[Prevalence of protein malnutrition on admission in internal medicine. Study of 260 hospitalized adults].

The nutritional protein status of 260 unselected adult patients admitted to an internal medicine hospital unit and of 60 healthy controls was evaluated by measurement of the brachial muscular perimeter and of the weight--height ratio and by assay of serum albumin, prealbumin, retinol-binding protein and transferrin. The values most frequently low were those of retinol-binding protein (66%), prealbumin (42%) and albumin (25%). The sensitivity of short half-life serum protein assays is underlined. Twenty-eight p.cent of patients were found to have protein malnutrition on admission, most of these being women and patients older than 60 years. In view of the scattering of most values in controls according to age and/or sex, it is suggested that individual reference values should be determined for this type of study and that no comparison should be made with data found in the literature.

[Antiperinuclear and antikeratin antibodies in liver diseases].

Antiperinuclear factor (APF) was detected in 7 out of 38 patients with autoimmune liver disease (AILD) and in 7 out of 83 patients with non-autoimmune liver disease (NAILD). Anti-keratin antibodies were found in 2 out of 18 patients with AILD and in 3 out of 32 patients with NAILD. Neither APF nor antikeratin antibody was significantly more frequent in AILD than in NAILD. The incidence of APF was shown to be greater (p less than 0.02) in patients with rheumatoid factor (RF) and/or antinuclear antibody. This supports the hypothesis that APF is connected with RF rather than with rheumatoid arthritis, although the presence of APF is evidence towards the diagnosis of rheumatoid arthritis.

Nutritional status in intensive care patients: evaluation in 84 unselected patients.

A prospective nutritional evaluation of 84 unselected patients admitted to a multidisciplinary ICU was performed using anthropometric measurements, creatinine height index (CHI), and serum protein assays. All values tested were significantly lower than those of 40 healthy controls. A matrix of correlation coefficients showed many similarities among the variables studied. Fatal outcome and poor prognosis indices, such as sepsis and renal failure, were analyzed separately. Anthropometric measurements and CHI were not different in the separate groups. Albumin (Alb) and transferrin (Tr) were not different in patients with or without sepsis. Retinol binding prealbumin was significantly higher in patients with renal failure. Thyroxin-binding prealbumin (TBPA) was significantly lower in all the groups. We emphasize the interest of this rapid turnover protein in evaluating nutritional status. We suggest: (1) a systematic nutritional assessment, and (2) an aggressive nutritional support in the ICU patient.

Critical study of staging in multiple myeloma.

130 patients with multiple myeloma were reviewed for a retrospective study of their classification according to Durie & Salmon. No significant difference was found in survival between patients in stages I, II and III (median survival 24.8, 32.1, and 17.3 months, respectively). Of the classical criteria affecting survival, only the Hb level showed any significant influence (P less than 0.04). In accordance with this finding, the survival time was much shorter (P less than 0.001) in patients with renal failure than in patients without (median survival 7.4 and 24.8 months, respectively); in addition, thrombopenia, fever, old age, and above all, the % of bone-marrow plasmocytes present, were shown to be decisive factors.

The incidence of Gm and Km allotypes in a group of Bretons, the Bigoudens.

Immunoglobulin allotypes were studied in a peculiar Breton community, the Bigoudens. Results showed that the incidence of the common Gm and Km phenotypes fell into the ranges quoted for Caucasian populations, except for the Gm1;..;5,10,11,13,14 and Gm1,2;..;5,10,11,13,14. The frequencies were found to be significantly different from those of non-Bigouden Breton neighbouring controls.

Plasma protein polymorphisms in a puzzling Breton community, the Bigoudens.

A total of 386 and 393 members of a Breton community, in France, the Bigoudens, have been tested for polymorphisms of C3 and haptoglobin (Hp) plus Transferrin (Tf) respectively. An interesting observation was a gap between the observed and expected phenotype C3 frequencies. The Hp and Tf gene frequencies were not found to be closer to the other Celtic peoples than to the French and English populations.

[Salivary lysozyme, immunoglobulins and beta 2-microglobulin in Gougerot-Sjögren syndrome].

