Radioactivity in breast milk and placentas during the year after Chernobyl.

After the April 1986 nuclear reactor accident at Chernobyl in the Union of Soviet Socialist Republics, samples of human placenta and breast milk were tested for 1 year to determine the levels of radioactivity. The radionuclide iodine 131 was never beyond the detection limit of our gamma detector for both matrices. As to cesium isotopes 134 and 137, the highest levels detected in breast milk (6 Bq.L-1) and placenta (15.8 Bq.kg-1) were recorded in March 1987. Study data for breast milk and placenta are in agreement with the values calculated by means of double-compartment food-milk and food-placenta models. With regard to placental content, the cesium contribution to the average dose during the year after the Chernobyl accident was calculated to be 40 to 60 microSv.

Selectivity of oral aspirin as an inhibitor of platelet vs. vascular cyclooxygenase activity is reduced by portacaval shunt in rats.

Oral aspirin can be extensively hydrolyzed to salicylate in the stomach and liver before it enters the systemic circulation. "Presystemic" acetylation of platelets may thus occur during aspirin absorption. This may result in concomitant sparing of peripheral vascular cyclooxygenase mainly exposed to salicylate. We tested whether the "biochemical selectivity" of p.o. aspirin as an inhibitor of platelet rather than vascular cyclooxygenase was reduced by elimination of the "first-pass" hepatic metabolism. A portacaval shunt was inserted in anesthetized rats by connecting the portal vein to the inferior vena cava through a heparinized polyethylene "Y" cannula. Sham-operated rats acted as controls. Ninety minutes after recovery from anesthesia rats were given aspirin p.o. (10 mg/kg) and 45 min later serum thromboxane B2 and 6-keto-prostaglandin F1 alpha formation by vascular rings was evaluated by radioimmunoassay. Serum thromboxane B2 was almost suppressed completely in all animals; vascular 6-ketoprostaglandin F1 alpha was reduced significantly (by 40-60% in aorta and vena cava) in rats with the portacaval shunt but not in sham-operated animals. The results in rats with the shunt were similar to those obtained previously after i.v. aspirin. Fifteen minutes after aspirin, plasma levels of unmetabolized drug measured by high-pressure liquid chromatography were significantly higher in rats with portacaval shunt (0.56 +/- 0.16 micrograms/ml; n = 5) than in sham-operated controls (0.16 +/- 0.02 micrograms/ml; n = 5). These findings support directly the role of first-pass hepatic metabolism in the "biochemical selectivity" of p.o. aspirin.

Gastro-entero-pancreatic hormones in amniotic fluid from normal and diabetic pregnant women.

Clinical applications of analyses of hormones in amniotic fluid (AF) have recently been increased. In diabetic pregnancy, determinations of insulin and C-peptide in AF have been suggested as good indicators of the status of the foetus. We have investigated the pancreatic alpha and beta cell function by measuring insulin (IRI), C-peptide (CPR), glucagon (IRG), somatostatin (SLI), and gastric inhibitory polypeptide (GIP) in amniotic fluid collected during basal conditions or 2 h after an arginine test in 92 diabetic and 32 non-diabetic pregnant women. During basal conditions, in diabetic pregnant women, IRI, CPR and the insulin: glucagon molar ratio (I/G) were all significantly higher while amniotic fluid-IRG was significantly lower than in the controls. After arginine stimulation, IRI increased in AF of the diabetic pregnant women but not in AF of the controls while no differences were observed in AF-GIP and AF-SLI concentrations. Higher IRI and CPR, as well as lower IRG values were significantly related to poor maternal metabolic control. The occurrence of neonatal morbidity including macrosomia was significantly associated with increased AF, IRI and CPR concentrations after an arginine challenge and these factors were the most sensitive predictors of neonatal morbidity in infants of diabetic mothers. Increased AF glucose concentrations and I/G ratios were related to neonatal hypoglycaemia; jaundice and respiratory distress syndrome were associated to low concentrations of SF-IRG.

Clinical study and results of surgical treatment in 146 oligospermic men with varicocele.

Data of 146 infertile men undergoing surgical treatment for varicocele are reported. Patients with the following features were treated by surgery: normal FSH (follicle-stimulating hormone) not severe hypotrophy of the left testis and normal right testis; absence of azoospermia; absence of autosome or sex chromosome alterations; absence of signs of genital inflammation. A clear-cut improvement in the condition of seminal secretion was obtained both in oligospermic patients with less than 10 millions spermatozoa per ml preoperatively, and in patients with a higher number of spermatozoa. Particularly, a marked improvement in the valid motility and morphology of spermatozoa was observed. The biological index of a successful treatment in correcting infertility is represented by the high number (51) of successful pregnancies in the wives of operated patients. It is to be underlined that measurements of spermatic vein pressure, phlebography and intraoperative cineradioscopy completely avoid recurrence that otherwise affects about 5 to 6% of operated patients.

Hydroxychloroquine versus phlebotomy in the treatment of porphyria cutanea tarda.

Hydroxychloroquine and phlebotomy were compared in the treatment of porphyria cutanea tarda (PCT). Thirty patients received hydroxychloroquine (200 mg twice weekly) for 1 year and thirty-one underwent twice-monthly phlebotomies of 400 ml whole blood each, also for 1 year. Clinical signs of disease improved equally in both groups. At the end of the year, urinary porphyrin excretion had significantly improved in twenty-two out of thirty hydroxychloroquine-treated subjects, but in only eight out of the thirty-one patients who received phlebotomy. Liver histology showed significant regression of steatosis and siderosis in both groups compared with the pretrial biopsy, but the activity of liver disease, as judged by the extent of necrosis, inflammation and fibrosis, worsened in twelve hydroxychloroquine and in seven phlebotomy-treated patients. It is concluded that hydroxychloroquine is more effective than phlebotomy in decreasing porphyrin production. However, further work is needed to assess whether long-term hydroxychloroquine treatment favours the progression of the chronic liver disease associated with PCT.

Influence of spasmolytic treatment and amniotomy on delivery times: a factorial clinical trial.

In a double-blind factorial clinical trial in 300 patients (150 primiparae and 150 multiparae), the effects of amniotomy, rociverine, and butylscopolamine bromide administration on the course of labor were investigated. Rociverine significantly reduced the dilatation time and had no effect on the delivery time. Butylscopolamine bromide had no appreciable effect on the dilatation and expulsion times. Amniotomy resulted in a lengthening of the dilatation time and no significant change in the expulsion time.

"Carnitine deficient" myopathy and cardiomyopathy with fatal outcome.

A 13-year-old boy with a mild limb girdle muscular weakness had a massive rhabdomyolysis and cardiac arrest after general anesthesia. A congestive cardiomyopathy then developed. Muscle biopsy revealed an unspecific "myopathic" degeneration of muscle fibers and a slight accumulation of lipid droplets. Carnitine content was markedly reduced in muscle and moderately in plasma. Both prednisone and carnitine therapies were unable to improve the heart insufficiency, and the patient died 1 year after the acute episode of rhabdomyolysis.

Plasma and semen androgens in normospermic and dysspermic subjects.

The seminal and plasmatic concentrations have been examined of testosterone, 5 alpha-dihydrotestosterone, and delta 4-androstenedione in normospermic and dysspermic subjects. The results obtained did not provide evidence of significant differences of androgen levels in fertile, hypofertile and sterile men.
