Prevalence of partial deficiency of red cell triosephosphate isomerase in Germany--a study of 3000 people.

During a heterozygote screening of nearly 3000 persons, triosephosphate isomerase (TPI) deficiencies in erythrocytes were discovered in 11 unrelated persons, showing a residual activity between 39 and 76% of normal activity. Extensive genealogic studies were performed to confirm that these persons with TPI deficiency were heterozygous carriers. The total heterozygote frequency of triosephosphate isomerase deficiencies was 3.7/1000. The persons with heterozygous deficiency could be divided into two categories. Subjects of category I had a mean residual activity of 49% of the expected normal activity and were represented by a frequency of 1.3/1000. Subjects of category II had a mean residual activity of 67% of the expected normal activity and were represented by a frequency of 2.4/1000. None of the heterozygous persons showed an electrophoretic variant. The immunologic specific activity was normal with one exception. Therefore, we assume that in many cases of our heterozygous TPI-deficiencies a TPI protein with a normal specific activity is synthesized to a diminished degree.

[Analytic and diagnostic validity of the determination of carboxyhemoglobin in blood and carbon monoxide in the breath of smokers and non-smokers].

The analytic and diagnostic validity of the determination of carboxyhaemoglobin and carbon monoxide in smokers and non-smokers was investigated. Both methods are of high analytic and diagnostic validity. Precision and accuracy fullfil the essential requirements. Comparison of the methods shows good correlation (= 0.97) and linear regression (y = 51.5 X 10(-3)x + 6.26 X 10(-6] within the investigated range. Values for the conditional probabilities of sensitivity, specificity, predictive values, and efficiency are higher 0.80. Taking into account the standardized sample collection for the CO determination, the two methods are of comparable validity when used in clinical and epidemiological investigations on smokers and non-smokers.

[The influence of different diets and smoking on the clinical chemical diagnosis of pheochromocytoma, neuroblastoma, and carcinoid syndrome (author's transl)].

The interference of various foodstuffs on methods to determine epinephrine (E), norepinephrine (NE), vanillylmandelic acid (VMA), metanephrines (MN), homovanillic acid (HVA), and 6-hydroxyindole acetic acid (5-HIA) in the 24 h urine for diagnosis of pheochromocytoma and carcinoid syndrome, respectively, was investigated. The foodstuffs included were: tea, coffee, almonds, pineapples, cheese, walnuts, vanilla pudding, bananas, tomatoes, and chocolate. Further, the interference of cigarette smoking on the determination of E, NE, VMA, and MN was also investigated. Walnuts caused a rather high elevation of 5-HIA in the urine. After eating bananas elevated excretion of E, NE, VMA, MN, and 5-HIA was observed. Small increases of the MN values were noticed after coffee and pineapples. Smoking of 20-30 cigarettes/day had no influence on the variables measured. If the methods described are used, thus, only bananas and walnuts have to be restricted some days before and during urine sampling, but not coffee and pineapples if consumed in the usual small quantities. There is no reason to insist on diet restriction except for bananas and walnuts.

[Platelet counting in whole blood by means of a new counter with hydrodynamic focused aperture (author's transl)].

Platelet counts in whole blood were performed by means of a new particle analyzer with a hydrodynamic focused aperture (Ultra-Flo 100) and its values were compared with the results obtained by an optical counting chamber. The comparison yielded good agreement of the values within the evaluated range of 3-963 G/l thrombocytes. Especially remarkable is the good correlation of the markedly thrombocytopenic values. Equally reliable is the counting of thrombocytes in blood with markedly elevated erythrocytes. The simple procedure of sample preparing combined with the stand-by technic of the analyzer provides advantageous conditions for using it likewisely in routine and emergency laboratories.

[Congenital diaphragmatic eventration].

