Deviated nose: technical proposition for prevention of recurrences.

After considering previously published studies and accordant with their observations during operations, the authors propose a surgical technique for the treatment of the deviated nose. Subpericondrial liberation of the upper lateral cartilages on the concave surface, with regard to the deviation, helps to prevent recurrences. After illustration of the surgical technique, some representative clinical cases are presented.

[Dental and skeletal malformations: 434 cases of II and III dental skeletal class. Our experience and review of the literature].

BACKGROUND: This study tests the patients who underwent to orthognatic surgery to compare the perioperatory treatment and the results with what related by international bibliography. METHODS: We have made a retrospective analysis on 434 patients affected by dentalskeletal malformations and surgically treated at the Department of Maxillo-Facial Surgery of the University of Rome "La Sapienza", from 1-1-1988 to 31-12-1997. We have examined these data: clinical evaluation of patient, pre and post-surgical orthodontic treatment, minimum age for surgical correction, surgical technique, use of osseus-synthesis with wire or RIF for mobile segment osseus stabilization, use of reference articular plaque and the possible association of additional surgical technique during the same time of maxillomandibular correction. We have made in all patients orthognatic surgery, which consist in osteotomy Le Fort I for upper jaw and sagittal osteotomy for mandible. RESULTS: In 25 patients was necessary remove the plaques and the screws which have made an infection. In 15 patients we have executed a subsequent operation for relapse of the dentalskeletal malformation. In other patients we have got a good functional and aesthetical result. CONCLUSIONS: From our study result that an exact presurgical analysis and a correct orthodontic-surgical treatment have permitted to obtain a good functional and aesthetical result in all patients.

[Lymph node metastasis of adenoid cystic carcinoma].

BACKGROUND: The adenocystic carcinoma (ACC) of the head and of the neck is rather uncommon. It is characterized by a very slow biological behaviour, which leads to locoregional recurrences and distant metastasis. Most reports concentrate on the unpredictable long-term behaviour of this tumor, whereas lymph nodes metastasis are not usually discussed in detail. The aim of this study was to evaluate the difficulties connected with the lymph node metastasis in ACC. METHODS: A group of 33 patients from 1980 to 1998, was observed. These patients have been subjected to surgical treatment in the Maxillo-Facial Surgery Department of the Policlinico-Umberto I of Rome. RESULTS: Among 33 patients with ACC, 10 patients (30%) revealed, initially or during observation, cervical lymph node metastasis, which is a most common event in male subjects (70%). The lymph node involvement is much more common in carcinomas localized in the parotid and in the sublingual glands with a solid rather than a tubular histologic pattern. Direct correlation between age and metastasis lymph node involvement have not been documented. CONCLUSIONS: Surgery for node disease has a very little impact on the locoregional and on the distant metastasis evolution of the tumor. Thus, our protocol of treatment of lymph nodes metastatic lesions follows the trend of most of the international authors, considering that the neck dissection should be performed only with a lymph adenopathy clinically evident or strongly suspected.

Central giant cell granuloma of the palate; unusual localisation in a five year old child.

The central giant cell granuloma of the maxillo-mandibular region is a relatively uncommon lesion (3.5%-0.1%). Essentially, it occurs in the second decade of life and it is mainly located in the mandibular region. The female/male ratio is 3:1. The authors present an unusual localisation of central giant cell granuloma in a five year old child's maxillary bone. After a surgical curettage of the lesion, an eighteen months follow-up examination did not show any recurrence.

Temporomandibular region in the Franceschetti's Syndrome. Anatomical study.

Franceschetti's syndrome is a rare, non-fatal, hereditary malformation, usually bilateral, which symmetrically affects orbits, mandible and ear. The authors propose an anatomical description of the temporomandibular region after the dissection of a newborn baby suffering from Franceschetti's Syndrome, dead soon after the birth. A discussion on the different etiopathogenical theories is made. The authors conclude that an alteration of the development of nerve trigeminal branches is the cause of the malformations.

[The use of an intermediate splint in surgical correction of facial asymmetry].

Facial asymmetries often presents maxillary and mandibular involvement. The surgical repositioning of the maxilla in three planes of space is a must that guarantees the achievement of good function and skeletal symmetry. The Authors, looking for a more precise and correct way to quantify the maxillary movements, suggest to use an acrylic splint that guarantees the reproduction of the skeletal movements decided during the treatment planning procedures. The technique proposed, that is based on a very precise work during the model surgery, offers, as reported by the authors, good results and a significant reduction in surgery time.

Considerations on a case of intranasal cerebral ectopia.

The Authors report a case of intranasal cerebral ectopia; the problems concerning the nosographic and diagnostic definition of this condition as well as the differences from other similar pathologies with the same location (such as meningoencephalocele and nasal glioma) are discussed. Particular attention is payed to the various surgical techniques that can be used in these cases.
