Intrasegmental proprioceptive influences on the period of the swimmeret rhythm in crayfish.

When the swimmerets of decapods beat, they do so because the muscles of each swimmeret are driven by a series of periodic bursts of impulses in its motor neurones. We investigated the effects of proprioceptive feedback on the period of this motor pattern by interfering with the movement of particular swimmerets. In different experiments, we observed three different kinds of results during interference with a swimmeret. Either the period decreased, or it did not change, or bursting was inhibited altogether. These different results are discussed in terms of the connectivity of different command fibres.

[Acute arterial obstructions and emergencies in vascular surgery (author's transl)].

Acute arterial obstruction due to embolism or thrombosis require surgery within 6 hours in case of complete ischaemia. The removing of the occluding material can be done in every hospital under local anaesthesia using a Fogarty-catheter. Reconstruction of injured arteries must be accomplished immediately after preliminary haemostasis by local compression in a surgical department with a special equipment for vascular diagnosis and surgery. The rupture of an aneurysm of the abdominal aorta signifies a poor risk for the patient. To obtain a chance quick diagnosis, shock-therapy and an immediate transport into a specialized hospital for surgery are indispensable.

[Total and organically bound mercury content in fishes from German fishing grounds (author's transl)].

In a survey sponsored by the Ministry of Food Agriculture and Forestry of the Federal Republic of Germany the mercury contamination of 759 specimens of fishes from the German fishing grounds were analysed. The determinations were performed by flameless atomic absorption (cold vapour system). -We found for the fishes of greatest economic importance herring, cod, saithe, haddock very low averages of less than 0,1 ppm. The legal limit of 1 ppm was only reached or exceeded by older fishes (ling 1.18 ppm, red-fish 1.09 ppm) and by specific kinds of fishes (shark 3.23 ppm, ray 1.69 ppm, tuna 0.80 ppm). -The organically bound mercury (methyl-Hg) which is considered especially toxic, has been determined by gas-liquid-chromatography. Methyl-Hg constitutes about 70--98% of the total mercury, a value which is in agreement with Japanese and Swedish reports on this subject.

[Stimulation of H+ secretion and serum gastrin by intra-operative electrical vagal stimulation before and after proximal selective vagotomy (author's transl)].

H+ secretion and gastrin concentration were measured before and after electric vagal stimulation during proximal selective vagotomy in 29 patients with duodenal ulcers. Before vagotomy, H+ secretion and serum gastrin concentration significantly rose after stimulation, while after complete vagotomy H+ secretion remained below basal values, although serum gastrin concentration was significantly increased. These results indicate that without vagal innervation there is no gastrin secretion within physiological levels. The method is suitable for testing the completeness of the vagotomy, the results not agreeing with those obtained by pressure measurements (Burge's method).

[Proximal selectivive Vagotomie. Intra-operative Vollständigkeitskintrolle].

In the treatment of peptic gastroduodenal ulcers the modified vagomotor electrotest according to Burge has yielded remarkably good results as an intraoperative test for the completeness of proximal selective vagotomy (PSV). The test is as useful for the surgeon in training as for the expert in vagotomy. Moreover the clinical value of the test is demonstrated. If, according to this test, vagotomy is complete, an extreme reduction of gastric acid and a very low rate of relapsing ulcers are seen in our patients. We cannot recommend performing the PSV without this test.

145 days survival of calf with total artificial heart (TAH).

A calf with a biolized TAH was maintained in reasonably good physiological condition for a major portion of a 145-day experiment as indicated by near normal growth rate and laboratory data. The mechanism for the high CVP and CBV at the beginning and end of the experiment were correlated with protein metabolism. No evidence of thromboembolism was found at autopsy and only modest congestion with minimal parenchymal damage was observed. The cardiac prosthesis itself was free of thrombus, although slight calcification was found on the valves and diaphragms. The pump performance, however, remained satisfactory. In addition, endothelialization appeared to be starting on a portion on the inside of the pump.

[Surgical aspects in arterial occlusion (author's transl)].

Acute arterial occlusion was found to be the most frequent emergency (10%) among vascular patients of the Department of Surgery of the University of Düsseldorf. Rheumatic heart diseases receded as etiological factors whereas atherosklerotic obstructive diseases exhibited a clear increasing tendency. Operation is urgent if acute ischemia is present. The best method is represented by embolectomy by means of the Fogarty-catheter, if it is performed within the first 6 to 12 hours after or vascular occlusion. Angiography is only indicated if ischemia is not complete or if the cartoid artery or great visceral arteries are involved. In cases of incomplete occlusion embolectomy may be successful even after days and weeks. Mortality after these operations is found to be 18 to 20%, the rate of amputation being 9%. The final results of operative results are better in arterial embolism than in arterial thrombosis.

[Spontaneous rupture of the oesophagus (author's transl)].

During the last 4 years three so-called spontaneous perforations of the oesophagus were treated, twice by surgical intervention 12 anys after the rupture. Two patients survived. The classical history of retching or vomiting and retrosternal splitting pain is indicative, wht medium, amidotrizoate sodium (Gastrografin), from the oesophagus. Prognosis depends decisively on the time of operation after the rupture. Direct suture of the rupture with plastic coverage of the defect by sewing on of the gastric fundus or by plication of the fundus have proved valuable.
