[Enlargement of the sella in primary hypothyroidism].

Length of the sella, depth of the sella and surface of the sella profile of 37 primary hypothyroids were compared with a normal group. All the three parameters were significantly different, though situated in the normal area. The results are discussed on the basis of literature. The reactive formation of hypophyseal tumours at adult age in primary myxoedema is relatively rare. However, in every enlargement of the sella also a primary hypothyreosis should be regraded differential-diagnostically. A therapeutic experiment with thyroid hormones should always be made.

Pharmacokinetics of Metronidazole in pregnant women.

Pharmacokinetic characteristics of metronidazole were studied in 19 pregnant women with Trichomoniasis urogenitalis who had been hospitalized for legal abortion. All patients received 250 mg or 1.0 g metronidazole (Vagimid) per os in a single or multiple dose. Peak levels were reached 1-2 h after application. Mean serum levels of metronidazole were somewhat lower in gravidae than those in nonpregnant women. The mean elimination half-life values were 5.7 and 7.7 h after intake of 250 mg and 1.0 g metronidazole, respectively. As suggested by our pharmacokinetic results, the dosage regimen for treatment of anaerobic bacteria infection or parasitic infection in pregnant women can be the same as in nonpregnant patients.

[Prognosis, chronification and therapeutic relations in unsatisfactorily improved secondary faulty developments with cardiovascular affections].

In a group of 36 neurotic women with unsatisfactory psychotherapy results, prognostic criteria, chronifying factors in the course, therapeutist-patient relations, test data, and social and clinical criteria were ascertained for an assessment of success by neutral examiners and statistically correlated. The significance of common prognostic criteria is relativised and the therapeutist-patient relation is determined as an influencing factor through which prognostic and chronifying factors are translated into the therapeutic process. The relevance of the paranoid scale of the MMPI as an objective prognostic sign is particularly noted.

[Antibody response to hemagglutinin and neuraminidase induced by an influenza subunit vaccine (author's transl)].

To study antigenicity, persistence of antibody (efficacy) and reactogenicity of a new bivalent influenza subunit vaccine, Sandovac, it was given in two different concentrations (1000 and 2000 IU) to 156 subjects from two epidemiologically destinct areas (119 students from Essen, average age 22 years, 37 residents from Kassel average age 56 years). Serum antibody response was measured by hemagglutination inhibition (HI) and neuraminidase inhibition (NI) tests before, one, two and six months after vaccination. The following results were obtained: 1. Four weeks post vaccination an excellent strain specific and crossreactive (H3 N2 variant-specific) antibody response was observed. Subtype specific (H0, H1 or H2) activity could not be detected. 2. Six months post vaccination a still notably higher antibody level as compared to the preimmunization status could be demonstrated. 3. A difference, however, existed between the immune responses of the Essen student population with an average age of 22 years, and of the Kassel resident population with an average age of 56 years. The immune response being significantly better in younger vaccinees. 4. High and low concentrated vaccines revealed neither objective nor subjective differences in reactogenicity and were very well tolerated. 5. In all immunological parameters tested the double concentrated vaccine (S 2000) proved to be slightly superior to the lower concentrated vaccine (S 1000) that is officially licensed in the German Federal Republic.

The estimation of drug plasma levels in epileptics.

The estimation of antiepileptic drugs in plasma is an indispensble resource in the handling of seizure patients in many countries. Using our results, simple methods for the estimation of antiepileptic drugs are discussed.

Problems in the kinetic evaluation of capacity-limited processes by means of the analog computer MEDA 82 T.

A method for the calculation of dose-dependent pharmaco-kinetics by means of a combined zero and first-order model at the analog computer is presented. The results of the combined model can be converted into Michaelis-Menten parameters using simple equations. The method is usable for i.v. injection and for e.v. modes of application in the case of cmax greater than c = co/e.

[Caffeine-stimulated lipolysis in relation to pathologic pregnancy].

In an acute experiment the effect of Coffeinum-Sodium-salicylicum (3 mg/kg body weight) on the parameter of lipolysis Free Fatty Acids (FFA) and free glycerin in early pregnancy is investigated. On the base of literature data the possibility of labour induction due to high blood-values of FFA and glycerin is explained and the importance of the coffeine-stimulated lipolysis in pregnancy is discussed. To prevent one possible cause of abortus or miscarriage it is recommended to restrict taking coffee during pregnancy.
