Bioavailability of metronidazole in fasting and non-fasting healthy subjects and in patients with Crohn's disease.

The possible influence of food intake on the bioavailability of metronidazole was examined in ten health volunteers by administration of a single dose of metronidazole on an empty stomach, and with a standardized breakfast. Food intake did not significantly alter the bioavailability of metronidazole. The interindividual variation in bioavailability appeared to be slight. In nine patients with Crohn's disease, the absorption of metronidazole appeared to be reduced and to be more variable than in healthy subjects. In both groups there was a clear relationship between the amount absorbed and dose/kg body weight. Thus, from the pharmacokinetic point of view, metronidazole can safely be given either with or between meals. The dose should be related to body weight.

Sisomicin treatment of serious neonatal infections. A clinical and pharmacokinetic study.

23 infants, 20 of which had verified or clinically highly suspected serious infections in the neonatal period, were treated with a new antibiotic aminoglycoside, sisomicin, i.m. in doses from 2.8 to 6.6 mg/kg/24 h. Clinical cure was obtained in 18 of 20 caes, and marked improvement in one case. Adverse effects were only observed in two infants with tenderness at the injection sites. Serum concentrations and half-life estimations showed that the concentrations were similar to those obtained for gentamicin, and that half-life was independent of postnatal age, but highly correlated to the gestational age and to body weight. Consideration should therefore be given to the prolonged half-life of the drug in immature and low birth weight infants.

New method for the serological grouping of Streptococci with specific antibodies adsorbed to protein A-containing staphylococci.

A coagglutinating reagent has recently been developed and utilized in capsule typing of pneumococci. The reagent consists of stabilized staphylococci coated with specific antibody via the gamma globulin Fc-protein A reaction thereby orienting the antibody molecules with the antigen-reactive Fab parts outwards. Reagent staphylococci coated with antibodies directed against streptococcal group-specific antigens have been used in the present investigations. Overnight cultures of streptococci were treated with trypsin and then directly tested for coagglutination of selected reagent staphylococci on glass slides. In all, 179 strains of streptococci were analyzed by the method described. The results were compared with grouping by using Lancefield extracts in counter-immunoelectrophoresis and were shown to agree completely. The coagglutination method described proved to be accurate, rapid, and simple.
