[Echocardiographic monitoring of atrial septostomy in the transposition of the great arteries].

In seven consecutive cases of transposition of the great arteries balloon atrial septostomy was performed under the sole echocardiographic guidance. The catheter, introduced into either the saphenous, femoral or umbilical vein, was advanced into the inferior vena cava, then into the right atrium, and, through the patent foramen ovale, into the left atrium. The correct location of the tip of the catheter within the left atrial body was easily identified by two-dimensional echocardiography, with a four chamber view from the subcostal approach. Under echocardiographic control alone the inflated balloon was then suddenly withdrawn into the right atrium according to the Rashkind's technique. The procedure was repeated 3 to 5 times. The balloon was inflated with 2 to 3.5 ml of saline solution. The septostomy was deemed to be adequate when the echocardiographic images revealed an atrial septal defect at least 5 mm in size and an increased flapping motion of the inferior rim of the atrial septum. With this technique the risks of complications due to an incorrect location of the catheter are minimized and the outcome of the procedure can be immediately evaluated by measuring the size of the atrial septal defect.

Age-related differences in kinetics and side-effects of viloxazine in man and their clinical implications.

Dose kinetics and side effects of viloxazine (VLX) in 16 healthy volunteers (range age 25-90 years) were studied after single oral administration of 200 mg of VLX. Significant differences in peak plasma values (P less than 0.01), t1/2 (P less than 0.01) and Cl/F (P less than 0.05) were found between subjects under 50 and over 60 years. Positive correlations were found between age and peak plasma values (P less than 0.05), age and AUC (P less than 0.01). No correlations were found between age, t1/2 and Cl/F, due to the high interindividual variability in pharmacokinetic profiles. Total reported and observed side effects scores were higher overall in subjects under 50 years than over 60 years and were inversely correlated to AUC (P less than 0.05). Drowsiness was inversely related to the age of subjects (P less than 0.05). Our data support the importance of single dose kinetic studies, particularly for new antidepressants, in relation to age, both to emphasize differences in pharmacokinetic profiles and to predict side effects during chronic treatment.

Tetralogy of Fallot with absent pulmonary valve: definitive diagnosis by two-dimensional echocardiography.

A case is presented of a 27-day-old boy with Tetralogy of Fallot and absent pulmonary valve. The two-dimensional echocardiographic examination demonstrated: subaortic ventricular septal defect, overriding aorta, absence of the pulmonary valve with restrictive anulus, and massive dilatation of the pulmonary artery. These anatomic details were then confirmed by angiography and surgery. The echocardiographic features of Tetralogy of Fallot with absent pulmonary valve are typical, and allow the correct diagnosis to be made noninvasively. The haemodynamic investigation becomes obviously easier, and angiocardiography may be devoted to the study of possible additional defects, that are difficult to detect by echocardiography, like multiple ventricular septal defects, or peripheral pulmonary branch anomalies.

[Atrioventricular connection anomalies associated with Fallot's tetralogy: bidimensional echocardiographic study].

From January 1979 to December 1981, 62 patients with Tetralogy of Fallot were studied by two-dimensional echocardiography (2-D Echo). In 10 cases various types of atrioventricular connection anomalies were found: type C atrioventricular canal in 8 cases, straddling tricuspid valve in 1 case, straddling and overriding tricuspid valve in 1 case. These associated anomalies usually are not suspected by means of clinical, electrocardiographic and radiological findings, whereas 2-D Echo appears to have a unique diagnostic value. Apical and subcostal four chamber views were the most helpful to detect the anomalies of the atrioventricular junction; in all cases with complete atrioventricular canal, the ostium primum atrial septal defect, the common atrioventricular valve and the inlet septal defect were clearly seen. In one case an overriding tricuspid valve was clearly demonstrated; in this and in another patient the straddling of the tricuspid valve could be only suspected, whereas its confirmation was obtained subsequently by angiography and surgery. A through 2-D echo evaluation of these anomalies is useful to select the most appropriate views for angiographic confirmation and to ensure optimal surgical planning.

Influence of age on mianserin pharmacokinetics.

The pharmacokinetics of Mianserin (MIA) after acute administration have been studied in nine volunteers, divided into two groups according to age. The subjects were given a single oral dose of 30 mg MIA. The plasma peak time was shorter in the younger subjects than in the older. In general, the concentrations of MIA in the plasma were higher in the older subjects than in the younger, and in the former group there was no relationship between administered dose (mg/kg) and plasma levels. The area under curve, volume of distribution and clearance were significantly different in the two groups. The side effects (both incidence and type) differed in the two groups. There were no significant changes in blood pressure, either supine or standing. The sedative effect was more marked in the young than in the elderly subjects. A relationship between drowsiness and MIA plasma levels was observed only in the younger subjects.

