Long-term exercise training with constant energy intake. 1: Effect on body composition and selected metabolic variables.

The effects of long-term negative energy balance generated by exercise while maintaining a constant energy intake were studied in five healthy young males (25 +/- 3 years; mean +/- s.d.). After 2 weeks of monitoring to determine habitual energy intake, they were subjected to a 100-day experimental period during which they exercised on a cycle ergometer, 6 days a week, twice a day at 55 per cent of VO2 max. The daily energy deficit was 4.2 MJ for a total of 353 MJ for the duration of the treatment. Body weight decreased from 86.7 to 78.7 kg (P less than 0.001). More than 80 per cent of the weight loss was accounted for by the reductions in fat mass. Fat-free mass did not change significantly. The data suggest that subcutaneous fat was lost about evenly from the trunk and from the limbs as estimated from skinfolds. Cardiorespiratory and metabolic responses to submaximal work loads indicate that the low intensity exercise regimen was capable of inducing some of the known adaptations of exercise training such as a decrease in plasma insulin and norepinephrine concentrations during exercise, reductions in heart rate, pulmonary ventilation and blood lactate concentration at a given exercise intensity, and an increase in skeletal muscle oxidative capacity. The energy cost of sitting and standing was not decreased in spite of the loss in body mass. However, the energy cost of submaximal cycling and walking exercises was reduced considerably. These results clearly demonstrate that it is quite possible to lose fat while preserving fat-free mass through regular prolonged exercise of moderate intensity if energy intake is kept constant at baseline level. They also emphasize the importance of the individual differences in response to negative energy balance.

[Enzymatic resistance to cefotaxime in 56 strains of Klebsiella spp., Escherichia coli and Salmonella spp. at a Tunisian hospital (1984-1988)].

The enzymatic and plasmid-encoded resistance towards oxyimino-beta-lactams has been recently reported as related to the production of an extended-spectrum beta-lactamase (e.g. SHV-2, CTX-1 or TEM-3), in particular in our hospital since 1984. The prevalence of that resistance has been examined from January 1984 to December 1988 in function of specimen, unit and type of enzyme among 8,421 isolates of Klebsiella spp., E. coli, Salmonella spp. Each isolate showing a diameter of inhibition zone size inferior or equal to 25 mm for cefotaxime, the double disk synergy test was performed between a disk of amoxicillin and a clavulanic acid disk of cefotaxime, ceftriaxone, ceftazidime and aztreonam. In case of synergy, sonicated extracts have been prepared and examined by isoelectrofocusing with the detection of beta-lactamase activity by ceftriaxone and nitrocefin. 56 isolates (K. pneumoniae, K. oxytoca, E. coli, S. wien, S. typhimurium scored positive including 27 in pediatrics, 18 in surgery, and 8 in medicine. 41% of isolates have been obtained from blood cultures and 26.8% from urines. A majority of isolates (49/56) produced the SHV-2 type, but other types mediating the resistance phenotype CTX have been individualized initially by their isoelectric points e.g. 5.4 (TEM-20 in K. pneumoniae in July 1986), 6.4 (TEM-21 in E. coli, 1 K. pneumoniae in July 1988). The prevalence of resistance to cefotaxime from 1984 to 1988 has increased (from 0.3 to 1.4%), the highest rate being observed in pediatrics (5.6% in 1984 and 22.1% in 1988).

[Cardiogenic shock caused by 5-fluorouracil].

The authors report the observation of one patient in whom the first administration of chemotherapy with fluoro-5 uracil has induced a severe but reversible cardiogenic shock. The latter was preceded by a prolonged, constrictive thoracic pain without myocardial infarction constitution. The cardiac toxicity of fluoro-5 uracil has been well described in the form of anginal pains receding with the discontinuation of the treatment. However, the possible occurrence of arrhythmias, of collapse and even sudden deaths leads the authors to suggest the daily clinical and electrical observation of the patients treated with fluoro-5 uractil.

[Antibacterial effect of cefixime in the presence of the type of beta-lactamases produced by Enterobacteriaceae].

