[Reserves for enhancing the efficiency of treatment for pulmonary tuberculosis in adolescents].

Out of 222 adolescents with pulmonary tuberculosis, 115 received a short-term intensive DOTS chemotherapy and 107 had a conventional triple antibacterial treatment (TAT) regimen. The advantages of intensive chemotherapy were relatively better results with a reduction in the duration of treatment by 1.5-2 months; its disadvantage was more frequent (37.7% versus 18.7%) adverse toxic reactions. The local administration of TAT (lymphotropic as aerosols) promotes a decrease in toxic reactions and an increase in the efficiency of treatment for the destructive and bacillary forms of pulmonary tuberculosis. This is also favored by the timely use of collapse therapy--artificial pneumothorax and pneumoperitoneum (following 1.5-3 months).

[Impact of the area burden of agrochemicals on tuberculosis morbidity and mortality].

The rate of use of pesticides and mineral fertilizers in the Republic of Daghestan was high in the USSR and Russia and was many times greater than the critical burden (3/93 kg/ha) and ranged from 3.5-4.2 kg/ha in the highlands to 17.0-21.7 kg/ha in the plain. Two-factor analysis indicated that out of the mineral fertilizers, pesticides and phosphate fertilizers had the most adverse effect on the incidence of tuberculosis whereas potash fertilizers exerted a less effect. Phosphate fertilizers and copper-containing pesticides were secondary risk factors of death from tuberculosis. The correlation coefficient was 0.53 for phosphate fertilizers, 0.59 upon total exposure to organochlorine compounds, and 0.68 upon total exposure to copper-containing pesticides.

[Antituberculous measures according to the results of Mantoux test].

Analyzing the results of Mantoux tests with 2 TE made 7 times for 5 years in 750-850 schoolchildren of the settlement of Sulak has demonstrated that the infection rates are on the increase on the one hand, and, due to material and transport problems, the proportion of examinees from a risk group decreases to 10-30% of those to be examined, on the other hand. The authors suggest that the remaining unexamined children should undergo chemotherapy with 2-3 antituberculosis drugs for 3 months so as to perform an examination when the first opportunity occurs. They propose to examine the coverage of children, except for children and adolescents with a hyperergic and increasing reactions and those with a highly positive (15-16 mm) Mantoux test. An algorithm for implementation of the above measures is offered.

[The typical structure of Mycobacterium tuberculosis and its relationship with an epidemic process and epizooties in Daghestan].

When Mycobacteria tuberculosis isolated from the sputums of patients with pulmonary tuberculosis in Daghestan were typed, the proportion of the bovine type mycobacteria was found to 20.5% in 1977-1978; that was 8.9% after long-term health-promoting measures in 1989 and 24.8%; after a decade of decreased measures in 1997-1998. The epidemiology and epizooty of tuberculosis in the region were closely interrelated and intensified each other. The proportion of bovine mycobacteria isolated from patients with tuberculosis is one of the sensitive indices of the epidemic situation in the region. Tuberculosis control measures in cattle-breeding regions should be implemented in a close contact and interaction of health care and veterinary facilities.

[Semeiotics of abdominal tuberculosis].

Examination of 119 patients with abdominal tuberculosis permitted the description of the characteristic semiotics of the illness. Today the patients with abdominal tuberculosis are mainly women of child-bearing age with a long-term tuberculosis catamnesis and intoxication, with a history of tuberculosis of different sites, those suffering from tuberculosis or its sequels at present (64%), those with pains (94%), discomfort or swelling of the abdomen (79%), malfunction of the gastrointestinal tract (65%), weight loss (86%), malnutrition (72%), anemia (63%), not infrequently with inflammatory induration (43%) or ascites in the abdominal cavity (39%). In addition to this characteristic semiotics, the patients with abdominal tuberculosis may demonstrate the most different and unexpected symptoms up to acute abdomen (23%). To make differential diagnosis of abdominal tuberculosis, one has often to resort to diagnostic laparotomy, laparoscopy, Koch's test and to trial therapy.
