[Cardiovascular effects of flunitrazepam (rohypnol, RO-5-4200) (author's transl)].

We reported on the haemodynamic effects of 0.03 mg/kg flunitrazepam during surgical procedures in neuroleptanalgesia in 39 patients with congenital or acquired heart diseases, functional class II-IV. The benzodiazepine derivative did not cause any relevant effect on the inotropic state of the myocardium. There were only minor changes in cardiac index, stroke index, right and left atrial pressure. Changes in arterial pressure and left ventricular pressure during and immediately after surgical procedures, and in arterial perfusion pressure during extracorporeal circulation, as well as an only short lasting increase in heart rate were demonstrative a peripheral vasodilator effect. The decrease in ventricular work and myocardial oxygen consumption are of value in patients with coronary heart disease, especially immediately after surgical procedures. Flunitrazepam is considered an additional drug during neuroleptanalgesia, when hypertension is causing some problems.

Changes in hemodynamic parameters, inotropic state, and myocardial oxygen consumption owing to intravenous application of nitroglycerin.

Changes in hemodynamics, inotropic state, and myocardial oxygen consumptom were investigated in a total of 70 patients with coronary disease after intravenous application of nitroglycerin (0.4 mg., 0.2 mg., and 2 mg. per hour). The results demonstrate that nitroglycerin enlarges therapeutic possibilities during and immediately after operations in patients with limited coronary reserve. The decrease in myocardial oxygen consumption, dependent on dose, ranged between 15.8 and 22.9 per cent.

[Monitoring of cardio-pulmonary parameters in cases of severe multiple injuries (author's transl)].

Haemodynamic parameters were monitored for one week (in greatest detail during the first 24 hours) in 35 severely injured persons who had been admitted for treatment within one hour after the accident and, as judged by the type of injury, had probably lost more than 1 1/2 litres of blood. The estimations included not only blood pressure, heart rate and shock index; but the cardiac index, pulmonary arterial pressure, pulmonary capillary pressure and parameters of oxygen transport were also determined with the aid of Swan-Ganz catheters. The importance of the data as guide-lines for the correct type of intensive therapy and their prognostic significance are discussed. The evidence provided by the cardiac index, pulmonary arterial and capillary pressure and the parameters of oxygen transport are of particular importance in these cases. A list is added of the values of some haemodynamic factors which should be taken into account in the initial treatment of severely injured persons.

[Cardiovascular parameters following severe multiple injuries].

Cardiovascular parameters of 35 patients with multiple injuries are monitored over a period of 7 days. Pulmonary artery pressure, pulmonary vascular resistance, intrapulmonary shunts and cardiac output appear to have a significant prognostic value.

[Combination of etomidate and fentanyl. Haemodynamics, inotropic state, myocardial oxygen consumption, and vascular reactions (author's transl)].

The influence of a combination of fentanyl (0.1 mg) and etomidate (0,15 mg/kg and 0.3 mg/kg) on haemodynamics, inotropic state, myocardial oxygen consumption and vascular reaction has been investigated in 60 patients with congenital or acquired heart diseases--mostly functional class III according to NYHAC. Because of minor changes in haemodynamics (decrease in mean arterial pressure, left ventricular pressure, peak dp/dt, cardiac index, stroke index) the combination of 0.15 mg/kg etomidate and 0.1 mg fentanyl seems to be of great value for induction of anaesthesia in patients with cardiac disease compared to other intravenous induction agents such as propanidid, thiopentone or althesin. In accordance with the changes in haemodynamics there was a 12% and 18% decrease in myocardial oxygen consumption, respectively.

[Regulation of intraocular pressure and haemodynamics in patients with severe polytrauma (author's transl)].

In 18 polytraumatized patients injuried by an accident, the intraocular pressure was measured and the aqueous humour production and regulatory capacity were determined by means of the perilimbal suction cup with simultaneous recording of haemodynamic parameters. The measurements were carried out 1. in the immediate acute stage, 2. in the period of circulatory recovery, and 3. under stable circulatory conditions. It became evident that the production of aqueous humour was suspended in the acute stage. With improving stabilization of the circulatory system, the aqueous humour minute volume and the regulatory capacity of the eye were raised. The cause of the reduction of both these parameters in the acute stage of a polytrauma has to be seen in an affection of the intraocular vascular nervous system in dependence of the general shock situation.

[Cardiac and selective vascular effects of canrenoate-potassium (aldactone pro injectione) in cardiosurgical patients (author's transl)].

Hemodynamic and inotropic effects of 200 mg and 400 mg Aldactone pro injectione (Canrenoate-Potassium, Aldadiene) were measured in three different series (n=60) during and after cardiac surgery in neuroleptanalgesia; results were compared to a control group (n=35). The first investigation was performed after sternotomy and pericardiotomy. During a measuring period of ten minutes no significant changes in inotropic parameter dp/dt max were found in comparison to individual control values. The second investigation during "steady state" extracorporeal circulation revealed a small decrease in arterial perfusion perfusion pressure after both dosages, which can be interpreted as an arteriolar vasodilatation. There was, however, no significant difference in comparison to the control group. Additional measurements were done after cardiac surgery in a total of 20 patients (200 mg and 400 mg Aldactone pro injectione) during an investigation period of 60 min. In comparison to a control group (n=15) no significant differences in arterial pressure, heart rate, cardiac index, stroke index and pulmonary arterial pressure were found. There were no acute and direct positive inotropic effects of Canrenoate-Potassium after administration of 200 mg and 400 mg respectively in cardiosurgical patients.

[Effect of spinal anesthesia on peripheral hemodynamics].

In eight persons central hemodynamic parameters as well as peripheral circulatory parameters were measured before and during spinal anaesthesia (L3/4; 5 ml Carbostesin mit Adrenalin): heart rate, arterial pressure, central venous pressure, peripheral venous pressure, peripheral blood flow, venous capacity (Cv 70) using a venous pressure of 70 mm Hg (lethysmographic method) as well as intravascular basic volume. Intravascular basic volume and the venous capacity are representing total intravascular volume; changes in this parameter- in correlation to a well defind intravenous pressure - give information on the distensibility of capacitance vessels. During spinal anesthesia an increase in resting blood flow of the calf as well as in total blood volume was noted as a result of a decreasing total peripheral resistance and changes in blood distribution (shunting, nutritive blood supply). Intravascular total blood volume in the calf increased from 9.7 +/- 1.0 ml/100 ml to 10.9 +/- 1.1 ml/100 ml, being caused mainly by changes in basic volume not so much by changes in venous capacity. These results may be interpreted as an increase in distensibility of the low pressure vessels during spinal anesthesia.
