[Dental enamel hypoplasia apropos of a case].

The developmental anomalies of dental hard tissues are relatively common in children. These anomalies can involve separately the enamel and they are due to many factors acting during odontogenesis. The paper deals with the main ethological factors and describes a case of idiopathic hypercalcemia. It is normally accompanied by aortic stenosis, mental retardation and a characteristic elfin face. This is called Williams syndrome. In this case we only found enamel hypoplasia on the cusps of the first molars.

[Cariologic and periodontal screening of children aged 7, 12 and 14 years in the city of Debrecen. II. Periodontal status and need for treatment].

The aim of present survey was to gain epidemiological data on periodontal condition of 7, 12 and 14 yrs old (40 children in 7, 92 children in 12 and 84 children in 14 yrs old groups) schoolchildren as assessed by the CPITN. The peridontal health of children proved to be very poor. Only 33.3% of 7 yrs old, 20.9% of 12 yrs and 13.1% of 14 yrs old children showed healthy gingiva. Bleeding on probing was observed in 66.7% of 7 yrs old group, in 19.8% of 12 and in 16.7% of 14 yrs old groups. The calculus prevalence was 0.0% of 7 yrs old children, 56.0% in 12 and 44.7% in 14 yrs old groups. Oral hygiene instruction was needed by 66.7% of 7, 79.1% of 12 and 86.9% of 14 yrs old children. Scaling was necessary in 11.1% of 7, 59.3% of 12 and 70.2% of 14 yrs old groups. Complex treatment was needed in none of the groups examined.

[Use of hydroxy-propyl-methyl cellulose (methocel) and carboxy-methyl cellulose containing artificial saliva in the symptomatic treatment of xerostomia].

Salivary gland hypofunction is a common sequela of Sjögren's syndrome and irradiation treatment of tumors in head and neck region. The aim of the present study was to evaluate the therapeutic effect of a recently developed hydroxy-propyl-methyl-cellulose (HPMC) containing artificial saliva and to compare it with that of the carboxy-methyl-cellulose (CMC) based saliva substitute. The therapeutic effects of both preparations were assessed by means of self-administered questionnaires which had to be completed before and after the use of each type of artificial saliva after one week treatment. We found significant improvement in symptoms of nocturnal discomfort and difficulty in speech, and results were better in case of HPMC based artificial saliva. The retention time in oral cavity and the frequency of daily administration also were better in case of HPMC containing preparation. It is concluded that the use of HPMC containing artificial saliva can be recommended in the treatment of salivary hypofunction.

Technique for fabrication of centric relation-based occlusal splint with central bearing device.

This study presents a technique for constructing an occlusal appliance. The central relation position must be located with Gothic arch registration. A central bearing post is fixed to the upper vacuum-adapted sheet and a registration plate is made for the lower cast. For the eccentric guidance construction, a prefabricated guiding ramp is secured just behind the most anterior point of the Gothic arch. The eccentric guidances, which harmonize with condylar movements, are printed by the mandibular canines on the surface of the self-curing resin as the post slides on the inclined surface of the ramp.

[Variations of pathological tooth loss with special reference to tooth erosion--apropos of a case].

Importance of tooth surface loss has increased in dental practice. It is difficult to determine aetiological factors and the exact diagnosis, because the symptoms are often combined. The aim of the present study was to summarise the most important factors of tooth surface loss, mainly of erosion which is a frequent form. Reviewing the literature aetiological factors, symptoms, differential diagnostic problems and therapeutic advice are discussed. Prevention, early diagnosis and treatment are very important in these patients.

[Correlation between meteorologic factors and periostitis of dental origin in Debrecen].

Correlation between bioclimatical and meteorological factors is well documented. The aim of this study was to investigate the correlation between dental periostitis and meteorological fronts, cyclon and anticyclon. We found a positive, statistically significant correlation between occurrence of warm meteorological front, cyclon, anticyclon and prevalence of dental periostitis. It could have important implications in organizing the work of dental teams.