The susceptibility of isolates or Enterobacteriaceae to orally absorbed beta-lactams (amoxicillin, cephalexin, cefaclor, cefuroxime, and cefixime), was maximum for the iminomethoxy-aminothiazolyl-cephalosporin but variable according to bacterial species. For E. coli, P. mirabilis, Salmonella, Shigella, K. pneumoniae, K. oxytoca (group 1) MIC50 were congruent to 0.06 mg/l, and MIC90 congruent to 0.12 mg/l. Finally for C. freundii, E. aerogenes, E. cloacae, S. marcescens, M. morganii, MIC50 were higher, congruent to 1, and MIC90 16 mg/l. The slight increase reported between MIC50 ans MIC90 of cefixime against isolates belonging to group 1 was related to stability towards beta-lactamases (TEM, SHV) unlike groups 2 and 3, where the drug was less stable to chromosomal type 1 enzyme. The discovery of extended-spectrum beta-lactamases (ESB) (e.g. SHV-2, CTX-1 or TEM-3), mainly in K. pneumoniae, led to further investigations of the behavior of cefixime. The in vitro activity of cefixime (CFM), amoxicillin (AMX) combined or not with clavulanate (AMC, 2 mg/l), ticarcillin (TIC), cephalothin (CF), cefotaxime (CTX), ceftazidime (CAZ) and aztreonam (AZM) was determined by the agar dilution method (inoculum 10(5) cfu/ml) against clinical isolates: E. coli (26), K. pneumoniae (42), K. oxytoca (9), Salmonella (3) producing several types of beta-lactamases, chromosomal (cephalosoporinase, broad-spectrum) or plasmid-encoded (TEM-1, TEM-2, CTX-1, SHV-2, SHV-3, SHV-4). The geometric mean MIC values of AMX, AMC, TIC, CF, CTX, CAZ, AZM and CFM were as follows: greater than 480, 21.6, greater than 285, 45.6, 0.53, 1.12, 0.51, and 0.77 respectively. An MIC increase of oxyimino beta-lactams (CTX, CAZ, AZM, CFM) was observed against over-producing isolates of K. oxytoca and isolates producing ESB (CTX-1, SHV-2, SHV-3, SHV-4). The comparative behavior of cefixime was determined after transfer by conjugation to E. coli K-12. Sixty-one derivatives were constructed producing TEM-1, TEM-2, SHV-1, CTX-1, TEM-4, CAZ-2, SHV-2, SHV-3, SHV-4, the MICs of cefixime were stable against derivatives producing a penicillinase (TEM-1, TEM-2, SHV-1), but not against those producing an ESB. The MIC increases of undigestible beta-lactams ranged from 1 to 135 fold (CFM), 40 to 200 fold (CTX) 12 to 232 fold (CAZ), and 4 to 240 fold (AZM).

Detection of extended broad-spectrum beta-lactamases in Enterobacteriaceae in four French hospitals.

In 210 strains of Enterobacteriaceae which were isolated in four hospitals and which showed reduced susceptibility to cefotaxime, high synergy was demonstrated between amoxicillin (20 micrograms) + clavulanate (10 micrograms) and cefotaxime (30 micrograms) using a simple double-disk test. Isoelectric focusing on gel and specific iodometric detection using ceftriaxone identified four extended broad-spectrum beta-lactamases (isoelectric points 7.6, 6.3, 7.0 and 5.9) produced by the strains.

Genetic effect in resting and exercise metabolic rates.

Two studies dealing with the contribution of the genotype in individual differences for resting metabolic rate (RMR), thermic effect of a 4.2 MJ carbohydrate meal (TEM), and energy cost of submaximal exercise are reported. The genetic effect for RMR and TEM was studied in 31 pairs of parent-child, 21 pairs of dizygotic (DZ) twins, and 37 pairs of monozygotic (MZ) twins, whereas the heritability of the energy cost of submaximal exercise was determined from data on 22 pairs of DZ twins and 31 pairs of MZ twins. The heritability of RMR reached approximately 40% of the variance remaining after adjustment for age, gender, and fat-free mass, (FFM). The genetic effect for TEM was equivalent to at least 40% to 50% of the variation in the energy expended during four hours after the meal test. A highly significant genetic effect was found for fasting plasma glucose (greater than .72), but the results for fasting plasma insulin are unclear. No significant genetic variance was seen for the glucose and insulin response to the carbohydrate meal. Finally, heritability for the metabolic rate during cycle exercise was high (greater than or equal to .46) at low power output, but it became nonsignificant when the energy cost reached about 6 times the RMR.

Teratological evaluation of Juniperus sabina essential oil in mice.

Juniperus sabina essential oil was evaluated for its fetotoxic potential on mice. Pregnant dams were injected s.c. (15-45 or 135 mg essential oil/kg body weight) on days 6 to 15 of gestation. They were killed and the uterine contents were examined on day 19 of pregnancy. The fetuses were removed for examination. The dams of the two higher treated groups showed a significant weight loss as compared to controls. An hepatotoxicity was observed among females that resorbed their whole litter, thus indicating a greater susceptibility towards Juniperus sabina essential oil during pregnancy. The essential oil induced, in the three treated groups, an embryotoxicity as manifested by a statistically significant increase in the number of affected litters; but no fetotoxicity.

[Primary malignant melanoma of the bladder. Apropos of a case].

