Effects of silica on serum phospholipid, lipid peroxide and morphological characteristics of rat lung.

The effects of instilled silica have been studied on the serum-phospholipid (PL), lipid peroxide (LPO) and histopathology of rat lung up to 140 days from the first day of instillation. Silica induced relatively higher serum-PL throughout the experiment. The level of LPO also increased appreciably. They presented positive linear correlation. The early lesion was acute alveolitis with silica particles. These lesions became silicotic nodules on the 30th day, which then were enlarged gradually and fused by fibrosis. Alveolar macrophages (AM) were activated and surface structure was damaged. These results indicate that instilled silica can induce lipid peroxidation of cell membrane and selective accumulation of lung PL.

Sodium-hydrogen exchange inhibitors improve postischemic recovery of function in the perfused rabbit heart.

OBJECTIVE: The aim was to examine the effects of the Na+/H+ exchange inhibitors amiloride and methylisobutyl amiloride (MIA) in buffer perfused rabbit hearts subjected to one hour of normothermic ischaemia (37 degrees C) followed by reperfusion. METHODS: Experiments were carried out in five groups of Langendorff perfused rabbit hearts: (1) control, (2) amiloride, and (3) MIA (agents in both the preischaemic and reperfusion perfusate), (4) amiloride-R and (5) MIA-R (agents added at reperfusion only). Functional evaluation included serial measurement of resting tension, force, rates of ventricular force development and relaxation, and coronary perfusion pressure. Samples of coronary effluent were obtained for creatine kinase assay and hearts were freeze clamped for metabolite assays. RESULTS: Reperfusion resulted in a marked increase in resting tension in group (1) which was statistically significant compared to groups (2) and (3). Groups (2) and (3) also showed significantly improved recovery of ventricular force, rate of force development, and rate of ventricular relaxation. Addition of either agent only during reperfusion failed to produce a significant beneficial effect. There were no significant differences among the groups with respect to postreperfusion creatine kinase release or end reperfusion metabolite levels. CONCLUSION: This study shows for the first time that both of the Na+/H+ exchange inhibitors amiloride and MIA produce improved recovery of ventricular function in rabbit hearts subjected to ischaemia and reperfusion, although the beneficial effect was not obtained with drug administration at the time of reperfusion only.

[Relationship between thymectomy and syndrome differentiation-typing treatment of myasthenia gravis].

According to the theory of traditional Chinese medicine, 37 patients suffering from myasthenia gravis were Syndrome-differentiated into following types preoperationally: (1) Both Qi-Yin Deficiency of Kidney-Spleen, 21 cases. (2) Yang Deficiency of Kidney-Spleen, 15 cases. (3) Qi Deficiency of Spleen only, one case. Thymectomy were done in all cases. Among them, 19 cases had thymoma. The results showed the relationship between the types and the resectionability rate and post-operative crisis rate. On the first two types, they were 95.2% and 9.5%, 46.7% and 73.3% respectively. It was considered that Syndrome differentiation-typing in TCM in the myasthenia gravis patients would provide an positive and reliable index for predicting the resectionability and prognosis of the patient.

On the mechanisms of growth cone locomotion: modeling and computer simulation.

In this paper we put forward a model for growth-cone locomotion in which the actin cytoskeleton is represented by a two-dimensional contractile network and the growth-cone membrane is represented by contractile segments coupled with the actin network. Our computer simulation suggests that many mechanisms, such as the actin polymerization and depolymerization, the attachment of actin filaments to the substratum, the transformation of the growth-cone neck into the axon, and the axonal transport, co-operate to generate growth-cone movement.

[Long-term results of irregular hepatectomy for primary liver cancer at the right lobe].

