Intravascular papillary endothelial hyperplasia in the oral mucosa.

An intraluminal reactive hyperplastic lesion from the lower labial mucosa of an adult male patient is described. It has been termed intravascular papillary endothelial hyperplasia and may have arisen in an organizing thrombosed vein. The importance of distinguishing it from a malignant vascular neoplasm is stressed, and the differential diagnosis is discussed.

Chronic sclerosing osteomyelitis of the mandible. Histologic and histochemical findings.

The histologic features of chronic sclerosing osteomyelitis of the mandible were studied. Four different types of tissue reaction were found to be characteristic of the disease. Histochemical recordings were used in an attempt to facilitate differential diagnosis. Similar types of fibro-osseous lesions and biopsy technique are discussed.

Disappearing bone disease. A clinical and histological study.

The case histories of three patients, a man and two boys, with disappearing bone disease are reported. The clinical, roentgenographic, and histopathological features are described in detail. Histologically, in the early stages of the disease, the vanishing bone is replaced by numerous wide engorged capillaries. Eventually the bone is replaced by dense fibrous tissue. All three patients were treated by radiotherapy. Histochemical studies performed in one case revealed strong acid phosphatase and leucine aminopeptidase activities in perivascular mononuclear cells (possibly pericytes), suggesting that these cells took part in the bone resorption.

Congenital absence of the vagina. A clinical, histological and histochemical study on 17 patients with graft-constructed vaginas.

During a ten-year period 17 patients with total congenital absence of the vagina were treated by a split-skin inlay grafting technique. The average age at the time of operation was 21 years. Associated malformations, mostly afflicting the urinary tract, were found in 9 patients. The skin graft was applied on a perforated hollow acrylic mould, 12 cm long and 4 cm in diameter. The acrylic mould was removed on the tenth day and a soft silicon mould was then utilized for at least 6 months. The mean hospitalization time was 28 days. The grafts took completely in 11/17 patients and in the remaining cases the take averaged 80%. There was no major complication, such as fistulas, in the series. With the exception of two patients with male hermaphroditism all patients were contented with the operation and stated that they had a satisfactory sex-life. In 9/17 patients, a closer follow-up including a biopsy was made. The average depth of the vaginas was 8 cm (range, 6 to 10 cm), and the average diameter 3 cm (range 2.5 to 4 cm). The histological studies consistently showed that the original skin morphology was maintained in the recipient site.

Alterations in the lipid content of rat palatal epithelium during carcinogenesis.

The hard palates of 150 female albino rats of the Sprague-Dawley strain were painted 3 times a week either with the fat-soluble carcinogen 7,12-dimethylbenz(a)anthracene (DMBA) for 11 months with inhibited secretion of saliva or with the water-soluble carcinogen 4-nitrochinoline N-oxide (4NQO) for 8 months with intact salivary secretion. Specimens were taken regularly from the mucosa of the hard palate, and the content of lipids in the epithelium was studied histochemically and biochemically during the carcinogenesis. Changes in the lipid content could be observed histochemically as there was a focal loss of lipid stainability in the epithelium during the more advanced stages of carcinogenesis with severe epithelial dysplasia, carcinoma in situ and early invasive carcinoma. The biochemical method used (TLC) could not, however, verify the histologically observed changes in the lipid content of the epithelium, probably because the changes were very local.

Enzyme histochemical studies on the formation of hyalin bodies in the epithelium of odontogenic cysts.

Frozen sections of eight odontogenic cysts, including one keratocyst, were incubated to show the following enzyme activities: NADH2 diaphorase, NADPH2 diaphorase, glucose-6-phosphate dehydrogenase, acid phosphatase and leucine aminopeptidase. The disbribution of lipid was shown by the oil red 0 method. The activities of all three oxidative enzymes were strongest in epithelial cells bordering hyalin bodies and in basal cells in the epithelial lining. Hydrolytic enzyme activity was absent from all but the most superficial epithelial cells but was present in macrophages and, in lesser amounts, in granular material in the same sections. The granular material frequently contained lipid. The lack of hydrolytic enzyme activity in bordering epithelial cells is inconsistent with the theory that hyalin bodies form from degenerating blood vessels. High aerobic oxidative enzyme activity in the same cells also conflicts with the concept that the bodies are a keratinous product. The findings lend support to the theory that hyalin bodies are an epithelial secretion.

Traumatic injuries to enamel formation in rat incisors.

Twenty-three fullgrown white male rats were fitted with an inclined plane on the mandibular incisors with consequent raising of the bite by 2.5 mm. After an observation period of between 1 and 30 d the animals were killed. In all cases traumatic injuries to the enamel organs were seen. The defects were mainly of four different types. Type 1 occurred in the early maturation zone of the enamel as a local defect with dedifferentiated ameloblasts. Vascular injury was moderate and deformation of the dentin was seen. After 30 d there was still a local defect of the enamel organ. Type 2 occurred apically to the maturation zone and caused a two-layered ameloblastoma resembling a sandwich. Hemorrhages were commmon in the surrounding connective tissue, with accompanying appearance of phospholipids in the tissue. After 30 d some restitution of the enamel formation was seen. Type 3 was necrosis was seen in the surrounding tissue. After 30 d a new ameloblastoma had developed in some areas. These three types appeared separately or combined. Type 4 occurred as a folding of newly formed apical dentin in areas without differentiated ameloblastoma and was seen in all of the experimental animals.

Cytogenetic and histochemical analysis of experimental oral carcinogenesis in the rat.

The present investigation has shown that in chemical carcinogenesis progression of malignant epithelial changes in associated with an increased number of cells with chromosomal deviations. Each out of two oncogenic agents with different chemical characteristics had its own non-random pattern. There was usually a close relationship between such chromosomal changes and the simultaneous presence of morphological atypia (dysplasia) and metabolic atypia (aberrant G-6-P.DH activity) from moderate dysplasia and onwards during carcinogenesis. Changes in X-chromosomes, which i.a. in human code the synthesis of G-6-P.DH molecules, could perhaps explain and be reflected in deviating metabolic reactions in premalignant cells.

Histological and histochemical investigations of lupus erythematosus in the skin and the oral mucosa.

Skin and oral lesions of chronic discoid lupus erythematosus from 6 patients have been investigated histologically and histochemically. Intra-individual comparisons between skin and oral lesions and intrasection comparisons between clinically affected and unaffected regions were performed. Enzyme histochemical recordings consisted of oxido-reductase and hydrolase activities. A high degree of correlation was noted in the intra-individual comparisons. The intrasection comparisons provided information concerning metabolic dynamics of the disease. The data obtained support the view that serious vascular changes are involved in the development of the changes in the overlying epithelium. The enzyme histochemical results agreed generally with histological data and proved to be a valuable diagnostic aid.
