Renal function in idiopathic hydronephrosis in children. Follow-up after conservative and surgical treatment.

In 64 children (age 1-15, mean 7.3 years) with unilateral idiopathic hydronephrosis, measurements were made of glomerular filtration rate (GFR), separate glomerular filtration rate (SGFR, measured as 51Cr-EDTA clearance combined with renography) and renal concentrating capacity. Onset symptoms were urinary tract infection in 34 children and abdominal pain in 25, while 5 were asymptomatic. Surgery was performed soon after the first investigation in 34 children and during the follow-up period in 14, and 16 children received only conservative treatment. The mean follow-up time was 4.4 years. At the time of diagnosis the total GFR and the SGFR of the affected kidney were normal or almost normal in all the children (respective means 108 +/- 16 and 50 +/- 12 ml min-1 X 1.73 m2(-1)). The total GFR remained normal at follow-up, but SGFR became subnormal in two conservatively and two surgically treated children. The concentrating capacity was initially lower in the children with, than in those without urinary tract infection. The former values had increased significantly at follow-up examination. The study indicates that the parenchymal function in unilateral idiopathic hydronephrosis in children more than 1 year old usually is normal, but may deteriorate due to urinary tract infection. Such infection should be carefully searched for and treated. Since very few of these children seem to have pelvic obstruction, surgery seldom is primarily necessary.

Preoperative diagnosis of renovascular hypertension. The use of acute stimulation of renin secretion.

The gain in the diagnostic power of the routine preoperative bilateral renal vein renin investigation by the inclusion of an acute stimulation of renin release was studied in 25 hypertensive patients with fibromuscular (FMD) and 44 with arteriosclerotic (AS) renovascular disease. Nine FMD and 17 AS patients had renal vein renin ratios greater than or equal to 1.50 under unstimulated conditions. Of these 26 patients, 8 in each group improved after surgical treatment. Among the 14 FMD and 24 AS patients with ratios less than 1.50, another 9 FMD and 7 AS patients improved after operation. After acute stimulation of renin release, no less than 18 FMD and 26 AS patients had a unilateral renin secretion from the diseased kidney and of these, 14 FMD and 14 AS patients were successfully treated with operation of the stenosis. Thus the stimulation was necessary for correct preoperative diagnosis in 38% of 32 successfully operated patients, and at the same time the predictive value of a negative test increased from 0.58 to 0.80. It is concluded that acute stimulation of renin release greatly improves the diagnostic power of the renal vein renin investigation in renovascular hypertension.

Histamine symptoms and histamine metabolism in chronic granulocytic leukaemia.

Histamine metabolism was investigated in three patients with chronic granulocytic leukaemia (CGL) during symptoms suspected to be caused by histamine release. Plasma histamine levels were excessively high compared with periods when the patients were symptomless or with CGL patients without such symptoms. The patients seemed to be adapted to high plasma histamine levels as the symptoms were not typically systemic in nature, but rather localized phenomena like oedema and pruritus in the extremities. Also CGL patients without symptoms showed abnormally high plasma histamine concentrations, which were significantly related to the whole blood histamine concentration. The possibility must be considered that part of the plasma histamine increase is artificial due to rupture of the basophils during the blood collection procedure.

Studies on histamine metabolism in mastocytosis.

The urinary excretion of histamine and its main metabolite, 1-methyl-4-imidazoleacetic acid (MeImAA), was determined in 30 adult patients with the clinical diagnosis of urticaria pigmentosa (UP). Clinical and laboratory investigations including skin histology, bone marrow examination, and scintigraphy of the skeleton, liver, and spleen revealed systemic manifestations in 14 cases. Among the 16 cases with dermal proliferation of mast cells only 3 cases classified as telangiectasia macularis eruptiva perstans (TMEP). All patients with systemic mastocytosis and UP excreted increased amounts of MeImAA in the urine while normal amounts were found in 2 of the patients with TMEP. A significant correlation existed between MeImAA excretion and the extent of mast cell infiltration in skin and internal organs. No such correlation was found for urinary histamine. Urinary MeImAA but not histamine is therefore considered a useful indicator of systemic involvement by reflecting the size of the mast cell histamine pool. The main symptom of the patients was pruritus, which was moderate to severe in 17 and mild or absent in 13 cases. Gastrointestinal symptoms were present in 14 patients. However, there was no obvious correlation between the excretion of MeImAA and any of the symptoms recorded. Neither was the severity of pruritus correlated to the histamine content of the skin, which was measured in both lesional and unaffected skin in 23 of the patients. Thus, symptoms possibly caused by histamine in mastocytosis patients are not directly related to urinary histamine metabolite excretion or tissue histamine content.

