[Computer aided recording and analysis of dental dates of children from the aggregate list (printed form 2008)].

The authors represent a further program-system for computer aided data processing of the results from the systematic dental examinations in children. The data input starts with the summarized dates of each class fixed in the sumlist (printed form 2008). This program needs less computer capacity and considers the limited availability of personal computers in dentistry for children.

[Psychosocial maladjustment and criminal responsibility--results of a catamnestic study].

The introduction in 1968 of the legal concept of Grave Abnormal into the penal code, Development of the Personality Amounting to a Disorder made possible criminal exculpation on the basis of psychosocial maldevelopment. Statistically established criteria for its application have hitherto been lacking. A corpus of 276 offenders on probation under this legal provision has now been compared statistically with a control group homogeneous in respect of the significant parameters. It has been possible to arrive at differentiated syndromes which are well characterised by the corresponding criteria in the penal code.

[Pancreatic cancer--clinicopathology and etiology].

An analysis of 287 (1.42%) exocrine pancreatic carcinomas of an 18 years post-mortem examination includes pathomorphological and histological findings of the tumors, causes of death, important concomitant diseases, and relevant clinical details. The tumor prefers the old age. We found a medium age of 68.2 years and a nearly balanced sex ratio, but some age dependent differences in this ratio. Localization, histological type, staging and dependent findings of the tumours do not differ essentially from wellknown statements. Among the causes of death there was in the first position the tumour itself (48.5%), followed by general tumour associated (39%) and local complications (11.5%). We found no foregoing diseases respectively endogenous risk factors of pancreatic cancer. This statement concerns particularly the chronic pancreatitis, diseases of the biliary tract or the gastroenteron, and diabetes mellitus. The role of a particular blood group was to exclude. Special findings are discussed. The results rather support the significance of exogenous risk factors, especially of nicotine and ethanol, which, in conjunction with other in relation to the exocrine pancreas weak carcinogenic agents and hyperplasiogenic factors may contribute to the increasing incidence of the exocrine pancreatic cancer.

Problems and results of selective decontamination in leukemia patients.

Infectious complications play a major part during the cytostatic treatment of patients suffering from acute leukemia, as well as in bone marrow transplantations. For infection prevention, the method of selective decontamination (SD) of the digestive tract was used. This procedure eliminates the potentially pathogenic aerobic bacteria and yeasts while leaving the anaerobic intestinal microflora unaffected. The protocol of the Gnotobiotic Project Group was used in treating 33 patients with SD and the results were compared with those from a comparable control group consisting of cases selected from files who had been treated without SD in the past. A statistical difference was obtained in favor of the SD patients regarding frequency and severity of infection, time between admission and the first signs of infection, days febrile, and in the additional necessity of antibiotics (p less than 0.01). SD involves in addition to a scheduled intake of tablets, microbiological surveillance, personal hygiene and an intact hemostasis, and optimal results can be obtained given the help of the patients' compliance. SD has won a secured place in treatment tailored upon the pathophysiological concept of infection prevention, even though there are problems remaining, e.g., gram-positive cocci, yeasts and infection of the oropharynx.

[Determination of skeletal age using the Oxford Score in children with epiphyseolysis of the femur head].

Pelvis radiographs of 47 children with slipped capital femoral epiphyses (SCFE) and 24 without a hip disorder were reviewed in a retrospective study by four readers to determine the skeletal age by the Oxford method. 72% of the children suffering from SCFE had a body weight above the standard level but a normal body height at the time of diagnosis. The mean chronologic age was 3 years higher than the skeletal age determined by the Oxford-score. The linear regression analyses showed that children with SCFE have had a skeletal maturation delay in comparison with the normal group at the beginning of the age period in question. This deficiency became greater as chronologic age increased. All children with the SCFE disease showed a nearly uniform skeletal age clearly below the bone age standard irrespective of their chronologic age. These results, in connection with obesity in most cases, support the idea of hormonal disorders with delayed skeletal maturation development in the aetiology of SCFE disease.

[Experiences in therapy of bradycardic cardiac arrhythmias in childhood with ipratropium bromide].

