Prostacyclin in the circulation of patients with vascular disorders undergoing surgery.

The object of this study was to investigate clinical conditions in which increased production of prostacyclin (PGI2) has been reported. 6-Oxo-prostaglandin F1 alpha (6-oxo-PGF1 alpha) is the stable hydrolysis product of PGI2 and was measured in plasma from patients undergoing hepatic or cardiac surgery and in unoperated patients with vascular and hepatic disease, using gas chromatography/mass spectrometry. Blood obtained simultaneously from portal and peripheral veins, during emergency surgery for bleeding oesophageal varices in six patients with cirrhosis of the liver, contained very high concentrations of 6-oxo-PGF1 alpha (range 99-11,485 pg/ml of plasma). 6-Oxo-PGF1 alpha was higher in portal than in peripheral blood in five out of six patients. Six unoperated patients with cirrhosis and oesophageal varices which were not bleeding all had normal peripheral plasma concentrations of 6-oxo-PGF1 alpha less than 2 pg/ml (normal up to 5 pg/ml). Seventeen patients with severe vascular disease had normal basal plasma 6-oxo-PGF1 alpha concentrations (less than 2 pg/ml). Eighteen subjects with atheromatous coronary artery disease underwent aorta-coronary artery grafting, and plasma concentrations of 6-oxo-PGF1 alpha were markedly elevated during surgery (range 55-1207 pg/ml). We conclude that surgery stimulates PGI2 production substantially, and argue that the function of PGI2 may be to limit intravascular extension of thrombus from sites of haemostasis. Inappropriate PGI2 synthesis may contribute to the massive haemorrhage characteristic of oesophageal variceal bleeding.

Migration in vitro by blood and exudate neutrophils assessed serially during an inflammatory response.

Migration in vitro by blood and inflammatory neutrophils has been compared serially during an inflammatory response. Using an experimental pig model, neutrophils are isolated from the peripheral blood and from the pleural space at hourly intervals after an intrapleural challenge with zymosan activated pig serum (ZAS). Following acepromazine sedation and halothane anesthesia, blood neutrophil migration was transiently reduced. By 1 hour random and directed migration of blood neutrophils returned to normal. Directed and random migration of exudate neutrophils was markedly decreased to both a stimulus-specific (ZAS) and an unrelated (LTB4) chemoattractant. After 3 hours, migration by exudate neutrophils was similar to migration by blood neutrophils examined in parallel. These findings emphasize the importance of performing serial evaluations of cell function during an inflammatory response.

Semi-automated high-performance liquid chromatographic determination of cyclosporine A in whole blood using one-step sample purification and column-switching.

A highly sensitive and semi-automated high-performance liquid chromatographic method, utilizing acetonitrile protein precipitation and column-switching, is described for the determination of cyclosporine A in whole blood. Following a rapid manual acetonitrile treatment of the blood samples, the supernatant is loaded automatically onto a 5-micron high-speed protein separation column without any further clean-up operations. The fraction containing cyclosporine A is switched to a 3-micron C18 reversed-phase high-speed column by a microprocessor-controlled column-switching unit for final separation and detection by absorption at 214 nm. Minimal sample handling and efficient separation resulted in a high recovery (75 +/- 3%) of cyclosporine A from blood and a detection limit as low as 2 micrograms/l with a highly reproducible and linear response up to 2500 micrograms/1 using 0.5 ml of sample. A separation cycle including regeneration of the first column is finished in 15 min, and this system was used continuously for ca. 1000 blood samples from heart, liver, kidney, pancreas and bone marrow recipients without change in separation parameters or material replacement. The method described allows accurate and very fast daily routine monitoring of cyclosporine A in large numbers of blood samples from transplant recipients.

Variable schedules of ibuprofen for ankle sprains.

In a study of 144 patients, with Grade 1 and 2 inversion injuries to the ankle sustained in sport, treatment with a non-steroidal anti-inflammatory was considerably superior to placebo with respect to joint tenderness, level of training and injury severity. No difference in efficacy could be determined between ibuprofen 2400 mg given in two or four equally divided daily doses. Ten patients withdrew from the study because of side effects, five from ibuprofen twice daily, three from ibuprofen four times daily and two from the placebo group. This study confirms the efficacy, flexibility and tolerability of high dose ibuprofen.

Amino acid concentrations in plasma and skeletal muscle of patients with acute hemorrhagic necrotizing pancreatitis.

We measured amino acid concentrations in plasma and skeletal muscle of three groups of patients with acute hemorrhagic pancreatitis: (a) patients without secondary organ lesions, (b) patients also suffering from kidney damage, and (c) patients in whom the pancreatitis was accompanied by sepsis and multiple organ failure. In all three groups, especially the third group, the amino acid concentrations in both plasma and muscle were below normal. Glutamine was only 14% of normal in muscle tissue of the third group. Onset of renal insufficiency was indicated by increasing values for 3-methylhistidine and cystathionine; multiple organ failure, by increased concentrations of methionine and phenylalanine in plasma. The low amino acid concentrations of patients with acute pancreatitis can be explained as a combined effect of semistarvation and hypercatabolism. Changes in the plasma concentrations of amino acids did not reflect necessarily the concentrations in muscle tissue.

Radioimmunodetection in patients with suspected ovarian cancer.

