Infections of the nose and paranasal sinuses in adult patients with immunodeficiency.

Twenty-two patients with common variable immunodeficiency (CVID) and 18 patients with selective IgA deficiency were examined with respect to previous and present infections of the upper respiratory tract (URT), especially of the nose and paranasal sinuses. Recurrent acute rhinosinusitis was common in both groups of patients, but the development of chronic rhinosinusitis was only found in patients with CVID, indicating the more severe nature of this condition compared with selective IgA deficiency. The infections of the URT occurred several years before the appearance of lower respiratory tract (LRT) infections. Once the infections of the LRT had started, the patients had a tendency to neglect the symptoms from the URT. Early detection of antibody deficiency syndromes is of vital importance for prevention of repeated and chronic infections often causing tissue damage in the respiratory tract.

The effect of radiation on teeth. A clinical, histologic and microradiographic study.

54 teeth from 20 patients irradiated for treatment of malignant tumors of the head and neck region were examined clinically, histologically and microradiographically. 18 teeth from 8 patients were used as controls. A significantly higher degree of hyposialia, radiation mucositis and atypical caries was found in the original group as compared to the control material. Occurrence of narrow gaps between dentin and cementum was also higher in the former group as compared with the latter.

Goblet cell number in the nasal mucosa relates to cell-mediated immunity in patients with antibody deficiency syndromes.

Histopathological changes in the nasal mucosa were studied in biopsies from the inferior turbinate in 22 patients with common variable immunodeficiency (CVID) and 16 patients with selective IgA deficiency. Most patients with CVID had fewer goblet cells than the healthy controls (p less than 0.001), whereas patients with selective IgA deficiency often had manyfold increased numbers (p less than 0.001). The number of goblet cells showed a significant (p less than 0.001) relationship to the cell-mediated immune function. Patients with lowered cell-mediated immune function, as measured by in vivo and in vitro tests, showed reduced numbers or lack of goblet cells. These patients had an increased number of subepithelial, seromucous gland acini compared with healthy controls (p less than 0.01) and patients with selective IgA deficiency (p less than 0.05). The production of mucus may be a compensatory mechanism, participating in the protection of the mucosal surface of the antibody-deficient patient. The increase in the number of goblet cells, but not of subepithelial, seromucous gland acini seems dependent on the cell-mediated immune system.

Structural and functional aspects of cells in the nasal mucociliary system.

The regulation of the fluid balance of the mucus is important for a good function of the mucociliary system in the nose. The nasal glands and the goblet cells have been proposed to be the main source of fluid. Microvilli-equipped epithelial cells, ciliated and nonciliated, constituting 70% to 90% of the cells on the turbinates in the nose, are of importance for the regulation of the hydration of the mucus. These cells have the same characteristics as other fluid-secreting cells elsewhere in the body. They have microvilli, a terminal web, folding of the basolateral membrane, and numerous cell organelles. Blockage of the energy supply strongly reduces the fluid secretion.

Effect of disodium chromoglycate on changes in nasal airway resistance induced by platelet activating factor.

Thirteen healthy subjects participated in a double blind, randomized, cross-over investigation of whether or not intranasal disodium chromoglycate could block the change in nasal airway resistance induced by platelet activating factor (PAF). Placebo or active drug was given for 3 days before intranasal challenge with PAF. Nasal airway resistance before and at intervals after challenge was determined with a rhinomanometer. Pretreatment with disodium chromoglycate blocked the decrease in nasal airway resistance induced by PAF. This indicates an alternative mode of action of disodium chromoglycate in the treatment of allergic airway diseases.

Methods for evaluating the taste of paediatric formulations in children: a comparison between the facial hedonic method and the patients' own spontaneous verbal judgement.

The aim of the study was to evaluate two essentially different methods of assessing differences in children's taste preferences with regard to five different paediatric penicillin formulations. The study was performed with a parallel group design comparing five groups. A taste evaluation was recorded in 103 children with upper respiratory tract infections after a single therapeutic test dose. First the patient's own spontaneous verbal judgement was recorded then a judgement was arrived at using a hedonic scale of facial expressions. Both judgements were passed immediately and also 3-4 minutes after the test dose. In the children who were 6 years of age or younger, a better discrimination of taste differences between formulations was achieved by using the patients' own spontaneous verbal judgements instead of the facial hedonic method. Both methods seem appropriate in older children, but the hedonic scale is preferable since its use implies a more standardised procedure.

Effect of platelet activating factor on the human nasal mucosa.

In order to determine the effect of PAF-acether on the vascular bed in the human nose, 11 healthy volunteers were studied. Rhinomanometry was performed to determine the effect on the capacitance vessels, and the 133Xe wash-out method enabled determination of the mucosal blood flow, regulated mainly by the resistance vessels. These measurements were made before and after topical administration of PAF-acether in three different doses; 0.26 micrograms, 2.6 micrograms and 26 micrograms per nostril. The results indicate that PAF-acether induces a dose-dependent vasoconstriction of the capacitance vessels and a reduction in mucosal blood flow. That PAF-acether should be a principal mediator in the allergic reaction in the human nose is not supported by these findings.

Acute epiglottitis treated by intubation.

Acute epiglottitis is a severe disease which is especially common in children and may be fatal if not treated. The main problem in these patients is to secure a free airway. During the last decade there has been much debate as to whether one should perform a tracheotomy or secure the airway by means of long-term intubation. During the last 12 years we have treated 86 patients with acute epiglottitis by long-term intubation to secure the airway. Three of these patients have been tracheotomized. All patients have been investigated by direct laryngotracheoscopy about 3 months after recovery. During the last 8 years we have had no complications whatsoever and we consider long-term intubation to be a safe and convenient way of securing the airway in acute epiglottitis.

