The effect of calcitonin on osseous healing in guinea pig mandible.

Calcitonin (CT) is reported to be an effective medication for the treatment of inflammatory root resorption and to be capable of stimulating osteoblast proliferation in cell culture studies. In this study the effect of CT on the healing of osseous defects was evaluated in the mandibles of guinea pigs. After raising tissue flaps two experimental cavities were created on both sides of the corpus mandible of 33 guinea pigs. CT was applied into cavities either in hydroxypropyl methycellulose (HPMC) gel or gelatin as carrier. HPMC and gelatin alone and an empty cavity were also examined as control groups. Histopathological examinations under light microscopy were performed on weeks 1, 3, and 6. At week 1 in CT+gelatin and CT+HPMC groups, prominent osteoblastic activity was observed when compared with control groups. At week 3 the presence of woven bone in the experimental cavity areas reflected the increased osteoblastic activity in all groups. At the end of week 6 woven bone was gradually replaced by osteogenic tissue undergoing remodelization with Haversian systems in all groups. It is suggested that the osseous healing of the experimental cavity was enhanced by CT application in early stages (i.e. at week 1). However there was no significant difference of osteogenic activity between the control and CT-treated groups at the end of weeks 3 and 6.

Effects of abdominal location and epididymal or vasal obstructions either individually or in association on ipsilateral and contralateral testes. A histologic and DNA flow cytometric analyses.

OBJECTIVE: An experimental study was conducted to evaluate the damages in ipsilateral and contralateral testes in individual or associated presences of abdominal location and vasal or epididymal obstructions. METHODS: Six groups each consisting of 8 rats were established. The groups included sham operation, ligation of the vas deferens, detachment of the epididymis from testis, abdominal placement of the testis, abdominal placement of the testis with vas deferens ligation, and abdominal placement of the testis with detachment of epididymis from testis. After 30 days, bilateral orchidectomy was performed. Mean seminiferous tubular diameters (MSTD) and mean testicular biopsy scores (MTBS) were obtained for each testis. Relative proportions of haploid, diploid and tetraploid cells were determined by DNA flow cytometry. MSTD, MTBS and the proportions of haploid cells were compared through one-way analysis of variance. RESULTS: While vas deferens ligation has diminished MSTD only in the contralateral testes, abdominal testis and detachment of epididymis have diminished MSTD in both ipsilateral and contralateral testes. MTBS were depressed only in the ipsilateral testes in groups of abdominal testis, vas deferens ligation and detachment of epdidymis. However, ratios of haploid DNA were depressed in both ipsilateral and contralateral testes. Abdominal testis together with vas ligation or detachment of epididymis has further depressed the ratios of haploid DNA in both ipsilateral and contralateral testes. CONCLUSION: Compared to their individual presence, the associated presence of abdominal testis and vasal or epididymal obstructions may augment the damages encountered within the ipsilateral and contralateral testes.

Melanotic neuroectodermal tumor of infancy: report of two cases and review of literature.

Melanotic neuroectodermal tumor of infancy (MNTI) is a rare, distinctive neoplasm containing melanin; it primarily affects the maxilla of the infants during the first year of life. Approximately 150 instances of this tumor are reported in the medical literature. Genesis of the tumor is obscure and the diagnosis is challenging for the pathologist. Two cases operated by the first author are presented, and the diagnostic features and treatment alternatives of MNTI discussed.

Tumor thickness as a predictor of cervical lymph node metastasis in squamous cell carcinoma of the lower lip.

Tumor thickness is a relatively new prognostic factor that has been investigated for lower lip cancer. This study was performed in 27 patients, 13 of whom had histopathologically confirmed cervical metastasis, to investigate whether tumor thickness could be used as a predictor of cervical lymph node metastasis. The mean tumor thickness of those cases with neck metastasis was 5.60 mm (SD = 2.24), and the mean thickness of cases without neck metastasis was 3. 79 mm (SD = 1.68). The difference between tumor thicknesses of both groups was found to be statistically significant (P < 0.05). The tumor thickness of 5 mm was determined as a cutoff point, above which the cervical lymph node metastasis rate was significantly increased. As a conclusion, tumor thickness is an objective histopathologic factor that is easily reproducible; it significantly influences cervical lymph node metastasis in lower lip cancer, and it may be used in the assessment of prognosis.