Effect of antihistamins (H1 and H2), cortisone and beta-adrenergic stimulator on murine contact sensitivity and histamine metabolism.

A study on the influence of histamine receptor blockade, beta-adrenergic stimulation and cortisone on murine cutaneous inflammation and histamine metabolism has been performed. Cortisone was found to reduce the inflammation and urinary histamine excretion while histamine receptor blockade and beta-adrenergic stimulation did not influence the reaction. A good correlation existed between inflammation and urinary histamine excretion in all experiments. Contact sensitivity in the mouse was not found to be transferable with serum indicating that humoral antibodies play no essential role in this reaction in the mouse. On the basis of these results it is suggested that basophils might participate in murine contact sensitivity.

Increased urinary methylimidazoleacetic acid (MelmAA) as an indicator of systemic mastocytosis.

The urinary excretion of histamine and its main metabolite, methylimidazoleacetic acid (MelmAA), was determined in 25 adult patients with the clinical diagnosis of urticaria pigmentosa (UP). Extensive clinical and laboratory investigation, including skin histology, bone marrow examination and scintigraphy of skeleton, liver and spleen, implied systemic manifestations in 16 cases. All patients with systemic mastocytosis (SM) excreted abnormal amounts of MelmAA (greater than 4.1 mg/24 h) and most of them 8.0 mg or more per day, while histamine excretion was increased in only nine (greater than 40 microgram/24 h). Thus, the urine content of MelmAA, but not histamine, could differentiate between UP and SM. Severe pruritus was found concomitant with increased urinary MelmAA and indicated systemic mastocytosis.

Renal extraction ratios for 51Cr-EDTA, PAH, and glucose in early insulin-dependent diabetic patients.

Renal clearances and extraction ratios for 51Cr-EDTA and PAH were studied in six young patients with insulin-dependent diabetes. The duration of the disease was 1 to 4 years, and no patient had signs of diabetic complications. Catheterization of the right renal vein and the left brachial artery was performed. The extraction ratios for 51Cr-EDTA, PAH, and glucose were determined at two different levels of blood glucose. Clearance for 51Cr-EDTA was increased by 9.7% and for PAH by 8.5% compared to normal control subjects while the extraction ratios for 51Cr-EDTA and PAH were normal. Extraction ratio for glucose was very low. There was no correlation between the individual HbA1 values and the clearances for 51Cr-EDTA and PAH. Extraction ratios for these substances were not influenced by acute changes in blood glucose level. The normal PAH extraction ratio indicates that PAH clearance is a reliable estimate of RPF in early insulin-dependent diabetes.

Stroke volume measurement by impedance cardiography using a formula based on the delta z waveform.

In 12 patients with systemic hypertension the cardiac stroke volume was determined simultaneously by the dye dilution technique and by impedence cardiography. The impedance stroke volume was calculated using Kubicek's formula (using four different values of blood resistivity) and also by the formula sv=AL delta z/Z0 (from the transthoracic impedance change, the delta z waveform). Significant correlations were found between the stroke volume measured by the impedance and reference methods and there were significant differences between the correlation coefficients (range 0.81-0.92) irrespective of which formula was used or which value of blood resistivity was inserted in the Kubicek formula. Only the delta z formula gave no significant differences for both the mean difference and the regression coefficient. Measurement of the stroke volume with the delta z formula requires breath holding, but determination of the systolic ejection time or blood resistivity is not necessary, unlike the Kubicek formula. A low peak value of dz/dt indicated a poor correlation with the dye dilution method, and also indicated a reduced maximum rate of ejection of blood from the heart. The effective delta z formula affords a different approach for future investigations of the genesis of the cardiac impedance changes in the thorax.

The protective property of equipotent bronchodilating doses of inhaled KWD 2131 and terbutaline against allergen-induced bronchospasm.