Patients with abdominal trauma and fatal outcome: analysis of a 30-year series.

Patients with blunt abdominal trauma and fatal outcome during 30 years in a well-defined rural Swedish area have been analyzed. There were 127 patients. Several facts indicate that more severe trauma has been appearing more often during this period The mortality rate has, however, been stable, but the patients have become older. More patients die from pulmonary complications than from the trauma itself. One very important development is the significant decrease in mortality among children.

Abdominal injury from sporting activities.

The frequency of abdominal injury is rising. In an analysis of 1,354 cases of closed abdominal trauma sustained during a 30-year period (1950-1979) in Skaraborg County, Sweden, a distinct increase was found in the numbers associated with sporting activities. The severity of the injuries caused by sports likewise showed an increase. The representation of injured organs was of the same order as in the total series of closed abdominal injuries. An analysis of the 136 cases of sports-associated injury is presented with regard to cause and type of the injuries, diagnosis and prognosis.

A randomized trial of intranasal beclomethasone dipropionate after polypectomy.

Beneficial effects of intranasal beclomethasone dipropionate (Bdp) in patients with nasal polyposis have been reported earlier. This study was carried out to investigate whether long-term treatment with Bdp after polypectomy could prevent formation of new polyps and reduce the number of surgical removals. Forty consecutive patients without laboratory or other clinical signs of allergy but with severe nasal polyposis were included in the study. Twenty patients were treated with intranasal Bdp and twenty patients received no treatment after polypectomy. All patients were followed for at least 2.5 years. The size of the polyps that recurred was estimated at different time-intervals by the examining doctor. After six months there was already a significant difference in favour of the group treated with intranasal Bdp. Further results of the study and the clinical implications are discussed.

Abdominal trauma in persons older than 60 years.

Abdominal trauma occurring in persons older than 60 years over a 30-year period (1950-79) in a well defined region of Sweden was reviewed. The 177 patients comprised 12.5% of the total with abdominal trauma during that period. Road traffic accidents were the main cause of trauma (48% of cases). The incidence of motor-car accidents rose sharply during the first two decades of the study. Injuries to the liver and bile ducts and multiple intra-abdominal injuries were more common in the elderly than in younger patients, but injury to the abdominal wall was less common. A tendency towards more severe injuries and multiple trauma with extra-abdominal involvement emerged during the study period. The mortality rate was 27.6% in the patients older than 60, but 9.1% in the overall patient series with abdominal trauma. One-third of all the deaths among the older patients were directly due to the abdominal trauma. One consequence of the increasing numbers of old people in the general population is probably that more and more cases of abdominal trauma will be seen in the higher age groups.

Abdominal trauma during thirty years: analysis of a large case series.

Studies have been made in 1407 patients of the causes, the organs involved and the outcome of injury to the abdomen in patients needing admission to hospital in an area of Southern Sweden, between 1950 and the end of 1979. The proportion of female patients and those aged over 60 increased significantly. The seasonal distribution of the injuries showed significant change, with a drop in the initially high frequency sustained during the summer months. Penetrating injuries were rare, but increased in the 1970s. Road traffic accidents as the cause of abdominal injuries rose to a maximum of 56 per cent in the late 1960s. The numbers of injured organs and the frequency of other associated injuries rose gradually until the mid-1970s, after which there was a slight decrease. The spleen, liver and large blood vessels were the organs which were increasingly often injured. The annual incidence of various visceral injuries per 100 000 population was calculated. The number of patients with a delay of at least 24 hours before operation fell significantly and there was a tendency to shorter hospital stay. The mortality curve showed a peak in the late 1960s.

Intestinal trauma, Analysis of 101 cases.

Intestinal injuries sustained by 101 Swedish patients during the period 1950-1979 are reviewed. The abdominal trauma was blunt in 78 cases and penetrating in 23. Small-bowel and mesenteric, but not large-bowel, injuries showed increasing frequency. This was associated with rising numbers of motorcar accidents. Injuries to other abdominal organs were found in 56 of the 101 patients, particularly in those with mesenteric or large-bowel trauma. Clear physical signs of intra-abdominal injury led to rapid surgical exploration in most cases, but some operations were performed after relatively long observation. Most injuries, including those in the colon, were treated with primary repair or resection. Decompressive colostomy or exteriorization were rarely performed. Deaths were mainly caused by other factors than the type of operation. The mortality rate (35%) was constant.

Upper gastrointestinal trauma. Analysis of 45 cases of gastric, duodenal or pancreatic injury.

Injuries to the upper gastrointestinal tract occurring over a 30-year period in a geographically well defined region were analyzed. The incidence was low in comparison with other abdominal injuries, but duodenal and pancreatic injuries were increasingly common in the study's last decade. The stomach injuries were mainly caused by penetrating trauma. The duodenal and pancreatic injuries most commonly resulted from road-traffic accidents. Two duodenal injuries were overlooked at the initial exploration, in which the duodenum was incompletely exposed. Anastomotic insufficiency occurred in one of the three patients who underwent duodenal resection. Most of the pancreatic injuries were contusions that could be managed with drainage. The pancreas was severely damaged in five patients, all of whom had multiple intra-abdominal injuries. Three of the five patients died before or during operation.

Peroral chromones. A new way to treat allergic rhinitis?

Disodium chromoglycate (DSCG) is well documented in the topical treatment of allergic rhinitis. Its use does not lead to any major side effects. FPL 57579 is a new chromone compound which is well absorbed after oral administration. In a clinical trial, patients with allergic rhinitis underwent a nasal challenge before and after ingestion of FPL 57579. the effect on nasal airway resistance (NAR) was determined by rhinomanometry. In all cases there was a smaller increase in NAR after the intake of FPL 57579.
