogenetic defect affecting chiefly mesenchymal cells surrounding the epiphyseal plate is supposed. Stenosis of the lumbar spinal canal is not previously reported.

Disseminated intravascular coagulation in acute promyelocytic leukemia. Possibility of treatment with glucocorticoids at high doses.

Seven patients affected by acute promyelocytic leukemia and presenting a hemorrhagic syndrome with hypofibrinogenemia have been treated with Daunorubicin and high doses of Glucocorticoids. In three out of the seven patients we also added an antiprotease agent (Trasylol, Bayer). Hemorrhagic diathesis has been controlled in all patients but one without massive platelet transfusions. A complete hematologic remission with prolonged survival was achieved in five patients.

Circulating anticoagulant against factor XI and thrombocytopenia with platelet aggregation inhibition in systemic lupus erythematosus.

In a patient with systemic lupus erythematosus, anticoagulant activity directed against factor XI was found together with thrombocytopenia. In the serum globulin fraction, antiplatelet antibodies and an activity-inhibiting platelet aggregation could also be found. A possible correlation between the inhibition of platelet aggregation and the anticoagulant activity directed against factor XI is discussed.
