[Evaluation of the effect of cloxacillin on human lymphocyte chromosomes through the study of karyotypic changes and sister chromatid exchanges].

The aim of this work was the evaluation of the possible mutagenic effect of an antibiotic, the Cloxacillin. Therefore the authors have performed the analysis of the chromosomal alterations and the study of the SCE (Sister Chromatid Exchanges) induced with the exposition of cultures of lymphocytes to different concentrations of this drug. The work reveals that Cloxacillin induces chromosomal alterations at high concentrations, while at concentrations similar to its therapeutic levels the drug does not seem to interfere with human DNA.

[Effects of phototherapy with blue light on chromosomes from human lymphocytes].

The dangerous effects of phototherapy have been matter of discussion in recent years. In order to evaluate its in vitro action on the human DNA, the authors have performed the karyotypic analysis on 20 cultures of lymphocytes. In different times 16 cultures have been exposed to the action of a "blue light" fluorescent lamp, commonly used for the treatment of neonatal jaundice. The authors have not evidenced any morphological or numerical change of the karyotype in any of the cultures.

Comparison of three different methods of monitoring unwanted effects during antihypertensive therapy.

In a double-blind, randomized, crossover, multicenter study performed in 227 hypertensive outpatients, two antihypertensive drugs, oxprenolol and chlorthalidone, were investigated to determine unwanted effects. Three main methodologic procedures were applied: the conventional evaluation of unwanted effects by the physician and involving the whole patient population, the checklist, and the free questionnaire. The latter two were assigned by randomization to the patients themselves. The study showed that unwanted effects seem to be frequently overreported when a checklist is used, whereas the free questionnaire gives more information about the symptoms of the disease than about the tolerance of the drug. The conventional method, when physicians are well-informed and sensitized to the problem, seems to provide more reliable information about unwanted effects of a drug.

Post-traumatic headache: neuropsychological and clinical aspects.

Seventy patients suffering from post-traumatic headache were studied. Pain characteristics, personality and intellectual functions were assessed to be related to cranial trauma. No evident signs of brain damage were present, but an impairment related to pain in personal adjustment and well-being reducing work and study capabilities was identified. A psychopathological condition described as anxiety with somatizations and conversion mechanisms was found and when compared with the psychopathological characteristics from a group of common headache patients no differences were obtained between the two groups. DSM III diagnostic possibilities for post-traumatic headache patients were discussed.

Laparoscopic splenectomy: A retrospective review of 75 cases.

Laparoscopic splenectomy (LS) is considered a safe procedure for spleens of normal size as well as for larger spleens. Seventy-five consecutive patients underwent LS. Splenomegaly was defined by diameter >15 cm and by weight >400 g. Thirty patients had splenomegaly. The outcomes with spleens <15 cm and spleens >15 cm were compared. LS was successfully completed in 73 cases (97.4%). Spleens >15 cm required longer operating time and were associated with greater blood loss (P < 0.001), longer hospital stay, and more complications. Two patients needed blood transfusion. No overwhelming postsplenectomy infection was registered, and operative mortality was zero.

Osteoporosis in patients affected with thalassemia. Our experience.

The authors examine 72 patients affected with homozygous B-thalassemia; The study was conducted by clinical-hematological and radiologic examination. The Singh method is used to compare clinical data with the degree of osteoporosis. The results indicate that there is a high frequency of osteoporotic abnormalities in thalassemia. The authors postulate that osteoporotic lesions are principally caused by hyperplasia of the marrow, the overactive bone marrow widening the medullary space and the increased intramedullary pressure causing osteoporosis.

[A modified Putti-Platt surgical method in the treatment of habitual luxation of the anterior shoulder: our experience].

Fifty cases of recurrent dislocation of the shoulder operated according to the Putti-Platt method modified by lateralization of the insertion of the coracoid processes are reported. Excellent results were obtained in 86% of the cases. There was neither incidence of recurrence, nor degenerative joint alteration. Mild limitation in extrarotation (less than 10 degrees) was observed in 7 cases.

[Auto- and homoplastic grafts in the treatment of osteolytic lesions].

Based on 32 cases of neoplastic and degenerative lesions, the indications and limits of auto- and alloplastic grafts are discussed. Grafting was successful in all of the cases, obtaining excellent clinical and radiographic results. An auto graft may be used to more rapidly fill cavities which remain after curettage has been performed, but the limited availability of grafts, the need for a second operation, and the weakening of the donor site encourage us to develop more practical methods of removal and preservation of the allograft.

[Osteitis condensans ilii: therapy and diagnostic problems. Presentation of a case study].

The authors describe 4 female patients with Osteitis Condensans Ilii (OCI): 3 of them were pluriparous, ranging 32-36 years of age; while the remaining was nulliparous and 20 years old, with a previous trauma suffered in the iliac bone. Due to the inefficacy of the medical and physical treatment, two patients underwent surgical resection of the osteitic bone; then followed by a 45 days cast immobilization and a 3-4 months with restricted load. The other 2 patients were successfully treated with Indomethacin (a 100 mg/day cycle for a month, followed by a maintainance cycle with 300 mg/day for 3 months). The diagnostic aspects are then discussed, pointing out that absence of any peripheric neurologic damage, normal bone scan, female sex and younger age are the relevant characteristics for the diagnosis of OCI.

[Instrumental diagnosis in shoulder instability].

The authors call attention to the pathology caused by glenohumeral instability and, in particular, to painful shoulders in athletes which so often cause problems in diagnosis. An instrumental protocol for diagnosis is suggested, based on several specific radiographic views, Ct scan and arthro-Ct scan, with double contrast medium, the latter having the task of determining lesion which would not otherwise be able to be studied.
