Histamine metabolism and female sex hormones in women.

Oral combined contraceptives did not seem to alter histamine metabolism in females. During treatment with gonadotrophic hormones in four amenorrhoeic patients there was a tendency towards increasing excretion of methylhistamine (MeHi) followed by a sudden decrease corresponding to changes in the urinary estrogen. The excretion of methylimidazoleacetic acid (MeImAA) seemed to parallel that of MeHi. The findings support the hypothesis that an endogenous surge of estrogen may influence histamine turnover in women. Women of post-menopausal age have about the same histamine metabolism as younger menstruating women. Estrogen medication relieved symptoms of hot flushes or sweats but did not seem to affect the histamine turnover.

Histamine metabolism in polycythaemia vera.

Histamine metabolism was studied in 35 patients with polycythaemia vera (PV) at different stages of their disease and compared with controls and patients with secondary polycythaemia. In addition to blood and urinary histamine the main urinary metabolites of histamine, methylhistamine (MeHi) and 1-methyl-4-imidazoleacetic acid (MeImAA) were measured. In patients with active PV the excretion of MeHi and MeImAA was significantly higher than in controls and secondary polycythaemia, indicating an increased histamine formation. The MeImAA excretion was correlated to the blood histamine level, the degree of blood basophilia, the total white blood count and the spleen volume. The blood histamine level was significantly higher in PV patients compared with controls and secondary polycythaemia. No patients with secondary polycythaemia had an increased blood histamine level. With the bio-assay technique used in this study the urinary excretion of histamine in the PV patients was within the normal range. There was no correlation between the increased histamine formation and "histamine-related symptoms". Pruritus and duodenal ulcer occurred with a similar frequency in patients with and without increased MeImAA excretion. The similarity between the disturbance of the histamine metabolism in PV and that found by other authors in chronic myeloid leukaemia is pointed out.

Histamine formation from 14 C-L-histidine in man.

1. (14)C-L-histidine was given i.v. to one normal subject and two patients with chronic myelocytic leukaemia. Urinary excretion of histamine and two of its metabolites methylhistamine and methylimidazoleacetic acid, total as well as (14)C-labelled, was measured, as well as blood (14)C-histamine. In addition the total urinary and pulmonary elimination of (14)C was followed.2. Total (14)C elimination was high during the first days, then declined slowly except for a plateau at the 10th-14th day in the two patients. There was a measurable elimination even after some months.3. (14)C-histamine appeared in the blood of the leukaemic patient, whereas in the normal subjects the values were hardly measurable.4. The leukaemic patients excreted much more of the two (14)C-labelled histamine metabolites than the normal subject. The difference in excretion of the (14)C-labelled metabolites was largest around the 12th day after the infusion of (14)C-L-histidine.5. The results indicate that the leukaemic patients formed at least 20 times more histamine daily than the normal subject.

Diabetic cystopathy--a risk factor in diabetic nephropathy?

Twenty-seven patients with Type I diabetes and diabetic nephropathy were repeatedly tested (mean, 6 times per patient) for residual urine volumes with a noninvasive technique. Results in 43 of 162 investigations (27%) were abnormal, with residual volumes of greater than 15 ml. Twelve of 162 (7%) showed a residual volume greater than 100 ml. In any individual patient the occurrence of residual volumes was not a consistent finding, and the volumes varied. Pathologic residual volumes were more common in men, but all those with bacteriuria were women. All patients with residual volumes (N = 16) were given voiding instructions. There was no increase in residual volumes during the observation period (mean, 32 months), the median residual volume being 8 ml at the first observation and 5 ml at the last observation. The occurrence of residual urine could not be shown to correlate with progression of renal insufficiency. It is suggested that all patients with long-standing Type I diabetes should be tested by a noninvasive technique for residual urine volume and given voiding instructions to avoid acute retention episodes and complete atony of the bladder.

Effect of captopril on renal hemodynamics in the treatment of resistant renal hypertension.

The effects of 6 weeks of treatment with captopril on the renal hemodynamics of 16 patients with treatment-resistant renal hypertension (six had diabetic nephropathy, seven had other renal parenchymatous disease, and three had renovascular disease) were studied. Significant changes in glomerular filtration rate, filtration fraction, plasma renin activity, urinary aldosterone, and mean blood pressure were noted in the patients with renal parenchymatous disease, but not in those with diabetic nephropathy. Renal blood flow remained unchanged in all patients. Captopril was well tolerated.

Glomerular filtration rate estimated after multiple injections of contrast medium during angiography.

In twenty-six patients referred for angiography, clearance of contrast medium was determined with x-ray fluorescence analysis after multiple injections of contrast medium. A formula for correction of the injected amount, which takes into consideration the different times of contrast medium injections, approximating the total injected amount into one injection, was used. A single injection clearance of 51Cr-EDTA was determined at the same time. The results showed a good correlation between the clearance of contrast medium after multiple injections and the 51Cr-EDTA clearance after a single injection (r = 0.945). The correlation between contrast medium clearance calculated without correction for the different injection times, and 51Cr-EDTA clearance was the same (r = 0.946), due to short angiography time and rather low clearance values in our patients. It is concluded that total plasma clearance of contrast medium can easily be estimated after multiple injections. In this way patients with a risk of developing post-angiographic renal failure can be found.