[Cariologic and periodontal screening of children aged 7, 12 and 14 years of age in Debrecen. I. Prevalence of caries and need for treatment].

A WHO survey was carried out earlier on caries prevalence in the 12-year olds. No data are available in the 7- and 14 year old children. The aim of this study was to investigate caries prevalence (DMFT, dmft numbers), percentage of caries free children and treatment needs in these three age-groups. The clinical examination was based on WHO's criteria. DMF numbers were 0,12, 2,11 and 2,51 in 7-, 12- and 14-year old, while dmf numbers were 0,79, 0,51 and 0,05 respectively. Percentage of caries-free children was 25% (aged 7), 3,3% (aged 12) and 3,6% (aged 14) in the primary teeth, 87,5% (aged 7), 3,3% (aged 12) and 0 (aged 14) in the permanent teeth. There is no treatment need in 27,5, 22,8 and 17,9% of studied groups. Extraction need was 0, 4,3 and 3,6% in 7-, 12- and 14-year-old. Restorations need was the following: one surface: 45, 65,2 and 71,4%, two surfaces 57,5%, 37, 52,4%, three surfaces: 35, 8,7, 16,7%, more than three surfaces or crown: 2,5, 1,1 and 2,4% at the age of 7, 12, 14 years. The number of DMF and dmf teeth was lower at the 12-year-old children than the previous Hungarian data.

[Testing the reliability of face bow registration].

The reliability of the Quick ear face-bow to transfer an arbitrary hinge axis to a Quick articulator was clinically compared with a Dentatus face-bow. 40 measurements were carried out on 8 patients with both instruments and were compared. The statistical analysis of the data indicated no difference between these facebow recordings. Furthermore both showed good reproducibility. This suggests that the same clinical results can be reached by these instruments.

Extraoral Traction Appliances

[Dietary habits and oral hygiene in school children in the city of Debrecen].

Acid products formed by bacterial fermentation of dietary carbohydrates in the dental plaque, are required for the initiation of dental caries. The aim of the present study was to analyze nutritional and oral hygienic habits of 216 schoolchildren (108 boys and 108 girls) aged of 7, 12 and 14 years. In the studied population children brushed their teeth after meals in 42.5, 54.3 and 38.1% (in the 7, 12 and 14 year age groups respectively). The additional hygienic aids (interdental cleaning: 32.5, 42.4 and 34.5%, oral rinse: 7.5, 27.2 and 23.8%) were used only by a smaller part of the studied groups. Consumption of dairy products and fruits was satisfactory. The habit of drinking carbonic acid containing soft drinks was rather high (%). Some (2.5, 10.9 and 7.1%) of the children drink it several times a day. Fluoride containing Dentocar tablets were used only in 42.5, 3.3 and 1.2% in the studied groups.

[Short-term effectiveness of fissure sealants].

Fluoride therapy is a widely accepted method of reducing smooth surface caries. Sealant therapy is intended to provide similar results for the occlusal surfaces of the teeth. In this study the retention of sealants was examined and a comparison was made between the caries rate in the group with sealed molars and controls after 12 months... The caries increment was significantly lower in the sealed group.

[Chemical plaque control].

Most of the diseases with which dentists must deal with are related to microbial plaque and bacterial infection. The mechanical removal of supra- and subgingival plaque by the patient and by the dentist respectively, is time consuming and exacting, furthermore it is not one hundred per cent successful. There has been extensive search for many years for chemical agents that could supplement or even substitute mechanical plaque removal. Many chemical agents delivered in different ways were tested. Chemotherapeutics of various modes of action have to meet pharmacokinetic and biological requirements. Apparently the ideal anti-plaque agent is not yet available, so that they should be reserved for well defined clinical situations and short--or medium--term application.

