Comparison of broiler performance when fed diets containing corn grain with insect-protected (corn rootworm and European corn borer) and herbicide-tolerant (glyphosate) traits, control corn, or commercial reference corn--revisited.

A 42-d feeding experiment with growing Ross x Ross 508 broilers showed that the nutritional value of insect-protected and herbicide-tolerant corn was comparable to that of the genetically similar control and 5 commercial reference corn hybrids. MON 88017 provides protection from feeding damage by coleopteran pest corn rootworm and is tolerant to the action of glyphosate, the active ingredient in the Roundup family of agricultural herbicides. MON 88017 x MON 810 was developed by the traditional breeding of MON 88017 with MON 810, which provides protection from the European corn borer and other lepidopteran pests. A randomized complete block design was used with 8 dietary treatments in each of 5 replicated blocks of pens. No differences among diets were observed (P > 0.05) in performance (final live weights, feed intake, feed conversion, and adjusted feed conversion), carcass yield (chill, fat pad, breast, thigh, wing, and drum weight), or percentage of moisture, protein, and fat in breast meat and moisture and fat in thigh meat. Thigh protein was similar (P > 0.05) in broilers fed diets containing MON 88017 x MON 810 and conventional control or all commercial reference corns; however, differences (P < 0.05) were noted for the percentage of thigh protein among broilers fed the control and 2 of the 5 reference diets, attributable to biological variability among the conventional corn hybrids. Broilers overall performed consistently and had similar carcass yield and meat composition when fed diets containing MON 88017 or MON 88017 x MON 810 as compared with those fed the conventional control and commercial diets, supporting a conclusion of nutritional equivalence.

Severe ascites: efficacy of the transjugular intrahepatic portosystemic shunt in treatment.

PURPOSE: To evaluate the transjugular intrahepatic portosystemic shunt (TIPS) as primary treatment in patients with cirrhosis and severe ascites. MATERIALS AND METHODS: A TIPS placement was attempted in 54 consecutive patients with intractable ascites. Clinical assessment findings, shunt patency, complications, and survival were analyzed. RESULTS: A TIPS was successfully placed in 50 patients (93%). Follow-up for clinical effectiveness in 51 patients was a mean of 285 days (range, 1-981 days). Forty patients (78%) gained clinical benefit from the shunt. Of these, 29 (57%) had a complete response (required no further paracentesis) and 11 patients (22%) had a partial response (required less frequent but additional paracentesis for control of ascites). The absence of preprocedure renal insufficiency (creatinine < 1.5 mg/dL [< 130 mumol/L]) was the only characteristic identified as an indicator of clinical success (P < .05). Eleven patients (22%) required shunt revision during follow-up to gain or prolong control of symptoms. Cumulative survival in the population evaluated for clinical efficacy was 53% at 6 months and 48% at 1 year. A complete response was the only variable that indicated increased survival (P < .05; R2 = 12%) with a 6-month survival rate of 76% and a 1-year rate of 71%. CONCLUSION: TIPS placement appears to be effective as a primary treatment of patients with cirrhosis and severe ascites.

Detection of myocardial ischemia: value of combined myocardial perfusion and cineangiographic MR imaging.

OBJECTIVE: Established noninvasive methods for assessing myocardial ischemia have limitations that might be overcome by MR imaging. We investigated MR myocardial perfusion imaging and MR ventriculography, before and after dipyridamole-induced stress, to determine whether the superior spatial and temporal resolution of MR imaging has advantages for the evaluation of myocardial ischemia. SUBJECTS AND METHODS: Eighteen patients with symptoms suggestive of myocardial ischemia were examined by use of MR perfusion imaging and MR cineangiography before and during dipyridamole-induced stress. Multiplanar gradient-echo MR cineangiography and cardiac gated fast low-angle shot (turbo-FLASH) MR imaging during injection of gadopentetate dimeglumine were used. Results were compared with findings from perfusion scintigraphy and coronary arteriography. RESULTS: The accuracy of the combination MR technique for detecting myocardial ischemia was similar to that of scintigraphy. No significant difference was found between the MR technique and scintigraphy for detecting segments of myocardium supplied by stenosed coronary arteries (> 70% reduction in diameter, as determined by coronary arteriography). The sensitivity of the combination MR technique for angiographically detecting significant coronary artery narrowing was 92%, and the specificity was 100%. For scintigraphy, the sensitivity was also 92% and the specificity was 100%. CONCLUSION: Initial results indicate that a combination of stress MR myocardial perfusion imaging and MR ventriculography is feasible and that this technique can detect myocardial ischemia with an accuracy similar to that of scintigraphy. This technique may make more complete noninvasive assessment of myocardial ischemia possible.

Metoclopramide and renal vascular resistance.

We have studied the effect of i.v. metoclopramide on renal vascular resistance in nine healthy volunteers. Peak systolic and end-diastolic frequencies were measured using duplex Doppler ultrasound of a renal interlobar artery, before and after the administration of i.v. metoclopramide 10 mg, and the resistance index derived. There was no significant change in mean arterial pressure or resistance index following metoclopramide.

Percutaneous femoral artery puncture: practice and anatomy.

In a survey of the superficial landmarks used to select the site of puncture of the femoral artery for angiography, the inguinal skin crease was most popular, preferred by 39.2% of angiographers. The maximal femoral pulse irrespective of the position of the skin crease was the next most popular landmark (24.7%). Bony landmarks were least popular (13.0%). The majority (73.7%) of those using the skin crease punctured at the same level or distal to it. The relationship of these superficial landmarks to the common femoral artery (CFA) and its bifurcation were investigated. The inguinal skin crease was distal to the bifurcation of the CFA in 71.9% of limbs (mean, 0.61 cm). The maximal femoral pulse was over the CFA in 92.7% of limbs, and the CFA was projected over the medial aspect of the femoral head in 77.9% of limbs. The use of the inguinal skin crease is a popular though unreliable guide for puncture of the CFA. Use of the maximal femoral pulse will enable more constant puncture of the CFA.

Myocardial bridging of the coronary arteries.

Of 1102 consecutive coronary angiograms 16 (1.4%) were found to have systolic narrowing typical of myocardial bridging on the left anterior coronary artery or its branches. Four cases of bridging had otherwise normal coronary arteriograms, four were associated with left ventricular hypertrophy (three in cases of aortic valve disease and one case of hypertrophic cardiomyopathy) and eight were found in association with fixed coronary artery stenoses. Bridging was seen more commonly than expected on normal angiograms. When bridging was associated with left ventricular hypertrophy it affected a longer segment and caused more severe compression. When bridging was found in patients with coronary artery disease, it was not associated with atheroma at its site. Evidence for a pathogenic role of myocardial bridging in ischaemic heart disease is discussed.

Diagnosis and treatment of venous leakage: a curable cause of impotence.

Sixteen of 149 patients complaining of impotence of a non-neurogenic or endocrine aetiology and with a penile brachial index of greater than 0.7 failed to achieve an erection following intracavernosal injection of 30 mg papaverine and 1 mg phentolamine. All 16 were shown to have a significant venous leak using digital subtraction cavernosography. Nine of the 13 so far operated upon have had a full return of potency. Repeat cavernosography in three failures showed persistent leakage into crural veins in two and non-ligated superficial veins in one. Impotent men with a normal penile brachial index who fail to respond to papaverine and phentolamine have a venous leak, a curable cause of impotence. Surgical access should allow ligation of all identified leaking veins.
