Oncocytic lesions of the larynx.

True laryngeal adenomas are considered extremely rare and some doubt their existence. Laryngeal lesions having oncocytes as the predominant cell type have been described in the literature under a variety of different names: oncocytoma, oncocytic papillary cystadenoma, papillary cystadenoma, oxyphilic granular cells adenoma, etc. The oncocytes originate from changes in normal acinic and ductal cells. The significance of these changes is controversial. Whether the oncocytic laryngeal lesions represent true neoplasms or are degenerative in nature remains to be determined. We present clinical and histopathological details of four cases of oncocytic lesions of the larynx and compare their histology with microscopic findings in 30 cases filed as laryngeal cysts at the Minneapolis V.A. and University of Minnesota Hospitals. While the controversy regarding the neoplastic versus degenerative etiology of these lesions could not be resolved from a histopathological standpoint, their clinical behavior is that of a benign neoplasm. We review the literature on the subject and suggest a course of treatment for these lesions.

Anticipatory guidance in elective ophthalmic surgery.

With respect to the preoperative ophthalmic patient, anticipatory guidance is a technique of informing the patient in a detailed way about all aspects of the upcoming intervention, including the need for surgery, the degree of pain and disability to be expected, type of anesthesia to be used, some information about the postoperative course, and the expected disability following surgery on either a temporary or a permanent basis. The purpose of anticipatory guidance is to permit the patient to make a better psychologic adaptation to a crisis situation and to relieve preoperative anxiety through information, resulting in easier postoperative recovery. In addition, anticipatory guidance and improved understanding between physician and patient may go far to reduce those misunderstandings that lead to malpractice actions subsequent to surgical procedures.

Immunologic assessment of regional lymph node histology in relation to survival in head and neck carcinoma.

Histologic material from 84 cases of squamous cell carcinoma of head and neck regions was studied by double blind retrospective analysis. Sections of lymph nodes draining the tumors were examined microscopically to assess the morphologic pattern of response. Patients whose lymph nodes demonstrated active immunologic responses in the form of expanded inner cortices or increased numbers of germinal centers had 5-year survival rates significantly greater than those patients whose regional lymph nodes showed an unstimulated pattern. None of the patients whose lymph nodes showed the depleted pattern survived 5 years. These correlations were independent of the stage or grade of the tumor. Metastases occurred much more frequently in patients having regional nodes showing an unstimulated or depleted pattern than in those whose regional nodes showed evidence of immunologic activity. The data support the concept of a relationship between immunologic activity, progression of neoplasia, and survival. Morphologic assessment of immunologic activity in lymph nodes draining malignant tumors appear to be of significant value in the predicting survival.

Prognostic significance of lymph node histology in patients with squamous cell carcinoma of the larynx, pharynx, or oral cavity.

In a double-blind retrospective analysis, sections of lymph nodes regional to head and neck squamous cell carcinomas were microscopically examined to assess morphologically the immunologic pattern of response. Patients whose nodes showed evidence of immunologic stimulation had five-year survival rates significantly higher than those whose nodes showed no evidence of immunologic stimulation. None of the patients whose nodes showed the lymphocyte depletion pattern survived five years. The stage or histologic grade of the tumors did not influence these correlations. Metastases occurred much more frequently in patients whose nodes showed immunologic activity than in those whose did not. The data support the concept that immunologic capacities are important host defense mechanisms against malignancy. Histologic assessment of immunologic activity in regional nodes seems to be an important parameter for predicting survival.

Carcinoma of the lip.

A retrospective analysis of 206 patients with lip cancer is presented. Poorly differentiated primary lesions have a high rate of metastases. The metastatic potential of well differentiated lesions, when classified by size in the TNM staging system, is the same as that of intraoral carcinoma. A therapeutic approach is proposed.
