Medical ethics play an essential role in the practice of medicine, in the care of individual patients, in the allocation of health care resources, and in the formulation of health care policy. A specific body of knowledge, 'biomedical ethics', has developed which applies ethical theory to biomedical practice. This has provided doctors with tools systematically to integrate rational ethical analysis into clinical decision-making. Training in the discipline of biomedical ethics is now required for all doctors in Canada. The goals, content areas, learning objectives, and learning methods considered appropriate for advanced training in this field for medical specialists are provided in this paper. Six topic areas are discussed: introduction to ethical theory, clinical ethics, professional ethics, ethics of human experimentation, ethics of health policy, and independent study. Ways this curriculum could be organized and evaluated are also offered.

Intravenous indomethacin has been reported to diminish coronary flow in patients with angina. There is also evidence that indomethacin attenuates or abolishes the coronary dilator and venodilator effects of nitroglycerin. In 11 patients with stable angina of effort, prior administration of 100 mg of indomethacin appeared to increase (0.10 greater than p greater than 0.05), rather than reduce, the work load achieved compared with placebo on a double-blind basis. The increase in work load when patients were given nitroglycerin was not significantly changed after they were given indomethacin. Therefore, this study does not support the suggestion either that prostaglandins play a significant role in coronary vasoregulation on effort or that prostacyclin is an important mediator of the therapeutic action of nitroglycerin. From a practical therapeutic viewpoint, there is apparently no danger of aggravating stable, exercise-induced angina or of abolishing the benefit normally expected from nitroglycerin when patients with angina are given indomethacin in doses comparable to those used in this study.

The authors compared the results of 100 pacemaker insertion procedures: 50 endocardial and 50 epicardial. The late pacing threshold and the postoperative pain and pulmonary complications were higher in the epicardial group. Although dislodgement occurred in 28% of cases of insertion through the endocardial route, the authors concluded that this route was preferable.
