[Experience of a centre for the study of occupational adjustment disorders referred to psychological harassment-related diseases at the workplace].

BACKGROUND: The rising awareness of psychological harassment at the workplace means that it is ever more important to collect clinical data and test tools to assess psychiatric disease connected with this phenomenon. OBJECTIVES: The aim of the study was to assess the experience gained by the centre for work maladjustment of the University Hospital of Pisa on a sample of patients, using the methods proposed by the centre at the Clinica del Lavoro "Luigi Devoto". MATERIALS AND METHODS: The patients were submitted to a diagnostic protocol consisting of work history, medical examination, psychological assessment, psychiatric examination, subjective questionnaires, mood scale and Neutest. The analysis was made in two phases in which 109 subjects were examined to check occupational stress effects in the period March 2002 - July 2004, after which 50 subjects were selected (45.9%) with the adaptation disorder and a history of adverse working conditions. RESULTS: The data gathered showed a slight predominance of workers from the public administration sector. The most representative range of duration of the psychological violence varied from 6 months to 2 years (46%). From close examination of the causes of the maladjustment disorders, structural changes in organization appeared to be the most frequent (13 cases). Among patients with positive diagnosis of disorder caused by psychological violence at the workplace, three situations were prominent: attitude to being a social outcast, behaviour to ignore proposals, and professional declassing. CONCLUSIONS: The multidisciplinary approach was culturally very stimulating and fundamental in reaching a final diagnosis of occupational disease: diagnosis of bullying at work and related disease was possible only thanks to the fact that three specialists agreed on the aetiological role of psychological harassment at the workplace. Our experience confirms that psychological harassment can cause health impairment and the most common diagnosis is maladjustment disorder.

Medical surveillance of workers previously exposed to asbestos.

According to current Italian law health surveillance of workers exposed to asbestos in the past must be maintained even after cessation of asbestos exposure (D.Lgs. 277/91). The law, however, makes no reference to timescales or time limits for clinical monitoring, nor to who is responsible. The main aims of health surveillance of individuals formerly exposed to asbestos can be summarized as follows: early diagnosis of the principal diseases associated with asbestos; counselling for workers; work-related disease certification; epidemiological studies. Medical check-ups entail a complete personal and work history, Chest X-Ray, respiratory function tests, TLCO. In all cases the worker must undergo a LDCT. We propose two new possible biological markers for the health surveillance of past-exposed patients: serum mesothelin and osteopontin.

[Old and new pathologies proposed by epidemiology].

The results of a study made in the areas of Central Italy on a random community on the significant presence of cases with seroantibodies against Campylobacter, Yersinia, Legionella Pneumophila, Francisella Tularensis and Rotavirus confirm the hypothesis on the presence in the territory of microorganisms responsible. These positivities in man are not only single but also contemporaneous for at least two microorganisms. Hence the hypothesis of the presence in the environment (animals, soil, surface waters) of the microorganisms (Campylobacter, Yersinia, Legionella Pneumophila, Francisella Tularensis, and Rotavirus) responsible for the zoonoses of interest for man.

Successful embolization of a spontaneous carotid cavernous fistula fed only by the external carotid artery. Case report and literature review.

To the 38 cases previously reported of spontaneous carotid-cavernous fistulas (SCCF) fed only by the external carotid artery (ECA), we add one case in which a successful therapeutic embolization was performed. The importance of an exact angiographic diagnosis to differentiate between SCCFs fed by the ECA and those fed by both the ECA and the internal carotid artery (ICA), is emphasized.

[From a randomized assay, Francisella turned out to be epidemiologically present in the studied areas].

In the past Tularemia has largely affected animals, nowadays' in our country it could become actual one more because of wild animals repopulation actuated in many areas. Unlike the other countries, in Italy it's not well known if there are any consequences on population. The authors has conducted a research on this subject studying seroimmunologically samples taken randomly in some areas of south-central Italy. Their results confirm endemism and therefore announce the problem and the consequent pathology.

[Epidemiology of rotavirus infections in a randomized study in various central-southern areas of our country].

The etiology of diarrhea and intestinal deficiency relative to the absorbency of glucose and some enzymes can be found in Rotavirus infection. The epidemiology of this infection has been derived by the authors even if the research was limited to some areas of central-south Italy, analyzing seroimmunologically serums randomly selected. The research outcomes fully explain the first mentioned disease because antibody markers have been founded in most of the examined people.

[Randomized epidemiologic assay of Campylobacter infections in various areas of central-south Italy].

