[Psychosocial aspects of the long-term management of Parkinson patients].

1309 hospitalised parkinsonian patients were examined in respect of mental changes and the consequences for their participation in the life of the community. The paper expresses the association of their disability with the degree of rigidity, tremor, akinesis, and the gravity of the overall disorder.

[Acute cerebrovascular disease at the internal medicine hospital].

On the basis of the medical reports of 255 patients with an acute cerebral insult the clinical symptoms, the courses of the illness as well as the diagnostic procedure were discussed. 76 of 255 patients (29.8%) are taken ill before they reach their 65th year, which stresses the sociomedical importance of acute cerebrovascular syndromes. With patients under 65 years the clinical syndrome corresponds to the following stages of the cerebrovascular insufficiency: stage IIa-14.5%, stage IIb-43.4%, stage III and IV-36.8%. A brain embolism is found in 5.3% of the cases. If there is an acute cerebrovascular insufficiency the ultrasonic Doppler detection as a noninvasive procedure should be the beginning of the paraclinical diagnosis. Should the ultrasonic Doppler detection give an indication of extracranial stenosis or obstruction of the A. carotis, then an angiographic diagnosis ought to be started in order to make possible a surgical treatment of the A. carotis.

[Spontaneous ventriculocisternostomies].

A pantopaque ventriculogram is used to demonstrate spontaneous ventriculocisternostomy over the r. suprapinealis in a patient with benign occlusion of the aqueduct. Clinical remission corresponds to sufficient spontaneous liquor drainage. Thus, a total of thirteen real perforations of the third ventricle have so far been described in the literature in addition to nine complete lateral ventricular ruptures. The differences between incomplete ventricular wall ruptures, ventricular diverticula, and ventriculocisternostomies are discussed.

[Significance of the anterior falcial artery].

The author describes origin and course of the anterior falcial artery under physiological conditions. In the carotis communis arteriogram without using the subtraction technique it can be recognised in about one per cent of the cases. Of pathophysiological importance is a dilated anterior falcial artery serving as a collateral artery in occlusions of the carotis interna and as an affluent artery in falcial menigiomas, which is demonstrated in an example. The combination of an a. primitiva trigemini with a homolateral traumatic dural arteriovenous shunt and dilated anterior falcial artery is shown in another patient.

[The moyamoya syndrome].

The Moyamoya vascular convolutions are interpreted as an unspecific collateral system in unilateral and bilateral progressive stenosing of the A. carotis interna and additional transdural externainterna anastomoses. In such cases one should speak of a Moyamoya syndrome. The designation Moyamoya disease only applies to the congenital form. In an unclarified case observed by the authors, which however showed clinically and angiographically the typical findings of the Moyamoya syndrome, a haemodynamically effective anastomosis of the A. occipitalis externa to the A. vertebralis on the left side was also found for the first time in the series of transdural inflow variants.

[Current status of diagnosis of the cerebrospinal-fluid space using Amipaque (metrizamide)].

A report is given on 73 examinations using Metrizamide (8 thoracolumbar myelographies, 18 ventriculographies, 47 suboccipital myelographies) under consideration of the latest literature. Subsequent systematic EEG-controls were carried out in 52 patients. Exact observance of the indication, adequate dosage, optimum preparation and after-treatment of the patients as well as a appropriate operation technique can limitate the vegetativ side-effects and avoid rarely occurring serious complications. Publications in the literature as well as our own experiences stress the excellent representation of the details of the ventricular system and the spinal subarachnoid space.

[Electroencephalographic findings in liver function disorders].

Abnormal rhythmisations in the EEG are described in hepatoportal encephalopathy as an expression of a disturbance of more profound cerebral structures. The EEG gives an actual information about the state of the cerebral functions of all stages of a hepato-cerebral decompensation and allows a reliable prognostic estimation during the observation of the course. Practically significant are also bioelectric findings of a disturbance of the cerebral function with clinically symptom-poor course in compensated liver cirrhosis or obstructive icterus.
