[Lymphomatoid granulomatosis with mesenteric localization. Nosologic problems (author's transl)].

The authors report the case of a 70-year-old woman with a mesenteric tumor, apparently isolated. Histologic examination led to the diagnosis of lymphomatoid granulomatosis. A review of the literature found four previously published cases of lymphomatoid granulomatosis with mesenteric localization, and with or without pulmonary involvement. The general features of this affection are recalled. Special attention is given to forms without pulmonary involvement (Liebow granulomatosis) as well as to their relationship with polymorphic reticulosis. Separation of these two disorders, which depended mainly on a strict distribution of the various lesions, seems questionable. The authors tend to consider that both conditions belong to the same nosologic group, sincea pathological features are similar and borderline cases exist.

Parathyroid carcinoma, adenoma and hyperplasia in a case of chronic renal insufficiency on dialysis.

A 62 year-old female patient with polycystic disease and chronic renal insufficiency who had required dialysis for 3 years, presented with diffuse but predominantly lumbar bone pain associated with lassitude. Radiologic studies showed diffuse osteoporosis and destruction of the 4th lumbar vertebra. Calcium levels were normal (2.30-2.50 mmoles) with increased PTH levels (1820 microliter Eq/ml). The presence of a cervical mass suggested a diagnosis of hyperparathyroidism. A cervical exploration was performed with ablation of 4 parathyroid glands and autotransplantation of a portion of the left inferior gland into the arm. Pathological studies showed the existence of carcinoma in the right superior and inferior glands, adenoma of the left superior gland and hyperplasia of the left inferior gland. The association of hyperplasia, adenoma and carcinoma could be explained by an initial hyperplasia of the 4 glands due to renal insufficiency, with subsequent transformation of 3 into adenomas of which 2 became cancerous. The patient's status was significantly improved following surgery.

[Morphologic study of different types of venous grafts. Correlation trial with parietal fibrinolytic activity].

In this experimental work, morphological aspects of different vein grafts were studied using both light microscopy, transmission electron microscopy and scanning electron microscopy. The correlation was made with their fibrinolytic activity ; 21 dogs were operated on under general anaesthesia ; "venous grafts" (perfusion of the vascular segments with venous blood) were performed by excision of the femoral vein with reanastomosis at the same site (protocol P1). "Arterialised grafts" (perfusion of a segment of vein with arterial blood at arterial pressure) were performed according to two protocols : P2 in which a segment of femoral artery was replaced with a segment of reversed femoral vein ; P3 in which a segment of femoral artery was by passed with the femoral vein left in situ after destroying the valves using a Fogarty balloon without damage of the intima. The factors affected by surgery are, therefore, disconnection of the vasa vasorum by excision of the vessel in protocols P1 and P2 and increased oxygenation and luminal pressure in protocols P2 and P3. In all the cases, endothelial cells retained the morphological characteristics of venous endothelium. However, the vein grafts in the arterial circulation were thickened and the fibrinolytic activity was changed. Pathogenic mechanisms of these changes are discussed.

Clear cell sarcoma of the kidney in children: a distinct entity.

Four cases of clear cell sarcoma of the kidney in children are reported. The specificity of the tumor is examined. The pathologic features, the clinical presentation, and the occurrence of bone metastasis are reasons to distinguish clear cell sarcoma from the Wilms' tumor group. The poor prognosis may be improved in the future by radical resection associated with polyvalent chemotherapy.

[Tetralogy of Fallot, congenital coronary aneurysms with the histology of periarteritis nodosa: nosologic problems].

Coronary lesions of periarteritis nodosa are detected in a newborn suffering from a Fallot's tetrad. The authors remind us of the present concept of infantile P.A.N. similar to adults' and of the notion of Kawasaki's syndrom that is almost the same as the infant's serious P.A.N. The reported case, original in many a detail, seems to represent the later expression monosymptomatic of Kawasaki's syndrom as if this one had been amputated at that stage of the greater part of its elements and then, casually developed in utero upon a congenital cardiopathy.