From 1964 to 1990, 228 cases with primary liver cancer at the right lobe underwent irregular hepatectomy in our hospital. Of them, 47 were asymptomatic. Alpha feto-protein (AFP) was positive in 162 (71%) and 195 patients were associated with liver cirrhosis (85.6%). In 60 cases, the tumor size was less than 5cm in diameter. During operation, the following methods were used for interruption of hepatic blood flow: temporary blockage of the hepatic hilum in 117 cases, microwave coagulation of hepatic parenchyma in 77 cases, continuous mattress sutures in 29 cases, hepatic clamp in 3 cases and other methods in 2 cases. One segment resection was done in 116 patients, two segments in 67, three segments in 28 and four segments in 17. One hundred and fifty-five patients underwent radical resection and 73 patients underwent palliative resection. The operative mortality was 2.6%. The 3- and 5- year survival rates of the patients so treated were 48.3% and 38.8%, respectively. The 3- and 5- year survival rates of the patients with small tumor ( < 5cm in diameter) after hepatectomy were 59.9% and 53.3%, respectively. The medvan survival time after palliative hepatectomy was 15.7 months.

[Clinical factors affecting prognosis of small hepatocellular carcinoma after hepatectomy].

One hundred and two patients with histologically diagnosed small hepatocellular carcinoma (< or = 5cm HCC) underwent curative irregular hepatectomy in our hospital from Nov 1972 to Nov 1992. The postoperative survival rates at 1,3,5 and 10 years were 93.9%, 75.3%, 52.8% and 44.6%, respectively. We analysed the clinical factors using a Cox model and found that the clinical staging of HCC, gamma-GT, satellite tumor nodules, tumour capsule and postoperative recurrence have significant effect on the survival rate of patients. We conclude that early diagnosis of sub--clinical HCC, transarterial embolism after hepatectomy to reduce postoperative recurrence, and active treatment with operation and/or TAE after recurrence are effective methods to improve survival of HCC patients.

Regulatory action of Astragalus saponins and buzhong yiqi compound on synthesis of nicotinic acetylcholine receptor antibody in vitro for myasthenia gravis.

With overall analysis of symptoms and signs of myasthenia gravis (MG) basing on "Pi (Spleen) Deficiency" [symbol: see text] theory and with years of our clinical experience in treating MG, we performed a pharmacological study of Astragalus saponins and Buzhong Yiqi Compound (Tonic granulae invigorating vital energy) in 14 peripheral blood mononuclear cell (PBMNC) cultures from 10 MG patients. PBMNG from two groups of patients given dexamethasone (Dxm) and cobalt 60 (60Co) treatment were used as controls. The results showed that water soluble Astragalus saponins significantly reduced the titer of nicotinic acetylcholine receptor antibodies (nAchR-Ab) in the cell culture supernatants, from 418.8-2328 to nil in 6 cases, and from 1143-1235 to 43-157 fmol/ml in 2 cases, and that Buzhong Yiqi compound also had inhibitory immunoregulatory action.

Human internal thoracic artery reactivity to dopaminergic agents.

BACKGROUND: Vasospasm of the internal thoracic artery (ITA) is common. The purpose of this study was to characterize the vascular reactivity of human ITA to dopaminergic agents used in patients undergoing coronary bypass surgery (CBS): dopamine, dobutamine, and dopexamine. METHODS AND RESULTS: Segments of the distal ITA 3 mm long were obtained from patients undergoing CBS, mounted on two stainless steel wires in a tissue bath filled with Krebs' solution at 37 degrees C, and attached to a force-displacement transducer. To assess the functional status of the endothelium, the arterial segments were preconstricted with phenylephrine 10(-5) mol/L and then exposed to increasing concentrations of acetylcholine (10(-8) to 10(-5) mol/L). To study vessel relaxation, segments were again preconstricted with phenylephrine 10(-5) mol/L and concentration-relaxation responses to dopamine, dobutamine, and dopexamine (10(-8) to 10(-4) mol/L) were obtained. There was significant vessel relaxation with both dobutamine (log EC50 = -5.83 +/- 0.16) and dopexamine (log EC50 = -5.37 +/- 0.06) but none with dopamine. Vasoconstrictor effects of the drugs were studied from baseline tension. Neither dobutamine nor dopexamine caused vasoconstriction from baseline tension, whereas vessels exposed to dopamine showed constriction at high concentrations. The vessel responses to the dopaminergic agents did not differ according to the functional status of the endothelium as initially assessed by acetylcholine-induced vessel relaxation. CONCLUSIONS: High-dose dopamine may aggravate ITA vasoconstriction, whereas dobutamine and dopexamine appear to cause vasodilation only.

