Juvenile angiofibroma of the maxillary sinus. A case report.

The clinicopathological features of a rare case of juvenile angiofibroma originating from the maxillary sinus of a 13-year-old boy are reported. This tumor was composed of angiomatous and fibrous structures. The analysis of nucleolar organizer regions (NOR) of the various components of this tumor indicated that the fundamental elements were the fibroblastic cells, rather than the vascular endothelial cells.

Fractal properties of herpes simplex dendritic keratitis.

Fractal geometry can be used to analyze the complexity and self-similarity of many natural forms and structures. By using three techniques for fractal dimension estimation, a group of 11 herpes simplex virus dendritic corneal ulcers was analyzed and found to possess outlines with fractal properties. The mean surface fractal dimension approached 1.4 over 1.5 decades of scale. The demonstration and quantification of the fractal dimension of dendritic lesions is important in the understanding of their behaviour and, at a more fundamental level, the planar spread of viral infection.

[The nimodipine therapy of acute focal cerebral ischemia (minor stroke). A clinical study with an assessment of regional cerebral blood flow by SPECT].

Aim of the study is the evaluation of therapeutic effectiveness of nimodipine in acute focal cerebral ischaemia. Thirty patients affected by minor ischaemic stroke divided in two randomized groups have been studied consecutively: all the patients were treated with standard therapy, nimodipine was delivered in addition only to the patients of the first group. Both clinical evaluation using Mathew scale, modified by Gelmers, and flowmetric evaluation with SPECT were performed at different times. The results haven't shown any significant statistical difference in the effectiveness of the therapy between the two groups even if a positive clinical trend was evidenced in the group treated with nimodipine. The flowmetric study has shown the poor homogeneity of the groups from a physiopathological point of view not-with-standing the two groups were similar for the clinical severity, sex, age and vascular risk factors. We conclude that is advisable to carry out further trials in which the comparison study groups are more numerous and balanced also from a physiopathological point of view.

Immunohistochemical demonstration of type II collagen in the chondroid tissue of pleomorphic adenomas of the salivary glands.

A study was done to investigate the presence of type II collagen and elastin in the metaplastic chondroid tissue of 21 pleomorphic adenomas of the major and minor salivary glands. Type II collagen was detected with anti-bovine type II collagen antibody after double digestion of histological sections with trypsin and hyaluronidase. The immunoreaction was positive in the chondrocytic cells and intercellular matrix. Elastic fibers in the chondroid tissue were found by orcein staining; they were scarce and randomly distributed. Although the presence of type II collagen and elastin in the metaplastic chondroid tissue is not directly implicated in the genesis of the tumor, it reveals a unique and high grade of cellular differentiation in comparison with true cartilage.

A fractal model for periodontal breakdown in periodontal disease.

This paper presents a computer model of periodontal breakdown based on 1/f noise. The model is multifactorial and simulates the disease advancing with age in bursts and remissions, as seen in previous epidemiologic studies. Changing the weight of some parameters changed the simulated periodontal breakdown dramatically, disclosing antagonistic and synergistic effects between the causal factors.

Odontogenic epithelial hamartoma of the gingiva: a case report.

A rare case of odontogenic epithelial hamartoma of the gingiva in a 63-year-old Japanese female is reported. This tumor-like lesion probably originated from the reduced tooth-forming tissues such as rests of dental lamina lying dormant in the gingiva after odontogenesis. The differential diagnosis of this lesion from many other odontogenic and glandular tumors is important; however, through careful clinico-pathological examinations, it would not be a very difficult task.

Videodensitometrical study of the alveolar bone crest in periodontal disease.

The relative mineral content per unit volume of bone in the alveolar bone crest was determined in 81 bone crests from 25 human autopsy cases by means of contact microradiography and videodensitometry of 100 microns thick sections. Each bone crest was divided in 4 areas: crestal, mesial, distal, and trabecular; and the relative mineral content was determined for each area. The data were analyzed statistically and it was found that the crestal area had a significantly lower mineral content than the other 3 areas. Histologically, the crestal area had a reduced number of Sharpey's fibers and it was speculated that these features lower the probability of restoring the natural crestal shape and the fiber reattachment, increasing the risk of bone loss. No correlation was found between relative mineral content and age or extent of inflammatory infiltration of the periodontal tissues.