Prognostic value of p53, proliferating cell nuclear antigen, and Ki-67 expression in undifferentiated nasopharyngeal carcinomas.

In this study the prognostic importance of p53, proliferating cell nuclear antigen (PCNA), and Ki-67 expression was analyzed along with the clinical parameters in 35 consecutive patients with undifferentiated nasopharyngeal carcinomas. Immunohistochemistry was used to detect p53, PCNA, and Ki-67 staining. Among the clinical findings, stage IV disease (P = 0.01), cranial nerve paralysis (P = 0.02), and lymph node metastasis (P = 0.06) were associated with shorter survival. The p53 positivity correlated with the presence of lymph nodes, but it was not a significant factor to predict the outcome. PCNA expression was not found to be a prognostic indicator. On the other hand, the proliferative value of Ki-67 staining was suggestive of prognosis. A proliferation index of Ki-67 less than 10% indicated longer survival (P = 0.03). There was no correlation between Ki-67 staining and PCNA index. As a result, the prognostic value of Ki-67 may alert the physician to more aggressive and adjuvant treatment modalities.

Technetium-99m pertechnetate uptake in ectopic parathyroid adenoma.

A 37-year-old male with a persistent increased parathyroid hormone level, after subtotal thyroidectomy and parathyroidectomy, was referred for scintigraphic localization of a possible ectopic parathyroid adenoma. Tc-99m pertechnetate and Tc-99m MIBI scintigraphy were performed on separate days. There was marked uptake of both tracers in the mediastinum, which at surgery was confirmed to be an extrathyroidal parathyroid adenoma. Hypervascularity was suggested as a possible explanation for rare cases of pertechnetate avid parathyroid adenomas. And Tc-99m MIBI scintigraphy proved to be a successful imaging procedure for ectopic parathyroid tissue.

Prognostic significance of histopathological parameters in cancer of the larynx.

In this study we investigated the prognostic significance of differentiation, the mode of tumor invasion to surrounding tissues, the microscopic appearance of tumor, peritumoral lymphocytic infiltration and cartilage involvement according to disease-free survival, and the recurrence and presence of cervical lymph node metastasis in cancer of the larynx. Only the mode of tumor invasion to surrounding tissues was significantly related to survival (P < 0.05). The patients with "well-defined margin" tumors survive significantly longer than those with "groups of cells, no distinct margin." Patients with supraglottic tumors and a mode of invasion other than "well-defined margin" have a significantly higher risk of recurrence (P < 0.05) and therefore require adjuvant therapy. Patients with poorly differentiated, cartilage invading, ulcerative supraglottic tumors, and patients with glottic tumors having diffusely infiltrating margins, certainly need elective neck dissection (P < 0.05). According to the multivariant analysis, none of the factors significantly affect disease-free survival independently (P > 0.15). According to multiple logistic regression and cox regression analysis, in decreasing order of significance, the mode of invasion, microscopic tumor appearance and lymphocytic infiltration significantly affect the recurrence and time between surgery and the development of recurrence independently (P < 0.15).

Childhood splenic hemangiopericytoma: a previously unreported entity.

The first childhood case of splenic hemangiopericytoma in a 10-year-old boy is presented. The clinicopathologic features of this unique entity are discussed with special emphasis on differential diagnosis and treatment by comparing with the previously reported adult cases. There are no specific presumptive clinical and laboratory findings, including tumor markers and imaging techniques that distinguish hemangiopericytoma from other splenic masses. The most important diagnostic aid is to bear this entity in mind when a child presents with an unexplained splenic mass. Splenectomy associated with chemotherapy or radiotherapy in the presence of systemic or local recurrences is mandatory for the appropriate treatment.

Primitive neuroectodermal tumor of the orbit in an adult. A case report and literature review.