Ipratropiumbromid is used in adults for a variety of bradyarrhythmias. In contrast this therapy has not yet been established in childhood, where symptomatic bradyeardias are less frequent. Since Ipratropiumbromid has less side effects (especially concerning the central nervous system) and a longer half time than atropin, we investigated the efficacy of Ipratropiumbromid in 12 children (mean age 7.2 yrs.) in the course of 2 1/2 years. We found a significant increase of the minimal and mean heart rate in Holter-Eeg-recordings. Only in 7 of 12 patients we obtained a sufficient therapeutic effect. In one patient we had to terminate the therapy because of side effects. In two patients with atrioventricular block and two children with higher degree sinus node dysfunction could be removed only by implantation of a pacemaker. In another case we observed the proarrhythmical effect of Ipratropiumbromid. Summarizing we think, that the therapy with Ipratropiumbromid may be useful in symptomatical supranodal bradyarrhythmias in childhood in regard of side effects.

Tumour necrosis factor-alpha promoter polymorphism in erythema nodosum.

Erythema nodosum is a common skin disease characterized by erythematous, tender subcutaneous nodules, mostly located on the lower extremities. Little is known about its pathogenesis, although a wide variety of aetiological factors (e.g. bacterial and viral infections, neoplastic diseases and drugs) have been described. Sarcoidosis, a typical granulomatous disease, often occurs in association with erythema nodosum (Loefgren syndrome). Since granulomatous diseases have been closely linked to a deregulated tumour necrosis factor (TNF)-alpha production, it was tempting to speculate whether TNF-alpha might play a role in the pathogenesis of erythema nodosum, at least in cases associated with sarcoidosis. A previously described nucleotide exchange, (G-A) at position -308 in the human TNF-alpha gene promoter, has been shown to be a major cause for enhanced TNF-alpha production. In the present report, we investigated the genomic TNF-alpha promoter region in patients suffering from EN with and without underlying sarcoidosis. Our results showed a strong correlation between the uncommon TNF A II allele and sarcoidosis-associated erythema nodosum. Patients with erythema nodosum without underlying sarcoidosis displayed a similar allele frequency compared with controls. Taken together, we provide evidence that erythema nodosum in association with sarcoidosis might be pathogenically linked to altered TNF-alpha production due to a genetic promoter polymorphism.

Comparison of cystourethrography and dynamic MRI in bladder neck descent.

PURPOSE: The purpose of this work was to test whether there are statistically significant differences between dynamic MR and lateral cystourethrogram measurement results in patients with bladder neck descent. METHOD: Twenty-seven women (39-83 years old, mean 60.6 years old) with urinary incontinence and bladder neck descent were examined by dynamic MRI using a single shot fast spin echo sequence with half-Fourier data acquisition. Bladder neck position, angle of inclination of the urethral axis, posterior vesicourethral angle, and depth of cystoceles were measured at perineal contraction and at maximal pelvic strain. The nonparametric Wilcoxon test for paired values was used to analyze whether there were statistically significant differences between lateral cystourethrogram and dynamic MR measurement results. The Spearman correlation coefficient (rs) was calculated for all parameter pairs. RESULTS: Measurements at maximal pelvic strain showed the greatest levels of agreement between MRI and cystourethrography. The best results were attained for the cystocele measurements (p > 0.5, rs = 0.95). Bladder neck position showed the second best agreement; if MR measurements were corrected by 0.46 cm, no statistically significant difference (p > 0.2, rs = 0.92) was calculated. Measurements at perineal contraction tallied least, probably due to the different positions adopted during the two examinations. CONCLUSION: Measurement data on dynamic MRI for the bladder neck position and the extension of cystocele at maximal pelvic strain are comparable with lateral cystourethrogram data.

Efficiency of pupillary stretching in cataract surgery.

PURPOSE: To assess the efficiency of pupillary stretching with iris hooks in poorly dilatable pupils during phacoemulsification. METHODS: Forty eyes with maximal preoperative mydriasis less than 4.5 mm (mean 3.9 mm S.D. 1.0) received bimanual pupillary stretching according to the technique of Miller and Keener (1994). RESULTS: Small pupils were mainly caused by long-term pilocarpine therapy (n = 21), posterior synechia (n = 11) or diabetic iridopathy (n = 7). With multidirectional stretching, the pupil could be enlarged to mean 6.6 mm (S.D. 0.66), lasting throughout phacoemulsification. The effect of stretching was significantly greater in eyes with synechias, but no significant differences in post-operative pupil motility was observed between the three subgroups. Six eyes with pronounced postoperative inflammation responded well to antiinflammatory treatment. CONCLUSIONS: Done with routine surgical instruments, bimanual pupillary stretching seems to be an easy, safe and effective procedure for enlarging small pupils by an average of 2.7 mm during phacoemulsification, especially in eyes with posterior synechias.
