[Results of the differentiated use of catecholamines and vasodilators in the early post-bypass phase].

Presented are the results of a procedure to characterize the haemodynamic situation and the required therapy in the early post-bypass period by including haemodynamic parameters (Part, CVP, PLA, TPR, PVR, f, Cl) in a system of seven parameter constellations although directly measured values must be supplemented by clinical data. In this way situations with increased or normal cardiac output and pathological peripheral resistance can be more easily distinguished from situations with reduced cardiac output. The investigations were performed in 300 patients who underwent cardiac surgery in 1985 and 1986. In more than 80% of the patients increased volume alone was not able to end the bypass, and so therapy including catecholamines and/or vasodilatators was instituted. Due to a relatively intensive but specific treatment, a favourable result with a 3% mortality was achieved. The frequency of patients after coronary surgery, valve replacement or correction of congenital heart defects in the seven situations of parameters corresponded to the expected haemodynamic peculiarities. In the group treated with catecholamines only, including 34% of the coronary patients, no increased mortality occurred compared with the total number. In patients in whom the constellation of parameters required a treatment with catecholamines combined with vasodilators, a higher mortality of 8.6% occurred compared with the total result. Here patients are concentrated with whom a good surgical result could not be obtained or the changes of myocardial stimulation were exhausted. Our findings emphasize the importance of the following points for further practice: improvement of invasive monitoring to apply vasodilators more intensively especially in infancy, careful selection of patients for coronary surgery, reduction of negative inotropic drugs preoperatively in high risk patients for improving postoperative stimulation of the myocardium.

Tubulin oligomers and microtubule oscillations. Antagonistic role of microtubule stabilizers and destabilizers.

Several types of non-equilibrium phenomena have been observed in microtubule polymerization, including dynamic instability, assembly overshoot and oscillations. They can be interpreted in terms of interactions between tubulin subunits (= alpha, beta heterodimers), microtubules, and a third state, oligomers, which represent intermediates between microtubule disassembly and the regeneration of assembly-competent subunits by GTP. Here we examine the role of oligomers by varying conditions that stabilize or destabilize microtubules and/or oligomers. By varying their ratio one can drive tubulin assembly either into steady-state microtubules or oligomers. These regimens of assembly conditions are separated by a region where microtubules oscillate. The oscillations can be simulated by computer modelling, based on a reaction scheme involving the three states of tubulin and nucleotide exchange on tubulin subunits, but not on microtubules or oligomers.

Dynamics of the microtubule oscillator: role of nucleotides and tubulin-MAP interactions.

Microtubules can be induced to perform synchronous and periodic cycles of assembly and disassembly at constant temperature. The process depends on GTP hydrolysis. Time-resolved X-ray scattering using synchrotron radiation shows a cyclic interconversion of tubulin subunits, microtubules and oligomers (= short protofilament fragments). Oscillations are correlated with conditions that stabilize polymers and destabilize oligomers, and others of opposite effect. Microtubule stabilizers include GTP, Mg2+ or microtubule-associated proteins (MAPs), destabilizers include GDP or elevated ionic strength. K+ at intracellular concentrations noticeably increases the stability of tubulin-MAP oligomers, in contrast to Na+. ATP and the non-hydrolyzable analogue AMP-PNP enhance oscillations by mechanisms that are not directly linked to the role of nucleotide hydrolysis in assembly. We propose a mechanism of oscillations that include oligomers as microtubule disassembly products which transiently lock the protein in an unpolymerizable state; this may point to a role of oligomers in controlling microtubule assembly cycles in cells.

[Pharyngeal diphtheria diagnosed too late, fatal course of the disease].

In the course of an endemic lasting 13 months we saw three cases of toxic pharyngeal diphtheria. In spite of immediate treatment with serum and penicillin two patients died, including a 14 year-old girl who had been treated inadequately for two weeks. Serum along with antibiotics must be given as soon as diphtheria is suspected. Without the immediate administration of serum the mortality of diphtheria remains very high.

Enzyme-linked immunosorbent assay for detection of retroviral gp70 and gp70-anti-gp70 immune complexes in sera from SLE mice.

Since a retroviral envelope glycoprotein, gp70, present in sera is prominently involved in the pathogenesis of murine systemic lupus erythematosus (SLE), the detection of gp70-anti-gp70 immune complexes (gp70 IC) is particularly useful for the study of murine SLE. To facilitate the detection of gp70 and gp70 IC, we have developed a simple and sensitive enzyme-linked immunosorbent assay (ELISA). Using an affinity column coupled with whole mouse serum proteins containing serum gp70 or with Rauscher murine leukaemia virus (MuLV), antibodies specific to serum gp70 or to Rauscher MuLV gp70 were purified from hyperimmune goat anti-Rauscher MuLV gp70 antisera. Only affinity-purified anti-serum gp70 fraction, but not anti-Rauscher MuLV gp70 fraction, was able to detect serum gp70 efficiently in the ELISA, because only a minor fraction of goat anti-Rauscher MuLV gp70 antibodies is cross-reacting with serum gp70. This procedure could be applied to other antigen-antibody systems, in which only antibodies to heterologous cross-reacting antigens are available, to detect free and antibody-complexed antigens in pathological sera.

[Results of catamnestic studies on the social prognosis of childhood epilepsies].

Owing to the progress in pharmakotherapy the individual prognosis of epilepsy is characterized above all by brainorganic or psychoreactive caused disorders of the adaptability. The results of the examination of 153 children are presented, which were stationary treated for epilepsy. The special problems in the family, the school and in the choice of a job are referred.