Heterotopic central neural tissue of the tongue.

A rare case is reported of a pedunculated tumor containing heterotopic central neural tissue in the tongue of a 3-month-old girl. The lesion was located near the foramen caecum and was composed of glial cells, ependymal cells, a small number of neuronal cells and a relatively thick peripheral nerve.

Eosinophilic granuloma showing rapid regression. Report of a mandibular case with application of a modified PNA staining method for demonstration of Langerhans-type histiocytes.

A mandibular eosinophilic granuloma in a 16-year-old male is reported. This case showed rapid regression, which was clearly demonstrated by histopathological examinations of both preoperative biopsy and surgical materials. Transformation from an eosinophilic granuloma to a xanthomatous granuloma with multinucleated giant cells was observed after only 26 days. Special staining of paraffin sections with peanut agglutinin (PNA) and use of electron microscopy showed that the main component of the lesion in the biopsy material was Langerhans-type histiocytes. These cells had disappeared from the lesion by the time of the operation. At the same time, the number of infiltrating eosinophils was also markedly reduced. It seems appropriate to consider that the rapid regression of this disease was correlated with the rapid reduction in the number of Langerhans-type histiocytes appearing in the granulomatous foci, as well as the number of infiltrating eosinophils.

Nucleolar organizing regions (NORs) in pleomorphic adenomas of the salivary glands.

The number of nucleolar organizing regions (NORs) can be used to reveal the degree of cell activity or metabolism in histopathology specimens. Forty-one cases of pleomorphic adenomas of the salivary glands were studied with a silver nitrate colloid staining method to demonstrate the NORs in the cells of the solid and duct pattern areas and in chondroid areas. Acinic and intercalated ducts cells of the surrounding normal salivary glands were also examined. The data was analyzed statistically and it was found that the cells in the solid/ductal areas had a higher cellular activity than the chondroid cells. This difference is probably directly related with the possibility of malignant transformation and the incidence of the different types of malignant pleomorphic adenomas. Furthermore, it is concluded that the number of NORs in pleomorphic adenomas to define malignancy should be done discriminating between the different histologic patterns because of their different metabolic activities.

[Nucleolus organizer regions (NORs) in metastasizing pleomorphic adenoma].

The metastasizing pleomorphic adenoma of the salivary glands is a rare variant of pleomorphic adenoma with a benign microscopical appearance, but malignant biological behaviour and production of metastasis. The histopathological study with the routine techniques is not enough for disclosing the nature of these tumors and they are most of the times underdiagnosed as benign. The correct diagnosis, almost always too late, is evident after several recurrences and the detection of metastatic foci. The number of nucleolar organizing regions (NORs) detected with the silver colloid method can be used in histopathology to determine the degree of cell activity. We report the findings in a case of metastasizing pleomorphic adenoma of the submandibular gland. This technique demonstrated to be useful for the diagnosis and characterization of the metabolism of these tumors.

Chondroma and osteochondroma of the tongue.

The first case of two separate tumors of the tongue, one cartilaginous and the other osteocartilaginous, is reported. The cartilaginous matrix of these two tumors contained elastic fibers and type II collagen. The histogenesis of these tumors is discussed.

[Platelet aggregation inhibitory therapy in chronic arterial occlusive disease of the lower limbs: results of short-term clinico-instrumental study with indobufen].

Aim of our study was to evaluate the effectiveness of indobufen in the treatment of II degree stage lower limb occlusive arterial disease. Forty patients were randomly assigned to the iIndobufen (400 mg/die) treatment group or the no treatment control group (20 patients each). Doppler pressure measurement (Winsor index) and subjective symptomatology were evaluated at the time of enrollment and after one month. A significant improvement (p less than 0.01) was noted only in the treated group. We conclude that indobufen may be of therapeutic value in the treatment of peripheral occlusive arterial disease.