The authors on the basic of the diffusion of pediatric and gastroenterological pathologies in Italy, have conducted a research on the epidemiology diffusion of Campylobacter infection analyzing seroimmunologically sample randomly chosen and coming from some areas of central-south Italy. Their results confirm that in such areas the infection is endemic therefore they hypothesize that enteric vascular, bacterial pathologies and spontaneous abortions are related with it.

[There exists an epidemiology of Legionella pneumophila in our country].

The authors, after a brief relation on contribution given by international publications, relates about their study on Legionellosis epidemiology based on seroconversion results obtained comparing six different serotypes, precisely the ones from 1 to 6, over a sample of people living in the areas surrounding the two italian cities of Rome and Siena. This study has given proof of an high, and unaspected, seroconversion (over 27%) caused by Legionella Pneumophila. Titer was equal to or higher than 64, and there was a significantly higher incidence given by serotype 6 in both the examined areas.

[Investigations on herpes 1 and 2 infections].

The authors carried out a research on diffusion of Herpes virus 1 and 2 investigating specific serous antibodies over a sample of 1660 women living in Roma and in the surrounding area aged between 14 and 40 years; 595 of them were suspected to have an Herpes infection. The results of this research confirmed an high frequency of Herpes 1 and 2 infection in the examined sample. In order to correlate better the outcomes obtained by the investigation on the suspected sample 102 vaginal smear, executed on women suspected to have a developed infection, have been examined. The issues confirmed that it's useful to execute cytoimmunologic research in so far as such method seems to give more certainty in infection and disease identification than seroconversion investigation.

[Yersinia enterocolitica infection has taken hold even in our country].

The Yersinia enterocolitis is a kind of disease whose diffusion is gradually increasing in North Europe. The authors relates about the outcomes of a serum-immunologic research conducted during the period 1982-84 on a sample of 965 adults coming from three different italian districts (Roma, Siena, Napoli) and 65 children coming from Napoli. They interpret antibody titers painted out by their research comparing them with ones reported in bibliography. The issue of this research, even if it was conducted only in three bordering central-southern Italy regions, can anticipate an indicative epidemiological image of this Country.

[Toxotoxin can be etiologically determinant in toxoplasmic infections-diseases].

There have been some results of the antitoxotoxin investigation into antibodies in cases variously positive to the Dye test for the toxoplasmic infectious disease. The results point to various etiological considerations, therapeutic and therapeutic-prophylactic, on which the AA dwell briefly. They touch upon the possibility of a timely recourse to desensitization which, however, is sometimes not excluded even in cases which show a negative result to the toxotoxin.

[Type B viral encephalitis: HBsAg testing in saliva and cerebrospinal fluid].

The AA, taking the latest reports as a starting point, are attempting a first epidemiological reconsideration of Hepatitis, Virus B. They have found that the disease, which was generally thought not to be contagious, is actually deceptive in its potential for transmission from person to person. Hospital personnel, in particular, are greatly at risk as are their families.

Endovascular treatment of fusiform aneurysms with stents and coils: technical feasibility in a swine model.

PURPOSE: To assess the biomechanical feasibility of treating experimental fusiform aneurysms endovascularly with a combination of stents and coils. METHODS: An experimental model was surgically constructed in the necks of nine swine to simulate intracranial fusiform aneurysms possessing important "perforators" or side branches. Balloon-expandable metal stents were positioned across the aneurysms in eight swine. In five of these, additional treatment was intraaneurysmal placement of detachable microcoils. Attempts were made to deposit these coils strategically away from the origin of the side branch. RESULTS: Stent placement was successful in seven swine but failed in one swine because of stent-aneurysm size mismatch. Two swine treated with only stents showed no significant alterations in blood filling of the aneurysm or side branch. Satisfactory coil placement (outside the stent, within the aneurysm sac, and away from the orifice of the side branch) was achieved in four of the five swine treated with stents and coils. Careful fluoroscopic monitoring and controlled coil delivery were necessary to avoid covering the sidebranch origin. These aneurysms could not be packed densely after detachment of the first coil because of the resultant radiographic overlap of multiple coil loops on the stent and its lumen in all projections. In one swine there was inadvertent untoward reentry of the coil tip into the expanded stent lumen during its delivery. CONCLUSION: Endovascular treatment of experimental fusiforms aneurysms using stents and coils is technically feasible. The stent maintains patency of the parent artery while allowing strategic coil placement in the aneurysm sac away from the origin of side branches. This technique may prove useful in the future treatment of intracranial fusiform aneurysms. However, potential sources of technical difficulties have been identified, and further longterm studies using an appropriate intracranial stent will be necessary before human application.