The nomenclature of congenital defects of the diaphragm is not unequivocal. Congenital eventration means the incomplete ingrowth of muscle fibers between the pleural and peritoneal sheet of the diaphragm during the embryonal period. The differential diagnosis of eventration and hernia may be very difficult. The adequate surgical treatment differs. As concomitant congenital defects of the ipsilateral lung often occur, congenital eventration should be operated by transthoracic approach. The prognosis of symptomatic eventration is poor in newborns and good in adults. This is illustrated by two own cases. The one deals with a newborn who had a large congenital eventration of the left diaphragm; the other has been reported detailed elsewhere.

[Comparative ultrasonic Doppler blood pressure determination on the upper and lower extremities as a possibility in the early detection and course observation of peripheral arterial circulatory disorders].

The comparative measuring of blood pressure on the arm and foot artery by means of the ultrasound technique after Doppler is a method for the diagnostics of the disturbance of the peripheral arterial blood supply relevant to practice. Its advantages are in the short time of examination, the simple manipulation, the little technical expenditure and the providing of clear measuring data. The course of examination used by us is explained, the normal and borderline values got are reported. Patients with haemodynamically effective disturbances of the arterial blood supply of the vessels of the leg have on the malleolar artery a by more than 10 Torr lower blood pressure than on the brachial artery. For recognition of early changes we propose a simple loading test in form of a position test. The comparative ultrasound measuring of the blood pressure after Doppler on the upper and lower extremity is suitable for the observation of the course and should stand in the first place of the functional diagnostics of peripheral angioorganopathies.

[Long-term course of aortic arch syndrome caused by arteriosclerosis].

The course of the extracranially conditioned cerebrovascular disease is characterized by a worse prognosis in comparison with the average population of the GDR (11 years older) and by increased cardiovascular complication rate (every fifth patient suffered a myocardial infarction, every fourth patient suffered an apoplexy). Younger age of manifestation and increasing number of concomitant diseases form the basis of a course which is altogether endangered by complications, which, however, compared with peripheral and coronary arteriosclerosis, is more favourable. Operative reconstruction of the path of a vessel leads to an enduring improvement of the picture of the complaint. The total prognosis corresponds to the so-called spontaneous course. Under permanent treatment with anticoagulants a retardation of the general progressing of arteriosclerosis and improvement of the survival rates is to be proved.

Chondrosarcoma of the first metatarsal--primary or secondary to enchondroma?

Enchondromas are said to be extremely rare precursors of secondary chondrosarcomas. Peripheral enchondromas of the hand and foot may be considered as benign, even when the histomorphological study reveals pleomorphic features with atypical nuclei. Our case deals with a 56 year old woman who had an enchondroma of the first metatarsal. Its distinct histopathological signs of focal malignant transformation were only seen in later review. Nine months after curettage and autologous bone-grafting a chondrosarcoma was diagnosed. The enbloc-resection of the medial foot provided adequate surgical treatment of the chondrosarcoma.

Comparative evaluation of different spectrophometric methods for the determination of small amounts of carboxyhaemoglobin.

Four spectrophotometric methods measuring HbCO were investigated in comparison with an analyzer (CO-Oximeter) in order to find a method determining HbCO fractions) below 0.10 with sufficient reliability. The CO-Oximeter was chosen as "reference" method because results of other authors with this instrument yielded good agreement with other methods, e.g. IR-spectrometry, GLC, van Slyke. Only the five wavelength method of Commins & Lawther (1965) Brit. J. Industr. Med. 22, 139--143) can be recommended, for it yielded CV's and sensitivities comparable to the CO-Oximeter. The within-run precision had CVs of 2.3% to 4.7%, the sensitivity was 0.0021 to 0.0028. The method is suited even for HbCO measurements of shorter series.

[The determination of capillary blood glucose using the automatic clinical analyzer (aca) Dupont (author's transl)].

The determination of capillary blood glucose after deproteinization using the aca is described. The method, which uses the hexokinase/glucose-6-phosphate dehydrogenase reaction, is compared with the glucose dehydrogenase method. The comparison shows that glucose values measured in capillary blood are essentially the same in both methods. The requirements for quality control are fulfilled. The method is not influenced by hemolysis, bilirubinemia, and hypertriglyceridemia.
