Al2O3-ceramic as material for dental implants: experimental and clinical study for the development of screw- and extension-implants.

A screw implant and a blade-vent implant were developed for the alloplastic substitution of teeth. Forty implants were observed for 18 months in five beagles; these implants were under maximal functional mastication. No implant was lost. The screw implant showed a tight attachment of bone over the whole implant surface. With the blade-vent implant we observed a partial interposition of connective tissue. This difference seems to depend on the different insertion techniques. Photoelastic studies showed a good stress distribution through enlarged and rounded off attachment surfaces. The described insertion techniques allowed a primary tight attachment of the implant surface and bone support, which led to an immediate stability of position. In the screw implant the physiologic mobility of teeth was imitated by means of a resilient element. Data from 30 cases were gained in a clinical 3-year follow-up. One case failed. For conclusive judgement, a longer period of time and more clinical cases are necessary, but existing data are encouraging.

[The expert testimony in lesions of the lingual nerve].

Experiences acquired as a medical evaluator have shown that malpractice proceedings are increasingly being transformed into clarification proceedings. For this reason, the most important aspect of the dentist's obligation is to inform the patient of possible complications. In addition, injury to the lingual nerve can be prevented by protecting the mobilized lingual mucoperiosteal flap.

[Diagnosis and therapy of a retropharyngeal abscess].

A 53-year-old female patient developed a phlegmon of the floor of the mouth after external extraction of 38 acutely inflammed teeth. The phlegmon tracked retropharyngeally and settled on the opposite side. It was difficult to estimate the amount of tissue area affected by the abscessed phlegmon because of severe trismus. The fact that the inflammation had basically tracked retropharyngeally and inferiorly was ascertained via a lateral radiograph of the neck which showed that the lower part of the prevertebral soft-part shadow was considerably wider than normal. The life-threatening infection healed after the abscess was incised and drained externally. The abscess was approached medial of the vascular band.

[Studies on non-metal materials in oral surgery].

Two non-metallic implant materials are introduced and the indications for their use are described. A maxillo-facial indication exists in the reconstruction of the temporo-mandibular joint and the orbital floor. We find dental-surgical indications for the Aluminium-Oxide ceramics as well as for implants in the replacement of single tooth gaps and unilateral as well as bilateral shortened dental rows. The indication in the toothless jaw is the most unfavourable and should be restricted to replacement of both canine teeth.

[Studies on the pharmacodynamics and compatibility of the local anesthetic carticain].

Two local anesthetics were compared with a placebo with regard to subjective tolerance and influence on driving capacity in a double blind test in eight male probands. Neither performance, the ability to respond, reactivity, nor fine motor coordination were reduced or impaired five to sixty minutes after a submucous injection of 3.6 ml of the local anesthetics tested. There were no changes of self-evaluation in the sense of a changed active-passive polarity.