[Results of hepatectomy for huge primary liver carcinoma].

From 1964 to 1991, 166 cases of huge primary liver carcinoma (larger than 10 cm in diameter) had been resected in our department. One hundred and forty-four cases were male and 22 cases were female. The tumor was located in the left lobe of the liver in 75 cases, in the right lobe in 75 cases and in both lobes in 16 cases. The tumors were 10 to 14.9 cm in diameter in 109 cases, 15 to 20 cm in 45 cases, larger than 20 cm 12 cases. The largest tumor was 28 x 26 cm in size. In this series, most of the tumors were hepatocellular carcinoma (93.4%). The livers in 125 cases (75.3%) were associated with cirrhosis. Ten cases of them underwent regular hepatectomy. 156 cases underwent irregular hepatectomy. During the operation interrupted matress sutures, hepatic clamp, occlusion of the circulation of the hepatoduodenal ligament and microwave coagulation were employed to reduce blood loss. In this series, 95 cases underwent radical resection and 75 cases palliative resection. The post-operative complications were jaundice and ascites in 18 cases, wounds infection in 7 cases and right pleural effusion in 7 cases. Three cases died within 1 month after hepatectomy with an operative mortality of 1.8%. In our study, 64 cases had been followed up over 5 years. The 5-year survival rate of the radical resection group were 17.5%. All of the 67 patients who underwent palliative resection died 2 to 36 months after hepatectomy with a median survival time of 7 months. The biological characteristics of huge primary liver carcinoma and the approach to improve the prognosis were discussed.

Neurite branching pattern formation: modeling and computer simulation.

A model for nerve cell pattern formation is proposed in this paper. The model is based on some experimental results and an assumption that there is a kind of inhibitive interaction between growing neurites on the same nerve cell. In this paper, this interaction is termed lateral inhibition. A group of ordinary differential equations are used to describe the elongation of the terminal neurite segments of individual nerve cells. Computer simulation and comparison of it with in vitro studies are also made in this paper.

[Surgical treatment of primary liver cancer complicated with cirrhosis].

From 1964 to 1990, 318 cases of primary liver cancer were treated by surgery. Of them, 227 (71.3%) were complicated with cirrhosis to various degrees: mild, moderate and severe. The lesions in the latter group was greater than 10 cm in 3 patients, 5-10 cm in 7 and less than 5 cm in 17. All were confirmed by histopathology to be hepatocellular carcinoma. The method of operation was partial hepatectomy in 23 patients and intra-tumoral absolate alcohol injection supplemented by microwave coagulation in 4. The amount of absolate alcohol ranged from 18 to 30 ml with an average of 24 ml. The chief complications were jaundice and ascites (13 patients--48%) and gastrointestinal hemorrhage (3 patients--11%). Operative mortality was 15% (4/27). The 1-, 3- and 5-year survival rates were 82%, 39% and 10%.

A comparison of injury death rates in China and the United States, 1986.

BACKGROUND: The importance of injury as a public health problem is not well recognized in many developing countries. Data have recently become available on injury mortality in China. METHOD: We compared Chinese injury data based on a 100 million population segment for 1986 with data for the United States. RESULTS: The age-adjusted death rate from all injuries for China exceeds the rate for the US (69.0 vs 61.3 per 100,000). The US has higher death rates from motor vehicle crashes, fires, and homicide; China has greater mortality from drowning, poisoning, falls, and suicide. Especially noteworthy in China are the high drowning rates among young children and the elderly and the high suicide rates in rural areas among young adults and the elderly. CONCLUSION: Injury is an important public health problem in China, exceeding in many respects the problem in the United States. It is urgent for China to place high priority on injury research and prevention.