The anti-allergic capacity of nebulized KWD 2131 in inhibiting allergen-induced bronchospasm was compared with that of terbutaline in equipotent bronchodilating doses. It was a double-blind cross-over and randomized study also including placebo. Twelve symptom-free extrinsic asthmatics participated in the trial. Equipotent bronchodilating doses of the two beta-receptor-agonists were established in a histamine challenge procedure before the start of the study. Allergen challenge was performed with double concentration steps every 10 min until a greater than or equal to 20% decrease in FEV1 was achieved. Significantly (P less than 0.001) more allergen dose steps could be used after pretreatment with KWD 2131 and terbutaline than with placebo. No difference could be observed between the active compounds. The same allergen log dose gave a 20% decrease in FEV1 after pretreatment with KWD 2131 and terbutaline. The active compounds' protective properties did not differ at a lower degree of allergen challenge. The plasma histamine level was not significantly changed at any point of allergen challenge after pretreatment with either placebo or the active compounds. Therefore, plasma histamine determination was of no value for evaluating the inhibition of mediator release by these drugs. It is concluded that the anti-allergic property of KWD 2131 at allergen challenge does not give any further clinical value besides the bronchodilating property.

Cardiac output in normal pregnancy measured by impedance cardiography.

Impedance cardiography was used to study cardiodynamic changes throughout normal pregnancy. Stroke volume, heart rate and cardiac output were determined in fourteen normal primigravidae from early pregnancy until 6-25 weeks after delivery. To elucidate the influence of the patient's position on cardiac output, measurements were made in the supine as well as in the left and right lateral positions. Stroke volume and cardiac output increased up to the 28th week of pregnancy and then decreased during late pregnancy. These changes seemed to be independent of the patient's position. After delivery a further reduction in cardiac output due to a significant decrease in heart rate was found. No significant differences in cardiac output were found between right and left lateral position. However, during pregnancy as well as after delivery, cardiac output measured by impedance cardiography was significantly higher in the supine position than in the lateral positions. Thus, the impedance method does not seem to be reliable in measuring absolute values of stroke volume and cardiac output but may be used for relative measurements during pregnancy.

Decreased urinary histamine metabolite after successful PUVA treatment of urticaria pigmentosa.

The effect of PUVA therapy on pruritus, the skin mast cell population and histamine metabolism has been studied in 3 patients with urticaria pigmentosa and manifestations of systemic mastocytosis. Relief of itch was found concomitant with a significant decrease of the major histamine metabolite 1-methyl-4-imidazoleacetic acid in the urine. The decrease occurred during the first 2 mo after starting PUVA therapy and was sustained during an observation period of 3 mo after discontinuation of the treatment. At this time a reduction of the number of mast cells was found in skin biopsy specimens. No evidence of acute histamine release in association with PUVA treatment was obtained. These results suggest that this effective new treatment for urticaria pigmentosa reduces the histamine turnover in the skin by inhibiting mast cell proliferation.

Reference values for 51Cr-EDTA clearance as a measure of glomerular filtration rate.

Reference values for glomerular filtration rate (GFR) were defined using eight reports including epidemiological studies and studies in kidney donors. Studies using both inulin and 51Cr-EDTA were included. GFR decreased with age, by 4 ml/min decade below the age of 50, and 10 ml/min decade above 50 years of age. No sex difference was found. +/- 2 SD was equal to 25 ml/min at all ages. Based on these findings a nomogram for GFR is presented. Emphasis is given to the use of plasma clearance of 51Cr-EDTA estimated with single injection technique as the new reference method for GFR measurement.

Augmented parietal cell sensitivity to histamine after cimetidine treatment in healthy subjects.

Basal and histamine-stimulated (0.4, 1.6, 6.4, and 25.6 micrograms . kg-1 h-1 of histamine dihydrochloride) gastric secretion was studied in 2 groups of 10 healthy young subjects before and 60 h after cessation of 4 weeks of cimetidine treatment, 1 g/day. Basal acid and pepsin outputs were not significantly different before and after treatment. After cessation of treatment acid output in response to the two lowest doses of histamine had increased significantly, whereas the output in response to the two highest doses of histamine was unchanged. The results are consistent with a hypersensitivity of the parietal cells to histamine stimulation after cessation of cimetidine treatment in healthy subjects. Pepsin output in response to histamine stimulation and catabolism of exogenous histamine were not influenced by treatment with cimetidine.

Metabolism of 14C-histamine given intrabronchially to asthmatic patients.

14C-histamine was administered intrabronchially to asthmatic patients and controls. The urinary excretion of total radioactivity, 14C-histamine and its radioactive metabolites was measured. It was found that the excretion of total radioactivity was complete within 24 h. The excretion rate was equal to that observed after intravenous injection of 14C-histamine, indicating a rapid penetration of the bronchial mucosa. However, the diuresis seemed to be of importance for the excretion rate. Histamine inactivation by methylation was more pronounced after intrabronchial than after intravenous administration.
