[TSH state after thyroid hormone deprivation in athyrotic thyroid cancer patients (author's transl)].

The duration of thyroid hormone deprivation necessary on the one hand to achieve a sufficiently high endogenous TSH level for the identification of 131I-storing metastases or local relapses, and on the other hand to keep the consequences of thyroid hormone deprivation as low as possible, was determined in 111 athyrotic patients with thyroid carcinoma by means of the basal TSH level or a TRH stimulation test. From the first follow-up examination, a T3 deprivation of 8 days is to be recommended. Low basal TSH levels and marked stimulation in the TRH test should be sufficient cause to prolong the T3 deprivation to 10 days with further examinations. After a longterm suppression of athyrotic patients with thyroid hormone, a decrease in the TSH rise was observed in the course of 2 years, after appropriate interruption of the substitution.

Serum iron levels in depressed patients.

76 depressive patients (male and female) suffering from different kinds of depression (involutional, uni-and bipolar, neurotic, postpartum) showed no reduced serum iron levels. Even after a three to five weeks therapy with tricyclic antidepressants the serum iron levels of 17 of these patients were still within the normal range. On the basis of these results we can neither see a correlation between hyposideremia and the depressive syndrome, nor a hint for a decrease of serum iron levels under treatment with tricyclic antidepressants.

[Correction of tetralogy of Fallot and its influence to oxygen transport and lung changes. Part II: Lung changes (author's transl)].

22 children got lung scans 3 weeks respectively 12 months after the correction of a tetralogy of Fallot. In 18 cases previous operations were done: 12 times a Blalock-Taussig shunt and 6 times a Brock procedure. For the scan 20-70 mu diameter albumin macrospheres were used, which were labeled with Technetium 99m. The following pathologicla lung changes were seen: 1. Loss of perfusion, typical after Blalock-Taussig shunt procedure; these findings were always on the left side, the site of the anastomosis. 2. Anomalous flow distrubution (=more spheres in the upper than in the lower lobe) in the left lung; these changes were also caused by the Blalock-Taussing shunts, but disappeared within the one year follow-up after the correction. 3. Intrapulmonary rigt-left shunts (according to the dilatation of the alveolar capillaries). These decreased within one year from 9.9+/-1.3 to 4.6+/-0.9%.

Reduced growth hormone responses to amphetamine in "endogenous" depressive patients: studies in normal, "reactive" and "endogenous" depressive, schizophrenic, and chronic alcoholic subjects.

Several pharmacological stimulation tests of the pituitary-hypothalamic system have been used to investigate psychiatric disorders. This study introduces amphetamine sulfate as a stimulus for human growth hormone (HGH) release in various psychiatric patients. Peak HGH release after a single intravenous administration of amphetamine sulfate, 0.1 mg/kg, was significantly lower in nine "endogenous" depressives (P = .01) and significantly higher in seven "reactive" depressives (P less than .05) as compared to normal subjects, whereas peak HGH release in eight schizophrenics and six chronic alcoholics did not differ significantly from that in normal subjects.

Age-dependent changes of heart valves and heart size.

The mode and extent of the regressions of heart valves, valvular rings, and tendon chords, as well as the size of atria and ventricles in 316 hearts of different age groups (macroscopically normal at autopsy), were studied. With increasing age, the valves showed marked thickening and loss of elasticity. Deposition of lipoids and calcification occur to a variable extent in the valvular tissue and rings; Valve size shows a change of minor degree, but tendon chords and papillary muscles shorten in old age. The diameters of the atria, mitral, tricuspid, pulmonary, and aortic rings increase continuously up to the 9th decade of life. The width and length of the ventricles show minor alterations in size. In old age, heart weight increases.

Probability imputation revisited for prognostic factor studies.

The analysis of prognostic factor studies by Cox or logistic regression models is often impeded by missing covariate values. In 1990 Schemper and Smith recommended a conditional probability imputation technique (PIT) for the analysis of treatment studies which can be easily applied using standard software and which has been demonstrated to outperform the complete case and omission of covariates strategies. Recent research, however, showed that PIT cannot universally be recommended and it was concluded that model-based methods should be preferred. We agree with these conclusions but also think that there is enough empirical evidence to judge the performance of PIT to be satisfactory in typical prognostic factor studies. Furthermore, comparisons of PIT with multiple imputation in the same context did not indicate an advantage of the latter more involved technique. By means of an analysis of a prostate cancer data set various aspects of application of PIT are discussed, in particular that PIT permits direct comparability of marginal and partial effects analyses. We conclude that PIT continues to be an appropriate and attractive choice for analyses of prognostic factor studies.

The passage of granulocyte-macrophage colony-stimulating factor across the human placenta perfused in vitro.

OBJECTIVE: The purpose of this study was to investigate the placental passage of granulocyte-macrophage colony-stimulating factor in a placental perfusion model ex vivo. METHODS: In an open system, 11 placentas were perfused on both the maternal and the fetal side immediately after delivery. Granulocyte-macrophage colony-stimulating factor was added to the maternal perfusion medium in concentrations from 10-55 micrograms/mL. Maternal and fetal samples were taken, and granulocyte-macrophage colony-stimulating factor (GM-CSF) was measured by enzyme-linked immunosorbent assay. RESULTS: Accumulation of granulocyte-macrophage colony-stimulating factor in the fetal circuit averaged 2.42% of the concentration added initially to the arterial portion of the maternal circuit. CONCLUSION: There is only low transfer of GM-CSF across the fetal membranes. This finding is particularly remarkable in view of recently published results suggesting that administration of recombinant granulocyte growth factors to pregnant women with imminent preterm delivery helps prevent neonatal sepsis.

Placental expression and serum levels of cytokeratin-18 are increased in women with preeclampsia.

OBJECTIVE: To evaluate placental expression and serum cytokeratin-18 in women with preeclampsia. METHODS: Serum cytokeratin-18 was evaluated in 44 women with preeclampsia and 44 healthy pregnant women using an immunoradiometric assay. Placental expression of cytokeratin-18 was investigated in specimens from 23 women with preeclampsia and 20 healthy pregnant women by immunohistochemistry. RESULTS: Median serum cytokeratin-18 in women with preeclampsia and healthy pregnant women was 106.7 and 76.0 U/L, respectively (P =.02). Among women with preeclampsia, serum cytokeratin-18 was significantly associated with severity of disease (P =.001) and showed a sensitivity (standard error) and specificity (standard error) of 85% (7%) and 65% (12%), respectively. In placental specimens, the cytoplasm of the syncytiotrophoblast stained positive for cytokeratin-18 with strong and widespread staining in 83% and 45% of placental specimens of women with preeclampsia and healthy pregnant women, respectively (P =.01). CONCLUSION: Elevated serum cytokeratin-18 values are associated with disease severity in women with preeclampsia. Our data provide additional evidence that the placenta might be the source of the elevated serum cytokeratin-18 values in women with preeclampsia.
