[Preventive lymphadenectomy in cutaneous malignant melanomas. A matched pair study].

Data on 70 patients with melanomas classified as clinical stage I, who had been treated surgically with wide excision (n = 49) or wide excision plus prophylactic lymph node dissection (n = 21) at the University Dermatology Clinic, Freiburg i. Br. between 1975 and 1977, were reviewed in a retrospective study. The effectiveness of elective lymph node dissection was examined by creating 21 pairs of patients, matched according to sex as well as to the level, location and thickness of the tumors (to within +/- 5%). One patient in each pair underwent wide local excision and prophylactic lymph node dissection (WE + PLND) to others did not (WE only). Depending on tumor thickness there was a better chance of survival for the PLND patient group. However, these differences were not statistically significant.

Association of 239Pu with lysosomes in rat, Syrian hamster, and Chinese hamster liver as studied by carrier-free electrophoresis and electron microscopic autoradiography with 241Pu.

The binding of injected monomeric plutonium in the liver of rats, Syrian hamsters, and Chinese hamsters (species which show profound differences in their ability to eliminate 239Pu from the liver) was investigated by carrier-free electrophoresis using 239Pu and electron microscopic autoradiography with 241Pu. These studies are part of a program designed to obtain a better understanding of the mechanisms of the clearance of transuranium elements from liver of different mammals and man. Between 4 and 9 days after nuclide injection, a clear correlation between the majority of the 239Pu and lysosomal enzymes was observed when the mitochondrial-lysosomal (ML) fraction of the livers was analyzed by carrier-free electrophoresis. In the two hamster species, a second 239Pu peak exists from the beginning and increases with time to comprise 50% of the total radioactivity at later times. During electron microscopic examination 4 days after 241Pu injection, beta tracks were frequently observed over globular structures resembling dense bodies in Chinese hamster liver. They were also observed frequently over chromatin-rich portions of the cell nuclei. These results, together with those from previous density gradient studies, show that lysosomes are the primary deposition site for 239Pu in the liver cytoplasm of these three rodent species. The hypothesis of a morphologic transformation of these lysosomes with time in hamster liver and of rapid bulk exocytosis of the lysosomes in rats are still possible explanations for the extreme differences in the elimination among the three species.

[Multiple recurrent occlusions of the retinal and cerebral arteries in a 20-year-old woman].

A 20-year-old woman experienced recurrent and sometimes multiple retinal and cerebral arterial occlusions over a period of 2 1/2 years. Coagulation studies revealed only minor abnormalities such as raised beta-thromboglobulin and an increased euglobulin lysis time which, together with a history of migraine, may have been of etiological significance. The nature of this condition remains obscure and, no satisfactory treatment has been found so far.

Rapid detection of HBsAg and anti-HBs by enzyme-immunoassay.

Enzyme-immunoassay (EIA) for HBsAg is a reliable method with third generation sensitivity. Using a mechanized system for washing and reading and computerized data processing, subnanogramme amounts of subtypes ad and ay could be detected in approx. 5.5 h. Using EIA-kit reagents and an HGsAg 'standard', anti-HBs could conveniently be detected. Sensitivity comparisons with dilution series of the WHO anti-HBs standard showed RIA to be 8--128 times more sensitive than EIA for anti-HBs. However, similar percentages of positives were found in a patient population, and discrepancies were found between RIA and EIA results which cannot be explained on the basis of sensitivity differences only. A careful examination of the specificity of EIA and RIA is indicated.

Immunological recognition of specific antigenic determinants in UV- and gamma-irradiated phage DNA.

Thermally denatured DNA of coliphage T1 after treatment with uv-light (2537 A) and 60Co-gamma rays acts as a hapten with antigenic determinant groups specific for radiation-induced alterations of the macromolecule. After conjugation to methylated bovine serum albumin the DNA becomes immunogenic in rabbits. Antibodies against irradiated DNA do not react with unirradiated single-stranded DNA. Antigen-antibody complexes were demonstrated by CsCl-density gradient centrifugation. The decrease in buoyant density of the DNA is proportional to the amount of antibody protein bound to the antigen. By this means photoproducts as well as alterations due to ionizing radiation in DNA were detected independent of the type of antigen-antibody complex, i.e. precipitating or soluble aggregate.
