[Future research perspectives of a psychoanalytical gerontopsychophysiology--personality and the aging process].

Based on the irreversibility of physiological development during the aging process, a concept of permanent personality development throughout life is presented, which is at no time completed. Based on clinical experience, questions are induced for future research in the field of gerontopsychosomatics, which take into consideration the possibility of a revival of previously existing neurotic conflicts as well as possible new functional and psychosomatic symptoms understandable because of the actuating situation in old age. The question is raised whether such newly acquired psychosomatic symptoms must be considered as a multimorbidity aspect in older people and thus not accessible to adequate therapy. Finally, the concept "personality plasticity" and "life-long personality development" are related to one another. Gerontological research has emphasized time and again that in older and aged persons competences persist. It is the objective of the present paper to present a perspective, supported by empirical studies, to those who may be unduly impaired for a time in their potential by symptoms or conflicts.

[Study of the practicability of qualitative and quantitative documentation of outcome in inpatient psychotherapy].

The results of a study evaluating the practicability of two newly-developed documentation forms are presented (n = 336 inpatient psychosomatic-psychotherapeutic treatment episodes in 8 clinics). Parallel versions of the forms were completed by patients and therapists. The "ErgeDoku-A-Form" allows for the naming of up to five therapy goals determined at the beginning of therapy and evaluated in relation to their achieved quality at the end of therapy. The "ErgeDoku-B-Form" describes a variety of problem areas as well as questions related to medication and changes in data structure are presented. Results reveal high levels of acceptance in daily clinical practice (indicating clear and appropriate semantics) as well as highly significant relationships between patient and therapist evaluations.

[Life review: promoting the developmental potentials in aging].

In this paper the theoretical background and practical applications of the life review are discussed. Particular emphasis is placed on the relevance of the life review for the mastery of developmental transitions across the entire life span as well as for adaptation to the challenges commonly associated with aging. The terms guided autobiography, life review and reminiscence are explained as are techniques for guiding reminiscence and life review. The life review is presented as a method which can be effectively used in community or institutional settings.

[Personality development in the elderly--a psychoanalytic paradigm of development].

Traditional concepts within psychoanalytically oriented developmental psychology end with adulthood. The life cycle theories of Erikson as well as Colarusso & Nemiroff cannot, in their current form, explain the processes of conflict interaction in adulthood that are responsible for continued development. Currently existing aging-specific psychoanalytic and cognitive-motivational oriented theories do not provide solutions to this question. Based on the first qualitative results from 2-4-hour semi-structured, biographical interviews with older adults, a simplified model of development throughout the life-course based on the following three basic organizing principles (in accordance with Spitz) is presented 1) psychological processes until adulthood; 2) social relationships during adulthood; 3) somatic processes in late life. Further empirical investigation of this model will be carried out within the context of the Eldermen-study now in progress. First results indicate a variety of clinically relevant developmental conflicts in late life. A deeper understanding of these conflicts will provide the basis for more extensive and appropriate therapeutic interventions.

[Psychosomatic aspects of pain perception by the elderly--results of the ELDERMEN Study].

115 geriatric in-patients are included in the current study examining relationships between regions of experienced pain, subjective pain intensity, and psychological variables. An investigation of the connection between psychosomatic limitations (BSS) and pain indicated, that pain patients do not necessarily require psychosomatic-psychotherapeutic treatment (in our study only 21.6%). 61.7% of the study sample reported intense pain at least one body region. These patients differed significantly from those with less pain in terms of their level of functional capacity and need for assistance in daily activities (ADL/IADL). They also displayed a significantly more negative attitude towards aging (PGC) than patients with minimal pain. Hierarchical cluster analyses for applied coping strategies produced five groups within the subgroup of patients suffering from extreme pain. Only within one group "depression" was a significant reaction to health-problems. These results make clear that most of the patients with extreme pain are able to cope effectively with pain.

[Operationalized Psychodynamic Diagnosis in a trial--results of multicenter acceptability and practicability study].

During a multicentric field trial involving 134 diagnosticans from 16 centres in a first phase and 38 diagnosticans from 5 centres in a second phase a preliminary version of the manual "Operational Psychodynamic Diagnosis (OPD)" was tested. Focussing on feasibility, practicability and interrater-reliability the different axes of the new system were assessed as comparatively easy to use. The goodness of fit for diagnostic categories and the confidence in the diagnostic process showed a sufficient acceptance of the system. The percentage and chance-corrected inter-rater-reliability coefficients were acceptable for the most axes. It could be demonstrated that interrater-reliability depends on the degree of experiences with the OPD approach. Therefore a systematic training is necessary to use the system.