The authors report about a case of a primitive melanoma of the bladder that a man aged 77 years suffered of. The diagnosis was carried-out with certain difficulty on fragments of endoscopic resection and was discussed from the clinical facts as well as from the anatomo-pathological results and, more particularly, from the immuno-histochemical results. This study makes it possible that a comparison be made with those rare cases observed on a world-scale basis which are described in the literature. As far as the bladder is concerned, it constitutes also an attempt of histogenetic explanation of this kind of tumor.

[Percutaneous drainage nephrostomy guided by ultrasound. Apropos of 80 adult cases].

The carrying out of an ultrasound percutaneous nephrostomy requires the combined use of several technics: local anesthesia; ultrasound transducer-fixed puncture probe insertion; angiographic method; route dilatation. A study of a series of 80 DUPN operations carried out in patients with febrile upper urinary tract obstruction and/or acute failure, showed that this operation is simple and rapid (lasting 24 minutes on average). Failures occurred rarely (6.2%) and were most often related to insufficient dilatation of the renal cavities. Complications were limited: 1 case of hemorrhage, 1 case of urinoma, 4 cases of infection, no case of perforation of a neighboring organ. Incidents were more frequent and those were probe-related (9 dislodgments were required). Furthermore, the DUPN opens the way to many new diagnostic opportunities.

Characterization and identification of substances isolated from dialyzer extracts.

The organic phase of dialyzer dried extracts obtained from Cuprophan hollow fiber and polyacrylonitrile AN 69 parallel-plate dialyzers, all sterilized by ethylene oxide, were submitted to light and polarized light microscopy, infrared (IR) spectrophotometry, and gas chromatography coupled with mass spectrometry. Colorless polygonal (approximately 5 X 20 micron) and needle-like (approximately 3 X 50 micron) crystals were found in great quantity on microscopy examination. IR spectrophotometry of the crystals embedded in potassium bromide disc showed specific spectra in the 1400-800 cm-1 wave number range and typical peaks on wave number assigned for hydroxy or amine (3420 cm-1), aromatic hydrogen (3060 cm-1), methyl, methylenic, or methenyl (2960-2860 cm-1), and carbonyl (1715 cm-1) groups can be demonstrated. Seven peaks were detected on gas chromatography of the organic solution containing crystals. Fatty acids and undefined compounds could originate from the dialyzer. Phtalates may leach from blood tubing, used in the rinsing procedure, and BHT is a stabilizer of ethyl ether used as solvent. These compounds could also originate from the dialyzer since phtalates are widely used as a plasticizer and BHT an antioxidant for various polymers.

Effect of a three-day interruption of exercise-training on resting metabolic rate and glucose-induced thermogenesis in training individuals.

Exercise-training has been shown to influence resting components of energy expenditure in lean and obese individuals. Moreover, experimental data suggest that the effect of training on these components could represent an acute effect of exercise. In this regard, the present study was undertaken to determine whether resting metabolic rate (RMR) and glucose-induced thermogenesis (GIT) could be modified depending on the delay elapsing between the last exercise session of a training program and calorimetric measurements. Eight trained individuals were tested 16 h after a 90-min exercise bout and following a 3-day interruption of training. A significant decrease in RMR (-6.6 percent, P less than 0.05) was observed in response to exercise cessation. On the other hand, an increased GIT was noted following the 3-day rest period and this effect was also statistically significant. These results show that short-term interruption of training can affect substantially RMR and GIT in trained individuals, indicating that the time elapsing since the last exercise session must be taken into account in the interpretation of the effects of physical training on both these parameters.

Ocular abnormalities associated with cutaneous melanoma and vitiligolike leukoderma.

Several varieties of ocular pathology are associated with acquired cutaneous hypomelanosis (leukoderma; vitiligo). Our current study was undertaken to investigate the relationship between ophthalmologic disorders and a specific depigmentary phenomenon, the vitiligolike leukoderma of cutaneous melanoma. Over the past 14 years, eight patients with cutaneous melanoma and widespread areas of hypopigmentation were identified at the Pigmented Lesion Clinic of the Massachusetts General Hospital. The seven patients who underwent ophthalmologic examination had pigment-related ocular abnormalities. Among these were inflammations of the uveal tract in three patients, heterochromia in two, halo nevi of the choroid in one, and hypopigmentation and/or atrophy of the retinal pigment epithelium or choroid in four. Our findings demonstrate that ocular disease may be a component in a syndrome consisting also of cutaneous melanoma and vitiligolike leukoderma and suggest the need for complete ophthalmologic examinations in patients with melanoma and leukoderma.

[Pharmacokinetic study of pefloxacin in 37 hospital patients under resuscitation].

Pharmacokinetics of pefloxacin were studied in thirty-seven intensive care patients. Fluorimetric assay was used for determining pefloxacin concentrations. Serum concentrations were usually far above threshold levels; however, wide variations across patients were evidenced, with no demonstrable relation to physiologic status. For this reason, pefloxacin assay is useful in high risk patients to avoid subconcentrations.