Anti-Bacterial Agents

[Comparative study of the rheological properties of artificial saliva types based on hydroxy-propyl-methyl-cellulose (methocel) and carboxy-methyl-cellulose (CMC)].

Apparent viscosity at different shear rates was measured for two types of saliva substitutes based on methocel and CMC, and was compared with that of human whole saliva. Results indicate, that fresh mixed human saliva is viscoelastic, exhibiting pseudoplasticity. Methocel containing artificial saliva and human whole saliva are similar in their rheological properties, while the preparation based on CMC is almost a Newtonian liquid. From the results it can be concluded that the saliva substitute containing 0.5% of methocel appears to be a better substitute for natural saliva than of containing CMC as far as rheological properties are concerned.

The width of plaque-free zones on primary molars with attachment loss.

The width of plaque-free zones on stained surfaces of extracted primary molars showing attachment loss was measured. The teeth were divided into 4 groups: maxillary first and second molars, mandibular first and second molars. 30 measurements were made under a stereomicroscope on each of the 4 surfaces in every group. The plaque-free zone had a mean width of 1.75 +/- 0.49 mm and a range of 1.0-4.3 mm. The plaque-free zone was significantly (P less than 0.001) wider on the proximal surfaces (1.94 +/- 0.56 mm) than on the combined buccal and lingual surfaces (1.56 +/- 0.32 mm). There was no difference between the 2 proximal surfaces while the lingual surfaces gave higher values than the buccal ones. On the second and on the lower primary molars, the plaque-free zones were wider than on the first and on the upper molars.

[Probing and occlusal caries].

Occlusal fissures are one of the most frequent locations for dental caries. In order to detect early caries lesions in terms of "sticky fissures" tactile examinations with a sharp explorer are important parts of a classical clinical examination. The aim of the present investigation was to study whether probing with an explorer contributed to the development of occlusal caries in permanent of first molars. The study was carried out in 101 children (56 girls and 45 boys) who had intact pairs of first molars. Altogether 104 first molar pairs were studied. One and two years later both of the pairs were probed, but there was no significant correlation found between previous probing and occlusal caries.

History, 20th Century

[Periodontal effects of the bridge and neighboring anchors, based on follow up studies].

Two weeks after cementing in the dentures there is no evaluable difference in the Tm., the Pl. I. and the G. I. between the check-up examined surfaces on the approximal surfaces close to the bridge. Generally 8 months later a higher G. I. points on the approximal surfaces of neighbouring anchors by the employed periodontological examining methods. It means an unfavourable prognostic that the inflammation symptoms on the surfaces the second and third rate abutment teeth remained also after the prosthetic treatment to a similar degree.

Dental Abutments

[The size of the overlapping of the gingival margin in Class II amalgam fillings examined in the teeth and their radiographic images].

200 Class II extracted teeth provided with amalgam filling were examined. The size of the overlapping of the fillings were measured on the X-ray pictures and in the transverse section of the teeth embedded in self-cementing plastic under a stereomicroscopy by means of an ocular micrometer. The mean overlapping of the fillings was 0.52 +/- 0.44 millimeters on the teeth and 0.48 +/- 0.43 millimeters on the X-ray pictures. There was no significant difference between the values obtained by both methods. On the upper teeth [teeth: 0.64 +/- 0.50 millimeter, X-ray: 0.59 +/- 0.48 millimeters (significantly) P less than 0.001] the overlapping of the fillings was greater than on the lower ones (teeth: 0.39 +/- 0.49 millimeters, X-ray: 0.36 +/- 0.33 millimeters). The other comparisons did not show any significant difference by either method. The oro-vestibular extension of the overlapping gingival filling borders was, on the average 3.45 +/- 1.25 millimeters and amounted to 46.25 +/- 15.94% of the width of the surfaces. The percentual oro-vestibular extension of the gingival filling borders was significantly greater (p less than 0.05) on the premolars than on the molars. The gingivally overlapping filling showed also an oral and/or vestibular overlapping.
