Perhexiline maleate-induced hepatitis.

5 patients treated with perhexiline maleate, 200-400 mg/day for at least 6 months, exhibited evidence of hepatitis. The picture was very similar to acute alcoholic hepatitis, clinically, biologically and histologically with presence of necrosis, Mallory's hyaline, polynuclear infiltration and to a lesser degree, steatosis. Association with peripheral neuropathy, hypoglycemia, and renal failure appears strikingly frequently. The evolution was severe since 3 patients died within 6 months, even after treatment withdrawal. Further studies are to be done to understand the mechanisms of hepatic and neurologic toxicity, and to measure the hazards of this drug. These studies could bring a new insight to alcohol toxicity.

[Toxic effects of essential oil of Cannabis sativa L. and main constituents on planarian (Dugesia tigrina) (author's transl)].

Regenerating pieces of planarian worms are able to absorbe insoluble substances deposited on the base of the vessels in which they are cultivated. This biological test was used to study the toxic effects of the essential oil of Cannabis sativa L. The hydrocarbons such as pinene (alpha and beta), caryophyllene and so on were not toxic. On the contrary caryophyllene oxide was highly toxic. It was not possible to detect any protection by 5-hydroxytryptamine as it was the case against delta 1-tétrahydro-cannabinol and cannabidiol.

[Domiciliary assisted respiration by tracheotomy for chronic respiratory insufficiency (author's transl)].

Prerequisites for success in domiciliary assisted respiration for chronic respiratory insufficiency are the understanding of the patient and his active participation in the treatment as well as adjustment and attitude of his environment. The authors report their experience in 89 patients. Spirographic data were obtained in 72 patients which permitted subdivision into a group with restrictive disturbances and one with obstructive changes. The differences in prognosis in these two groups of patients were surprising. The survival rates in the obstructive diseases after 3, 5 and 10 years were 54%, 38% and 33% respectively; the corresponding values for the restrictive diseases were 84%, 84% and 69%. These differences are statistically significant. The restrictive changes are therefore considerably more favorable to assess, also with respect to a better quality of life.

[Determination of the lean body mass in adult using the impedance method].

It deals with a new formula which gives the estimation of lean body mass (LBM) by the electric impedance measure at 1 MHz. According to the authors the weight of LBM is set up in two parts: the total water (TBW) conductive of electricity and an inert part (ILBM) which is a bad conductive one: the weight is an individual constant at the grown up. ILBM is calculated from simple morphological datums (H = height and C = wrist circonference). From which one concludes the formula: LBM = TBW + ILBM.

Ophthalmic Graves' disease.

Ophthalmic Graves' disease is a peculiar clinical condition, characterized by ocular symptoms, without thyrotoxicosis. A survey of 22 observed cases is discussed. This condition is 25 times less frequent than classical Graves' disease. The eye symptoms include upper lid retraction, exophthalmos, and severe ophthalmopathy. Unilateral symptoms are frequent (16 out of 22). A negative T3 suppression test was found in half of the patients. LATS was detectable in 3 out of 17 of the cases. The stimulation of TSH secretion by TRH was normal in 3 out of 6 of the cases. When the tests were combined and repeated, a positive response was found in 8 out of 9 of the exophthalmic patients, and in 3 out of 9 of the patients with lid retraction. Appreciation of the course of the disease necessitates a prolonged follow-up; transformation into classical thyrotoxicosis have been observed. Our clinical studies bring into focus the relationship of ocular symptoms with Graves' disease and Hashimoto's disease.