[Clinico-epidemiological investigation of schistosome-induced hepatosplenomegaly: a community-based study in Jishan, Xinjian County, Jiangxi].

Since the primary objective of mass chemotherapy in schistosomiasis control is reduction of schistosome-induced morbidity, it would be reasonable to assess the impact of a control program on the morbidity in a given population by investigating hepato-splenomegaly associated with schistosomiasis in a schistosomiasis endemic area. In this paper, the authors described the relationship between the prevalence, intensity and morbidity of Schistosoma japonicum infection in terms of stool egg count and ultrasonographically detectable hepatosplenomegaly in a community-based study. It was found that the epidemiological pattern of the infection in this study community was quite different from our usual understanding, that is, the prevalence remained relatively high (39.4%) when the intensity became lower. This unusual pattern might be resulted from intermittent and sporadic chemotherapy associated with frequent exposure of people to the infection in an area of high transmission. It was surprised to note that despite the praziquantel treatment carried out over the past years, the prevalence of hepatosplenomegaly induced by schistosomiasis in this community was still very high, suggesting that intermittent and sporadic chemotherapy might render little impact on schistosomiasis-induced morbidity. The investigation also showed that ultrasonography was a sensitive tool for assessing morbidity associated with schistosome infection.

Localization of small liver tumors.

Thirty-six patients with small liver tumor were diagnosed by alpha-fetoprotein (AFP); sonography, and computed tomography (CT), and underwent hepatectomy. The pathological types included 23 hepatocelluler carcinoma (HCC), 11 hepatic cavernous hemangioma, and 2 secondary liver cancer. In 22 patients, the tumor nodules were located in the right lobe and 14 cases in the left lobe. The diagnostic accuracy rate of CT was 100% for HCC and secondary liver cancer, but for hepatic cavernous hemangioma it was only 72.2%. However, the accuracy rate of sonography was as high as 81.8% for hepatic cavernous hemangioma and only 60.4% for liver malignancies. The positive rate of AFP for the HCC patients of this series was only 66.6%. The method of intraoperative detection of small liver tumor is introduced, if the tumor was invisible grossly or nonpalpable during exploratory laparotomy. In the series, 7 cases in whom the right lobe lesion was too small to be located by routine manual examination during exploratory laparotomy were detected by this method, and all small liver tumors were resected successfully.

[Experimental study of warming and recuperating kidney yang by you-gui-yin].

Yang deficiency animal models were induced by hydrocortisone 1 mg/day for 7-day to male mice. Then these model mice of experimental group were perfused by gastrogavage with 0.5 ml You-gui-yin (YGY) solution, the control group 0.5 ml normal saline 10-day. The plasma circular nucleotide and cortisol were radioimmunoassay by r-immune counter. The content of cAMP of experimental group was 144.24 +/- 33.35 pmol/ml, the control group 109.11 +/- 31.98 pmol/ml (P less than 0.05). The contents of cGMP were 28.39 +/- 10.22 and 45.39 +/- 15.33 pmol/ml respectively (P less than 0.05). The contents of cortisol were 12.42 +/- 2.21 and 8.96 +/- 1.19 micrograms/dl respectively (P less than 0.05). On the other hand YGY to the model mice could raised the living ability at low temperature. The results suggested that YGY had the effect of adjusting system circular nucleotide and promoting the secretion of adrenal cortex.

[Clinical analysis of 107 cases with chemical burns].

From 1978 to 1987 107 cases of chemical burns were treated. The patients were mainly young and middle age people (83.08%), with mean burned areas 17.8%. The majority of involved parts were low extremities. 57.44% of burned eyes became blind. Mean days in hospital were 43 days. Cure rate was 98.13% and death rate 1.87%. We emphasized that burned wounds must instantly be washed down with water, intoxication resulting from chemical matters must be effectively treated. Diagnosis of depth of burned wound by acids and means of prevention of chemical burns were also discussed.