[Symptomatic vascular rings in infancy. Surgical treatment in 19 patients (author's transl)].

The Authors report their experience in the surgical treatment of 19 patients under two years of age (75% under six months), with severe symptoms for the presence of a complete or incomplete vascular ring. Twelve patients had double aortic arch, two had a right aortic arch with a left ligamentum arteriosum, four a symptomatic retroesophageal right subclavian artery and one an abnormally placed innominate artery. Symptoms, methods of diagnosis, indications to the surgical treatment, surgical techniques and late results are considered. The Authors remark that even if correction of these rare congenital malformations may be simple and with a low operative mortality, many problems exist in the early diagnosis and in the post-operative treatment of these patients.

Alien H-2 antigens on a chemically induced fibrosarcoma: further evidence in crude membrane and soluble extracts of the tumor.

Crude membranes (CM) were obtained from in vivo subcutaneous nodules of the methylcholanthrene-induced BALB/c fibrosarcoma C-1 by forcing tumor fragments through a mechanical press and subsequent differential centrifugation. This immunogenic tumor has been previously shown to express both H-2d and extra H-2k-like antigens. Original H-2d and alien H-2k antigenic activities were present in CM C-1 as judged by the specific inhibition of the C'-dependent cytotoxicity of monospecific H-2 alloantisera on normal 51Cr-labelled lymphoid cells. Both K- and D-end private H-2d antigens (31 and 4), and H-2d public antigens 8, 29, 35 were detected in CM C-1. In addition, the alien H-2Kk.23 private specificity and the public H-2k.1, 5, and 25 were also found in CM C-1. A weak but reproducible activity attributable to the Dk private antigen 32 was also revealed in this material. A hierarchy in the expression of both H-2d and H-2k specificities was evident in CM C-1 which paralleled, although with an overall lower antigenic activity, those of two other BALB/c (H-2D) FIBROSARCOMAS AND OF A C3Hf (H-2k) lymphoma, respectively. CM from normal BALB/c and C3Hf spleens, while expressing higher amounts of all the tested H-2 antigens, displayed a hierarchy of the different specificities similar to that of neoplastic tissues. Crude soluble (CS) material was obtained from CM C-1 by deoxycholate treatment and was tested in the inhibition assay for the presence of H-2d and alien H-2k antigens. Only specificities with the highest expression in CM were found in CS, i.e. H-2.4 and 29 for H-2d and H-2.25 for H-2k. Both CM and CS from C-1, but not from another control BALB/c sarcoma, were able to significantly inhibit the activity of an oligospecific serum to the Kk-coded antigens.

Biochemical characterization of alien H-2 antigens expressed on a methylcholanthrene-induced tumor.

A methylcholanthrene-induced tumor of BALB/c (H-2d) origin had been shown to express H-2 antigens normally associated with the H-2k haplotype. The molecules expressing the alien antigens have been partially purified and shown to be distinct from molecules expressing normal H-2d antigens. Like normal H-2 antigens, the alien antigens have a molecular weight of 48,000, are glycoproteins, and are noncovalently bound to beta 2-microglobulin. The alien antigens differ from normal H-2 antigens in their susceptibility to papain digestion, behavior during gel filtration, and overall stability. Possible mechanisms accounting for the expression of the alien antigen are discussed.

The Senning operation for transposition of the great arteries.

The original Senning technique of interatrial transposition of venous return was selectively employed for physiological correction in 24 patients with transposition of the great arteries (TGA) ranging in age from 74 days to 26 months (median 7 months). Twenty-three had intact ventricular septum and one had a large ventricular septal defect and diaphragmatic subpulmonary stenosis. There were no hospital or late deaths, and at follow-up examination 1.5 to 12 months postoperatively, each patient was asymptomatic, in normal sinus rhythm, and had no clinical evidence of caval or pulmonary venous obstruction. Nine patients underwent cardiac catheterization and angiocardiographic studies between 1.3 and 9 months postoperatively which demonstrated widely patent venous pathways and effective "left" and "right" atrial contraction. This experience lends support to the continued use of the Senning technique for interatrial transposition of venous return.
