[Antibody deficiency syndrome of the "common variable type" with intestinal lymphatic hyperplasia and secondary gastric carcinoma].

Case report on a patient with agammaglobulinaemia, repeated respiratory infections, diarrhoea and nodular lymphoid hyperplasia of the small intestine. Giardia lamblia was seen in duodenal aspirates. B-lymphocytes were not decreased, but no plasma cells could be found. Later the patient developed a gastric carcinoma, which was also described in previous case reports.

Common errors in the treatment of suicidal patients.

As a suicidology consultant to a county coroner and to several hospitals, the author has noted 5 common errors by the mental health caretakers in the treatment and management of suicidal patients. Prototypical cases of these errors are presented and discussed along with suggestions for prevention.

[Hodgkin's disease, stage IVb: survival for 12 years (author's transl)].

Long survival in Hodgkin's disease after diagnosis of the stage IVb is very rare. A 45-years old woman is reported on, who survived for 12 years after detection of Hodgkin's disease involving lymph node regions on both sides of the diaphragm, bone, and lung parenchyma, including night sweats and weight loss (stage IVb according to the Ann Arbor classification 1971). This unexpectedly favorable course is possibly due to a so-called host resistance.

[Syndrom of hypercalcemia in a case of hyperthyroidism, primary hyperparathyroidism and cancer of the gall-bladder (author's transl)].

A hypercalcemia syndrom in the case of a 63 year old female patient is reported on, who simultaneously suffered from hyperthyroidism and suspected primary hyperparathyroidism. During autopsy an ectopic (ventral mediastinum) parathyroid adenoma and a gall bladder carcinoma were discovered. Each of these three diseases can induce hypercalcemia by themselves.

[Collagen disease with pure red cell anemia and perimembraneous glomerulonephritis (author's transl)].

Record on a case of collagen disease with pure red cell anemia and perimembraneous glomerulonephritis. The direct antiglobulin tests with anti-IgG and anti-kappaserum were positive as were the tests on antinuclear serum-antibodies. During therapy with fluocortolon and azathioprin the anemia remitted completely even when therapy was stopped. The antiglobulin tests became negative, the glomerulitis remained active, there was no deterioration of renal function.

The management and mismanagement of a suicidal patient.

The author begins with the premise that the actions of therapists and hospital staff can contribute to the likelihood that a suicidal patient will eventually take his life. He recounts a case history of a depressed 51-year-old man who was hospitalized following a suicide attempt and delineates eight "mistakes" that contributed to the patient's eventual suicide and that are common occurrences in dealing with a suicidal patient. He believes that if more therapists and mental health professionals share mistakes and experiences of this kind with each other, such errors might be minimized or even prevented.

Is there a biochemical predictor of suicide?

There is a need for additional instruments to alert the clinician more effectively to the high self-destructive potential of some of his suicide-risk patients. In this study it was found that suicides-to-be tended to have higher plasma cortisols than suicidal patients who did not subsequently commit suicide within two years of the test. The article discusses the possible mechanisms involved and reviews some of the literature indicating that there may be a biochemical substratum that may play a significant role in some suicides.

Suicides in San Mateo County.

The usual surveys of completed suicides, encompassing, as they do, large geographical areas, are of limited value to physicians of a particular community. The unique and differentiating characteristics of the suicides in his locale may be "washed out" in these large surveys.San Mateo County has an annual suicide rate of 17 per 100,000 and a disproportionately high incidence in persons over 65 years old. In this particular county females, widows and Orientals are more prone to suicide than has usually been reported elsewhere. Alcohol was directly or indirectly involved in a significant number of instances. Many of the persons who killed themselves were under a physician's care at the time of self-destruction. There are probably important ecological and sociological variables as well as personal factors involved in the suicidal process that are of significance to any suicide prevention program. It is urged that there be more extensive and comparative research in this important public health problem.
