Cytology of papillary adenoma of the nipple: a case diagnosed on fine-needle aspiration.

A 45-yr-old woman being treated for carcinoma of the uterine cervix was found to have a right nipple nodule. Fine-needle aspiration of the nodule showed a very cellular smear with clusters of ductal cells and many scattered naked nuclei. Papillary clusters with and without a vascular core also were present and enabled a cytological diagnosis of papillary adenoma. Excision of the lesion demonstrated the histopathologic features of a papillary adenoma of the nipple.

Thyroid tumors: cytomorphology of medullary, clinically anaplastic, and miscellaneous thyroid neoplasms.

Of 2,012 fine-needle aspirations (FNAs) of the thyroid performed between the years 1984 and 1988, detailed cytomorphologic analysis of 95 cases diagnosed as neoplastic on histology and/or cytology and those that received an equivocal cytodiagnosis are presented in this article. Discussed are medullary thyroid carcinomas (nine cases), clinically anaplastic thyroid tumors (CATT; eight cases), two cases of non-Hodgkin's lymphoma (NHL), and one primary leiomyosarcoma of the thyroid, for a total of 20 cases. Included in the category of CATT are all the thyroid tumors presenting clinically with an anaplastic growth pattern. The cytomorphology of these tumors varied, but the giant- and spindle-cell pattern was predominant. An accurate cytodiagnosis was possible, as per cytohistologic correlation, in seven cases, while in one case histological material was not available for study. Medullary carcinoma of the thyroid (MCT) showed a mixed spindle-cell and round-cell population in eight cases and an entirely spindle-cell population in one case. All cases of MCT were correctly diagnosed on cytology, and amyloid could be demonstrated in the cytologic smears in three cases. The cases of NHL and leiomyosarcoma could also be correctly interpreted on cytology.

The emergency petition process in Maryland.

Maryland's Emergency Petition statute allows a violent or suicidal person with a mental disorder to be brought to an emergency facility for rapid evaluation regarding the need for emergency treatment. Although many states have similar laws, little has been written in the psychiatric literature about the emergency petition process. The investigators evaluated emergency petition documents, demographic data, and the adequacy of emergency room records for all patients brought to a large county hospital in Prince Georges County, Maryland, by emergency petition during a one-month period. All emergency petition patients in Prince Georges County are brought to this hospital site. Of 94 petitioned patients examined during the study period, 92 records were available for review. The emergency petition was found to meet appropriate legal criteria in 94 percent of cases. More than half of all patients evaluated were intoxicated on alcohol or illicit drugs, and the majority of these patients were released from the emergency room as no longer dangerous after their acute intoxication resolved. In contrast to previous studies most of the patients evaluated were affluent, had health insurance, and were employed.

Ventricle-brain ratio, computed tomographic density, and brain area in 50 schizophrenics.

A sample of 50 DSM-III-diagnosed schizophrenics (mean age, 34 years) intentionally biased to contain a relatively high proportion of persistently unemployed persons was compared with a sample of 87 normal volunteers on three computed tomographic measures. These were lateral ventricle-brain ratio, regional brain computed tomographic density values, and brain slice area. Images were made with the same computed tomographic scanner and identical scan parameters. Computed tomographic data were assessed blindly using a computer-linked image array processor and electronic planimeter. Ventricle-brain ratios were significantly higher in schizophrenics, with 28% of the patient sample exceeding 2 SDs of the control mean. Brain area measures were not associated with an enlarged ventricle-brain ratio. Contrary to our prediction, ventricular enlargement was unassociated with most negative symptom ratings, but was correlated with the absence of positive symptoms. A history of abnormal delivery and the presence of left-handedness were significant predictors of an enlarged ventricle-brain ratio on multiple regression analysis. Schizophrenics had a significantly smaller brain slice area compared with normal controls, a finding not attributable to height differences between groups. Brain slice area was inversely correlated with computed tomographic brain density across all subjects. After correction of computed tomographic density values for area using a linear regression model, no significant regional density differences were detectable between normal controls and schizophrenics. Within normal controls there was a significant relationship between social class and brain slice area, but not ventricle-brain ratio.

Sarcoidosis diagnosed on fine-needle aspiration cytology of salivary glands: a report of three cases.

Three female patients, two presenting with bilateral parotid gland enlargement and one with bilateral submandibular gland enlargement, were subjected to fine-needle aspiration cytology (FNAC). Smears showed noncaseating epithelioid cell granulomas with or without giant cells and salivary gland acini with varying degrees of degenerative changes. After excluding other granulomatous lesions, sarcoidosis was suggested and was subsequently proved in all three cases. FNAC, therefore, may be considered a useful diagnostic modality in cases of sarcoidosis presenting primarily with head and neck involvement.

Retroperitoneal fibrosis due to tuberculosis, Aldomet and Daonil.

Two cases of retroperitoneal fibrosis are discussed. The first patient was a known hypertensive and diabetic and had been on Aldomet and Daonil for a long time. The second patient was a tuberculotic and is improving on antitubercular therapy. Retroperitoneal fibrosis due to the above-mentioned drugs and tuberculosis are relatively unknown.

Increased ventricle-to-brain ratio in late-onset schizophrenia.

For 29 patients with schizophrenia that began after age 44, the mean ventricle-to-brain ratio was significantly higher than for 23 age-matched normal subjects but significantly smaller than for 23 patients with Alzheimer's disease and hallucinations or delusions.

Relapse in chronic schizophrenics treated with fluphenazine decanoate is associated with low serum neuroleptic levels.

Seventy-three chronic schizophrenic outpatients were studied for 1 year to determine if neuroleptic drug levels could be useful in preventing relapse. All patients received fluphenazine decanoate i.m., and serum level of neuroleptic was determined by radioreceptorassay. Clinical improvement was measured using the Global Assessment Scale. Results indicate that serum neuroleptic drug levels may be useful in the outpatient management of chronic schizophrenics.
