[Biopathology of renal cancer in adults].

Most renal cancers are clear cell adenocarcinoma. The classification is based on the tumor size, the renal vein and lymph node involvement, and metastases. Renal cancer originates from the proximal convoluted tubule. The murine Renca tumor and the Wistar-Lewis rat renal adenocarcinoma are experimental models which well reproduce the evolution of the human renal cell carcinoma. They allow the study of the effect of chemotherapeutic agents, of lymphokines and of transfected cytokines. However, experimental models have limited value.

Middle cerebral artery occlusion in rats studied by magnetic resonance imaging.

Ischemia due to middle cerebral artery occlusion was studied in 29 rats from 1 to 24 hours after occlusion using magnetic resonance imaging. Images were made before and after the injection of a superparamagnetic iron oxide compound, AMI-25. Subtraction images demonstrated the region of perfusion deficit as early as 1 hour after occlusion, earlier than conventional T2-weighted images. The area of altered perfusion detected by this technique (subtraction imaging after AMI-25 administration) correlated with that demonstrated by iodoantipyrine autoradiography. Since this magnetic resonance technique can be used to serially estimate the location and size of the ischemic area, the technique can be an important adjunct to metabolic studies of focal ischemia using magnetic resonance spectroscopy. The technique may have clinical applications as well.

Pharmacokinetics and tissue penetration of cefpirome, a new cephalosporin.

The pharmacokinetics and tissue penetration (as measured by a blister fluid model) of cefpirome were studied in six male volunteers following a 1 g intravenous dose. A mean peak serum concentration (at 5 min) of 97.4 mg/l was followed by rapid distribution into an apparent volume of 21.3 1. The serum elimination half-life was 2.3 h. Cefpirome penetrated rapidly into inflammatory fluid with a mean peak concentration of 39.2 mg/l at 1.9 h. The mean inflammatory fluid elimination half-life was 2.5 h. The availability of the drug in inflammatory fluid was high with a mean per cent penetration of 123%. The plasma and renal clearances were 109.5 and 82.1 ml/min respectively. Twenty-four hour urinary recovery was 75.5% of the administered dose. This study suggests that a twice daily dosage may be sufficient to treat tissue infections with susceptible pathogens.

Utilization of medical information systems in American occupational medicine. A committee report.

In an attempt to define the nature of medical programs and the extent of utilization of information systems in occupational medicine, a questionnaire was directed to 230 corporate medical directors of the largest employers in the USA. Responses were received from 163 (nearly 70%) of those contacted. While industrial injury data and sickness absence statistics are the principal areas of present computer analysis, respondents indicated that future plans called for a shift in emphasis to the analysis and correlation of human medical data with work exposure data. Nearly three-quarters of the respondents are planning more extensive medical information systems. It is concluded that there is an apparent need for both trained personnel to develop appropriate systems and enhanced communication between the users and suppliers of the required computer technology.
