[Effect of ciprofloxacin therapy in duration of bacterial excretion in acute salmonella gastroenteritis].

In view of the increasing incidence of non-typhoid salmonellosis, the effect of early treatment with ciprofloxacin on the permanent elimination of salmonella was evaluated. In a prospective study, 14 patients with non-typhoid salmonellosis were treated with 2 x 500 mg/d ciprofloxacin for 10 days within 5 days of onset of symptoms. Relapse occurred in 4/14 (29%) patients 2-3 weeks after termination of therapy. Using sero- and ribotyping, relapse was confirmed and reinfection ruled out in 4/4 patients. Furthermore, ribotyping suggested double infection in one patient. Development of resistance to ciprofloxacin was not observed. We conclude that the use of antimicrobial chemotherapy to treat non-institutional salmonellosis in immunocompetent patients cannot generally be recommended but must be considered carefully in each case. Indications for ciprofloxacin therapy in the treatment of non-typhoid salmonellosis are discussed.

Characteristics of CDC group 1 and group 1-like coryneform bacteria isolated from clinical specimens.

Fifteen strains of CDC group 1 coryneform and biochemically similar bacteria were isolated from clinical specimens. Of the 15 strains isolated, 11 were derived from abscesses and purulent lesions, mostly from the upper part of the body, and 3 were grown from blood cultures. Nine strains were associated with mixed anaerobic but no other aerobic flora. Seven strains exhibited the classical biochemical profile of CDC coryneform group 1; however, eight strains were unable to reduce nitrate and were called "group 1-like." Other reactions to differentiate CDC group 1 and group 1-like coryneform rods include alpha-hemolysis on human blood agar, fermentation of adonitol, and the presence of alkaline phosphatase. Fifteen strains showed marked CAMP reactions on different erythrocyte agars. Gas-liquid chromatography of volatile and nonvolatile fatty acids as well as cellular fatty acid patterns and the composition of cell wall components suggest that CDC group 1 and group 1-like coryneform bacteria do not belong to the genus Corynebacterium but possibly to the genus Actinomyces or Arcanobacterium. DNA-DNA hybridization studies revealed that group 1 and group 1-like strains represent different species.

Isolation of Escherichia fergusonii from four different sites in a patient with pancreatic carcinoma and cholangiosepsis.

Escherichia fergusonii was isolated from a 69-year-old male with pancreatic carcinoma and cholangiosepsis from gallbladder fluid, three blood cultures, feces, and a superficial wound of the abdomen. Biochemical reactions, antimicrobial susceptibility patterns, susceptibility to polyvalent phage 0-1, and rRNA gene restriction analysis suggested that the four strains were of clonal origin. Our data indicate that E. fergusonii possesses a pathogenic potential in humans.

Pitfalls in the diagnosis of intussusception.

Unusual sonographic behavior of intussusception is demonstrated in 4 cases. The results of 54 well-documented cases have shown two possible deviations from the normal sonographic pattern of an intussusception. The first was a highly reflective target sign, and the second a loss of regular sonographic structure in a tumor-like mass.

[Interference factors in the in vivo diagnosis of the thyroid].

Anamnestic iodine contamination and medication with thyroid drugs were registered in 260 patients of a clinical thyroid care unit and 200 patients of a thyroid doctor's office in the southern German endemic goiter region. We found in the university clinic 54% patients with interfering factors, 30% with iodine contamination and 20% with multiple interferences. In the physician's practice there were 18%, nearly all treated with thyroid hormone. The influence of those factors on thyroidal technetium uptake and the resulting restriction on its diagnostic value is discussed.

Infections due to Actinomyces neuii (former "CDC coryneform group 1" bacteria).

Sixty-seven patients with infections in which the recently proposed species Actinomyces neuii was involved were observed during a 4 1/2-year period. Microbiological as well as clinical features of the patients are presented. A. neuii does not cause typical actinomycosis and is detected most often in material from abscesses or infected atheromas. One case of a fatal septicemia caused by A. neuii in a non-immunocompromised host was seen. Antimicrobial susceptibility patterns of A. neuii isolates and results of various treatment regimens are discussed.

Diversity of coryneforms found in infections following prosthetic joint insertion and open fractures.

In a 5-year period, 73 coryneform isolates from prosthetic joint and open fracture infections in 60 patients treated in a hospital specialized in orthopedic surgery were speciated. The most frequent species were Corynebacterium amycolatum, Corynebacterium striatum, Corynebacterium diphtheriae biotype mitis, and Corynebacterium jeikeium. At least 14 isolates were deemed clinically significant as sole agents of infection.

Emergence of related nontoxigenic Corynebacterium diphtheriae biotype mitis strains in Western Europe.

We report on 17 isolates of Corynebacterium diphtheriae biotype mitis with related ribotypes from Switzerland, Germany, and France. Isolates came from skin and subcutaneous infections of injecting drug users, homeless persons, prisoners, and elderly orthopedic patients with joint prostheses or primary joint infections. Such isolates had only been observed in Switzerland.
