Redefinition of school nursing practice: integrating the developmentally disabled.

Results of three needs assessment studies which had the purpose of providing direction for school nursing practices which would address some of the unmet health needs of today's children and adolescents are reported. Results provide specific examples of how school nurses have not been meeting school health needs through an analysis of one specific need -- integrating the developmentally disabled into the regular school setting. Data were collected from parents, teachers and other supportive personnel in schools by interview and survey methods. Environmental assessments were conducted and the policies and procedures governing health-related activities required by special need students were reviewed. The data indicated that for developmentally disabled students, nurses were not performing the commonly accepted functions of nursing and were not transposing their knowledge of the components of health care ordinarily provided in health care settings to care provided in nontraditional health care settings. Examples of nursing functions and the activities needed to fulfill those functions are provided.

Dipalmitoylphosphatidylcholine in amniotic fluid quantified by fast-atom-bombardment mass spectrometry.

Dipalmitoylphosphatidylcholine (DPC) is quantified by taking advantage of stable-isotope-labeled d9-DPC as internal standard. Use of a mass spectrometer to measure the ratio of d9/d0 makes this procedure a quantitative one. d9-DPC was synthesized by refluxing dipalmitoylethanolamine with d3-methyl iodide in methanol in the presence of sodium bicarbonate for 26 h. The yield of d9-phosphatidylcholine (d9-lecithin) was 89% after column-chromatographic purification. Fast atom bombardment was used to desorb the preformed phosphatidylcholine ions in a mass spectrometer of Nier-Johnson geometry. In our assessment of accuracy and precision of this technique, we found a correlation coefficient of 0.9994 between signal and sample concentration. The method was less precise when the total d0- plus d9-DPC was less than 0.2 micrograms or when the ratio of d0- to d9-lecithin exceeded 100. The within-run CV was about 1.0%. The amount of DPC in amniotic fluid samples assessed by mass spectrometry was compared with results for total phosphatidylcholine quantified by thin-layer chromatography. The fate of DPC in various laboratory manipulations was also studied.

[Interaction of terbium ions with inorganic pyrophosphatase from bakers' yeast: characterization of the binding sites].

Terbium ions bind with a 2:1 stoichiometry per subunit to inorganic pyrophosphatase from bakers' yeast (EC 3.6.1.1) as measured by an increase of terbium fluorescence. The Tb3+ inhibition of the Mg2+ activated pyrophosphate hydrolysis is caused by a competitive binding at the substrate site of the active centre. The second Mg2+ binding site--the so-called "stabilization site"--is discussed as an additional binding site for Tb3+. Thereby, Tb3+ causes also a stabilization of the enzyme against heat denaturation. The dissociation constants of the terbium-pyrophosphatase interaction are in the micromolar range.

[Thermitase, a thermostable serine protease of Thermoactinomyces vulgaris: interaction of the active center and the SH-group of the enzyme].

Modification of the serine and histidine residue in the active centre of thermitase with diisopropylfluorophosphate (DFP) or L-1-tosylamide-2-phenylethyl chloromethylketon (TPCK), and of the only SH-group of the enzyme, with Hg-compounds causes an activity loss against hydrolysis of 4-nitrophenylacetate. While the modification of cysteine prevents reaction of serine and histidine in the active centre of the enzyme with DFP and TPCK, respectively, the Hg2+- and CF3Hg+-binding to the SH-group after modification of essential amino acid residues in the active centre is retained. To elucidate the interaction of the SH-group with the active centre, the modified products of thermitase were investigated for their thermostability. Ca2+-ions were found to have a stabilizing effect on all the modified products of thermitase, as well as on the native enzyme. Simultaneous modification of the cysteine and serine leads to an increase in thermostability of thermitase, whilst double modification at the cysteine and histidine causes destabilization of the enzyme.

In vivo imaging of the rat anatomy with nuclear magnetic resonance.

Live rats were imaged by nuclear magnetic resonance (NMR). These images demonstrated fine detail and high object contrast. Motion artifacts are not apparent in 4-minute images, and major blood vessels are demonstrated as regions of low signal intensity because of blood flow. Selective contrast enhancement is possible by varying NMR imager accumulation parameters.

Ultrasonic evaluation of abdominal gastrointestinal tract duplication in children.

Ultrasonographic evaluation of a suspected abdominal mass in children is becoming routine. Recognition of a rare congenital anomaly known as duplication of the gastrointestinal tract might aid in diagnosis and obviate the need for an extensive workup. Two cases are presented that clearly demonstrate the ultrasonographic spectrum of findings in this condition, ranging from a sonolucent cyst to an echogenic mass. An echogenic inner rim seen ultrasonographically is highly suggestive of the diagnosis.

Influence of pindolol (Visken) on respiratory function in 20 asthmatic patients.

The effect of a single oral dose of pindolol on bronchi was studied in a double-blind cross-over trial in 20 adult patients, most of whom had moderate bronchial asthma. A single dose of pindolol 5 mg, sufficient to cause a significant fall in heart rate and blood pressure at rest, did not produce a significant fall in the forced expiratory volume (FEV1.0). However, two patients showed marked deterioration in lung function after pindolol. A conventional therapeutic dose of salbutamol by inhalation produced a significant increase in ventilatory function relative to placebo, but it did not outweigh the bronchial effect of pindolol.

ECG recording in emergency home visits.

During the first half of 1974, 756 emergency home calls have been made by 6 doctors in the City of Oslo. A portable ECG instrument was available at all visits. The visits were unselected as regards complaints. ECG was recorded in 7% of all patients and in 19% of all patients over 60 years of age. The main indication for recording an ECG was chest pain (nearly 2/3 of all recordings). In only a small number of cases did the recording give a definite result as regards diagnosis of management of the case. However, a slight additional benefit was obtained with a majority of the ECG recordings, including what we defined as a phychotherapeutic benefit. The results of this study indicate that the value of a portable electrocardiograph is very limited for emergency home calls in a city.