Twenty-five patients, having either unilateral ovarian tumors of unknown etiology or suspected of having ovarian cancer recurrence were investigated by the method of immunoscintigraphy to rule out primary and/or metastatic tumor sites. Four-hundred micrograms of the tumor-associated monoclonal mouse antibody HMFG-2, raised against human milk fat globulin membranes and labeled with 123I, were used for each patient to display the tumor sites by external scintigraphy. The dose ranged between 0.5 and 2.2 mCi, the specific activity between 1.25 and 5.5 mCi per mg of antibody. Nineteen of the patients underwent operations a few days after immunoscintigraphy. The remaining six patients were investigated by transmission computed tomography (TCT) to establish the presence or absence of tumor of the imaging. In 22 of the 25 cases the scintigraphic results correlated with the situation found at the subsequent operation, or by TCT, respectively, as well as with the histological diagnosis of the tumor type. Overall, there were just two false-negative and one false-positive scan report, the latter due to faulty reading of the scintigrams. Sixteen out of 18 tumor sites in 25 patients could be revealed by immunoscintigraphy, the smallest one being 1.5 cm in diam. In four of the patients immunoscintigraphy was the only noninvasive investigation method that could reveal the malignant tumor sites prior to the operation.

Measurement of the lineal density of red blood cells in capillaries in vivo, using a computerized frame-by-frame analysis of video images.

In order to study the oxygen supply system within a capillary bed it is desirable to know the lineal density of red blood cells (RBCs) in individual capillaries; viz number of cells per millimeter. We have developed a video computer method for "continuous" measurements of lineal density, based on frame-by-frame analysis of the spatial-average of blood opacity over a selected length of capillary. Each capillary is calibrated separately, in order to determine the relationship between mean opacity and lineal density for that vessel. Since the method does not attempt to detect individual RBCs, it can be applied to capillaries with RBCs which overlap each other to some extent. Also, since the opacity data is normalized with respect to "background" light intensity and "contrast," this technique can be applied to thick tissues such as skeletal muscle. The method has been tested on capillaries in frog sartorius muscle and is able to predict, on average, the number of frog RBCs in an 80-micron length of capillary to +/- one-quarter cell. At present, computation times limit "real time" measurement of lineal density to a sampling rate of 10 sec-1. The data may also be used (1) to compute red cell flux (cells X sec-1), if corresponding velocity measurements are available, and (2) to estimate capillary hematocrit.

Neutrophil migration in vitro and in vivo during hypothermia.

The effect of hypothermia on pig leukocyte migration in vitro and in vivo was studied. Neutrophil chemotaxis in vitro under agarose was significantly impaired at 29 degrees C (2.7 +/- 0.6 [mean +/- standard error]; 37 degrees C, 7.1 +/- 1.1). Leukocytes isolated from hypothermic pigs and tested at 37 degrees C migrated normally (7.8 +/- 0.6). Neutrophil and monocyte migration in vivo was markedly reduced at 29 degrees C. Reduced inflammatory responses may contribute to increased infections during hypothermia.

Sex-role values and bias in alcohol treatment personnel.

During a two day training workshop with 45 alcohol treatment agency personnel, the impact of sex-role bias on clinical practice with alcoholic clients was examined. More specifically, data were gathered on: 1) client comfort level of surrogate clients as related to perceived sex-role values of treatment personnel; and 2) the impact of patient sex and sex-linked characteristics of presenting problems (whether problems are "typically" male or female, and whether "sex-appropriate") on each of the following: perceived importance of client's presenting problems, specificity of the treatment plan to problems presented, and clinician's estimate of client prognosis. The results suggest that sex-role values of treatment personnel influence client comfort, that female clients are seen as having a poorer prognosis than males, and that presenting problems are perceived to be more important if they are sex-appropriate than if they are not sex-appropriate (the least important problems being female problems in a male client).

Metrizamide epidurography.

The results are reported of lumbar epidurography in 110 patients suffering from low back pain. By using the water-soluble non-ionic contrast metrizamide, it has been possible to perform the examination on an outpatient basis. None of the patients has suffered from headache, nor any other complaint often associated with other diagnostic techniques which involve lumbar puncture. Epidurography is sufficiently accurate to be regarded as the examination of choice in the diagnosis of lumbar disc prolapse.

The normal right ventricular response to supine exercise.

Nine normal, healthy young males performed graded, maximal supine bicycle exercise with monitoring of hemodynamic and gated blood pool parameters. Right ventricular ejection fraction (RVEF) was greater than 0.45 in all acquisitions at rest (mean +/- SD = 0.53 +/- 0.05) and increased greater than 0.05 in all subjects with maximal exercise (mean +/- SD = 0.69 +/- 0.06 at stage 4). There was a very close correspondence between RV and LV stroke counts from the radionuclide angiograms at rest and all exercise levels in all patients. Cardiac output changes by radionuclide data from the RV correlated closely with those obtained by the Fick technique. Multivariate analysis of RVEF vs pulmonary artery pressure, total pulmonary resistance, and heart rate showed that RVEF was inversely related to total pulmonary resistance in these patients (n = 43 acquisitions, r = 0.61, RVEF = 0.75 -- 0.001 TPR in dynes-sec/cm5 Sy.x = 0.07) and positively related to heart rate (n = 43, 4 = 0.78, RVEF = 0.34 + 0.0025 x HR Sy.x = 0.05). RVEF was not significantly related to the other hemodynamic parameters. These results suggest that in normal subjects RV systolic function is afterload-dependent and that total pulmonary resistance may be a suitable afterload index. The results emphasize that heart rate must be considered when comparing RVEF changes.