OBJECTIVE: The authors describe the salient clinical, radiologic, and histopathologic features of an orbital primitive neuroectodermal tumor in a 28-year-old man. This is an extremely rare tumor of the orbit, previously reported exclusively in children. DESIGN: Case report. INTERVENTION: Excisional biopsy of the tumor en bloc was performed. MAIN OUTCOME MEASURES: Histopathologic examination was performed by standard techniques and immunohistochemical stains on formaldehyde-fixed, paraffin-embedded tumor tissues. RESULTS: Histologic examination of the sections of the tumor showed small, blue, round cells with occasional Homer-Wright rosette formations. The tumor cells stained positively with neuron-specific enolase and vimentin. CONCLUSIONS: This newly recognized, highly unusual peripheral primitive neuroectodermal tumor should be considered in the differential diagnosis of hypercellular, small, round cell tumor of the orbit in adults.

A comparative study of the effects of thrombin and electrodesiccation used for haemostasis on inflammation and adhesion formation.

Electrodesiccation or chemical agents, such as thrombin and fibrin sealant, may be used to control oozing in the peritoneal cavity. Electrodesiccation is time consuming and associated with adjacent thermal damage. Adhesion formation remains a concern with the use of thrombin and fibrin sealant. In this study, adhesion formation and various histological parameters of inflammation were evaluated following haemostasis with electromicrodesiccation or thrombin in the rabbit model (n = 36). Following laparotomy, the right uterine horn was subjected to a measured injury producing sufficient oozing. After the injury was effected, the animals were randomized to haemostasis with electromicrodesiccation (n = 18) or thrombin (n = 18). In the first phase of the study, the histological parameters of acute injury and haemostasis with either modality were evaluated in two animals in each group. In the second phase, one, two and 10 animals, in each group, were submitted to second-look laparotomy on post-operative days 2, 7, and 15, respectively and the type and extent of adhesions were quantified. Histological parameters of inflammation as well as the type and extent of adhesions were comparable between the two groups. We conclude that local application of thrombin is not associated with a statistically greater degree of post-operative adhesions when compared to electromicrodesiccation.

Arterial thrombosis leading to intestinal infarction in a patient with Behçet's disease associated with protein C deficiency.

Behçet's disease may be a possible cause of both occlusive and aneurysmal arterial involvement as well as recurrent venous thrombosis. A case of Behçet's disease complicated with vascular involvement leading to intestinal infarction is presented. A 41-yr-old man suffering from Behçet's disease for 15 yr presented with a 2-day history of severe abdominal pain and bloody diarrhea. Intestinal infarction secondary to thrombosis of the superior mesenteric artery had been diagnosed during surgical exploration 3 yr previously. He was started on anticoagulation with nutritional support. The patient was readmitted with severe diarrhea and malabsorption symptoms 3 yr after intestinal resection. A thrombus located in the posterior wall of the infrarenal portion of aorta was detected by aortography and ultrasonography. Although thrombosis is a relatively common complication of Behçet's disease caused by vasculitis, protein C deficiency, which is a pertinent laboratory finding in this case, might be a secondary factor in the thrombotic event. This is the first case reported of mesenteric artery thrombosis leading to bowel infarction and abdominal aorta thrombosis associated with protein C deficiency.

Carcinosarcoma of the urinary bladder.

A case of carcinosarcoma of the urinary bladder is reported. Transurethral resection was performed. Histologically the tumorous mass was composed of osteosarcoma, squamous cell carcinoma and transitional cell carcinoma. The histological findings, immunohistochemical features and clinical manifestations of bladder carcinosarcoma are discussed in relation to the literature.

Effects of antiglaucoma drugs on ocular surface.

Topical medications have a very important role in chronic glaucoma treatment. Long-term use of these medications can cause significant changes on ocular surfaces. In this study, the ocular surfaces of 20 control subjects (group I), 20 primary open-angle glaucoma patients (group II) treated (mean 21.20 +/- 1.32 months) with 0.50% timolol maleate, and 20 primary open-angle glaucoma patients (group III) treated (mean 21.70 +/- 1.34 months) with 0.50% timolol maleate + 1% dipivefrin hydrochloride were evaluated. Studied parameters included Schirmer's test, tear break-up time, conjunctiva impression cytology and goblet cell density. These results suggest that long-term applications of topical anti glaucoma medications damage the ocular surface.
