[Microsurgical methods of bone replacement].

Vascularized bone grafting techniques are ideal for long bone defects in the extremities. The authors report on their experience in this field. From January 1991 to 2nd November 1995, 17 patients were treated by vascularized bone grafts. In ten cases iliac crest, in six cases fibula, and in one case a radial forearm composite flap was used. All patients have shown optimal healing and good bony consolidation, except for one case of circulatory disorder of the fibula graft.

[Comparison between various treatment methods in scaphoid pseudarthrosis].

In this study we compare the results of various methods of treatment in 159 patients who were operated on between 1980 and 1990 for scaphoid pseudarthrosis at the Budapest Central Institute for Traumatology. Rated subjectively, 84% of the results of the Russe-I plasty were regarded as good, as were 75% of the spongiosaplasty, 88% of the Ender plate synthesis, and 75% of the AO navicular screw. Of the palliative methods retaining movement, denervation with styloidectomy yielded good results. Silicon prosthesis and proximal pole resection only led to temporary improvement. In patients with severe radiocarpal arthrosis who do heavy work, even today radiocarpal stiffening is regarded as the procedure of choice. In our experience, the best functional results can be expected when immobilization is as short as possible and fixation as stable as possible - which corresponds to Ender plate synthesis according to our results.

Chondromyxoid fibroma of the foot.

Chondromyxoid fibroma is a rare benign cartilaginous tumour seen most frequently around the knee in children and young adults. Up to 17% occur in the foot, where more aggressive appearances have been reported. Four cases of chondromyxoid fibroma in the forefoot are presented, demonstrating the wide range of radiological appearances of this primary tumour. Three of the four tumours recurred, one at 19 years after the surgery. These cases illustrate the need for thorough preoperative assessment to ensure complete surgical resection in an attempt to reduce the incidence of local recurrence.

The stability of thiamine in total parenteral nutrition mixtures stored in EVA and multi-layered bags.

The compounding of complete total parenteral nutrition (TPN) mixtures into big bags with extended shelf-lives has been made more possible by the introduction of reduced gas permeable bags, using multi-layered plastic laminates. Since this produces a highly reduced chemical environment, the stability of thiamine; which degrades by reduction, may be compromised. It was the purpose of this study to investigate the degradation of thiamine at two different concentrations in TPN mixtures containing different commercial amino acid sources stored for extended periods in EVA or multi-layered bags. Results indicated that thiamine was unstable in mixtures containing Freamine III 8.5%, a metabisulphite-containing amino acid infusion, but was relatively stable in mixtures containing Vamin 14, Aminoplex 12 or Eloamin 15% as the amino acid source. Degradation of thiamine in Freamine III 8.5%-containing mixtures stored in multi-layered bags was more rapid than in EVA bags. Degradation rate was not greatly influenced by thiamine concentration. Results indicate that thiamine is stable in complete TPN mixtures for periods of at least 28 days in multi-layered bags, provided metabisulphite containing amino acid infusions are avoided. The shelf-life of complete mixtures containing Freamine III should be restricted to ensure patient receive minimum acceptable daily thiamine requirements.

Effects of treatment duration and severity of depression on the maintenance of gains after cognitive-behavioral and psychodynamic-interpersonal psychotherapy.

One hundred four clients completed a mailed follow-up 1 year after completing 8 of 16 sessions of treatment, either cognitive-behavioral (CB) or psychodynamic-interpersonal (PI) psychotherapy. Although mean scores on outcome measures at 1 year suggested that gains were, in general, well maintained, only 29% of clients were asymptomatic on all 3 occasions of testing (end of treatment, 3 months and 1 year later) without recourse to further treatment. However, only 11% of those asymptomatic at end of treatment experienced relapse or recurrence of depression, albeit on the limited evidence of just two follow-up assessments. The results of comparisons among treatment conditions at 1 year differed substantially from those obtained earlier: Eight-session PI treatment now appeared less efficacious than the other 3 treatment conditions, and there was now no measurable benefit of 16-session over 8-session CB, irrespective of initial severity of depression. These findings confirm the importance of follow-up in evaluation of psychotherapies for depression.

[Substitution of soft tissue defects on the lower leg and foot in septic cases].

After the spontaneous epithelialisation of the defects of soft parts so called unstable scars remain which, because of ulceration, are sources of infection, endangering both soft parts and bones. Substitution with soft tissues of adequate blood supply enables the complete sanation of the septic foci and by this the rapid healing of the patient with full function. Beside this a better cosmetic result can be achieved. In septic environment, if possible, local muscle flaps and free flaps are used, depending on the extent and localization of the defect. Experiences, gained in these operations are described.

[Hand injury caused by a funnel-producing machine].

Authors describe a case of severe hand injury, caused by a funnel producing equipment. In the region between the metacarpals II and III a high pressure, penetrating, mechanical and thermal injury has arisen. The secondary necrosis expanded and this made a covering with soft tissue of full value necessary. Because of the local relations and allergic complications the local and distant possibilities were narrowed. It was possible to find a final solution by the transfer of flaps from the index and III finger and supplementary plastics, using split thickness skin grafts. The result was functionally perfect, esthetically adequate.

[Management of malacia of the os lunatum by implantation of a dorsal metacarpal vascular bundle].

In the treatment of malacia of the lunate bone, beside the prosthesis and procedures aiming the decrease of load, the revascularization of the bone is the third basic therapeutic principle. One possibility of the later, not yet current in this country, is the implantation of the metacarpal vascular bundle in the lunate bone. As far as we know we were the first to perform this operation in Hungary. In this paper our first cases are described together with a detailed description of the operative technique and we would call at the same time to further common examinations.