Lysozyme (LZM), immunoglobulins and beta 2-microglobulin concentrations were measured in the saliva of 79 patients with primary (n = 29) or secondary (n = 50) Sjögren's syndrome and in a control population. beta 2-microglobulin and immunoglobulins A, G and M concentrations were higher, and LZM concentrations lower than in controls. A significant correlation was established between LZM and immunoglobulins, but no correlation was found between immunoglobulins and beta 2-microglobulin, nor between the latter and LZM. These results indicate that salivary LZM concentration may be of value in the diagnosis of Sjögren's syndrome.

[Treatment of urinary tract infections by dibekacin].

In this cooperative trial, 181 patients with various urinary tract infections were treated by dibekacin, a new hemi-synthetic aminoglycoside. Clinical and bacteriological results confirm the efficacy of dibekacin in this indication. Local and systemic tolerance, and thus renal and cochleo-vestibular, were very satisfactory.

Dibekacin↗

[Dibekacin in the treatment of septicemia].

During an open multicentric trial (17 centers), we have treated 62 septicemia by dibekacin, alone or associated with other antibacterial drugs. Taking into account the degree of severity of these patients, the results are considered satisfactory in 47 patients. 15 failures were noted (including 9 deaths). General and local tolerance were good, in spite of the duration of treatment.

[Muscular involvement in adult Still's disease. Apropos of 5 new cases].

Still disease was first described in adults ten years ago. Muscular involvement was already mentioned in the first reports. However, it's incidence is underestimated as it is often mistaken for arthralgia. Diagnosis of muscular involvement is suggested by clinical features and established by biological, electromyographical, and histological investigations. We describe five new cases of adult-onset Still disease with muscular involvement. All five cases followed the usual favorable course.

[Still disease in adults].

In 1971, less than one-hundred years after Still's publication (1897) concerning children, Bywaters described the adult-onset form of Still disease. Over one-hundred cases of this disorder have been recorded since. In adults, Still disease affects mainly women, and the average age at onset is twenty-seven. The well-known features demonstrated by clinical examination and complementary investigations are recalled. They include fever, arthritis, rash, lymphadenopathy, enlarged spleen, and serositis. Attention is drawn to hepatic and muscular involvement as well as to articular prognosis. Additional information is provided on questions which remain open to debate, particularly concerning management and therapeutic results.

[Rheumatoid arthritis with monoclonal gammopathy. Retrospective review of twelve case-reports (author's transl)].

Monoclonal gammopathy (MG) occurs in the course of rheumatoid arthritis (RA) in approximately 3.7% of cases. Twelve such cases were compared to two-hundred and fifteen controls with RA. RA was of later onset in patients with MG. Patients with MG had lower latex and Rose-Waaler titers; a decrease in these titres heralded onset of MG. The twelve patients with both RA and MG were then compared to nine-hundred and seventy-one controls with monoclonal gammopathy. As in previous studies, we recorded both IgM excess and unexplained lambda light chain excess.

[A new case of carcinoma arising in Caroli's disease (author's transl)].

A new case of carcinoma involving Caroli's disease in a 51 years old woman is reported. This papillary carcinoma is developed in left hepatic duct. The type is quite different from the previous cases already described. It is not a cholangiocarcinoma but a papillary mucoid carcinoma.

Folic acid deficiency and neutrophil dysfunction.

Polymorphonuclear leukocyte functions were studied in 92 patients with protein-calorie malnutrition. Serum folic acid levels were higher than 3 ng/ml in 38 patients and 3 ng/ml or less in 54 patients. Significant differences were found between these two groups of patients with regard to phagocytosis (81.5 +/- 1.9 versus 69.2 +/- 2.0 percent, p less than 0.001) and bactericidal ability (90.6 +/- 1.1 versus 84.5 +/- 2.3 percent, p less than 0.05). Correction of folic acid deficiency in 22 patients was associated with recovery of normal phagocytosis (p less than 0.001) but not bactericidal function. Adding folic acid to the serum of eight patients also restored normal phagocytic function (p less than 0.001). A correlation was found in vivo and in vitro between changes over time in folic acid levels and in phagocytosis.
