Palliative cryotherapy in inoperable prostate carcinoma.

Cryotherapy was applied by us as a palliative in 91 cases of carcinoma of the prostate. 1. Disobstruction was achieved in 87% of patients. 2. Results proved stable over time. 3. Operating mortality, survival curve, number of deaths, etc. was not statistically different in patients treated with cryotherapy compared with patients undergoing other treatments. 4. Precutions are required to combat infection of cryotherapy patients in the first few weeks. We feel authorized to pursue cryotherapy in the future exploiting all its immunologic possibilities.

Effect of adrenocorticotropic hormone on pure exocrine pancreatic secretion in man.

The effect of adrenocorticotropic hormone (ACTH) on pure exocrine pancreatic secretion was studied in 7 subjects with external transduodenal drainage of the main pancreatic duct performed after biliary tract surgery. Intravenous injection of 0.25 mg of ACTH during a prolonged intravenous infusion of secretin (0.5 clinical units per kg-hr) plus cholecystokinin (0.5 Ivy dog units per kg-hr) significantly reduced protein and lipase (both concentration and output) without affecting volume and bicarbonate secretion. The reduction appeared soon after ACTH injection (peak inhibition in the first 15-min period) and lasted about 75 min. The adrenocortical response to ACTH reached its peak at the 60th min. The mechanism by which the pituitary hormone selectively inhibits pancreatic enzyme secretion without affecting volume and bicarbonate remains to be clarified.

An evaluation of echography in the diagnosis of pancreatic disease.

In the present study an attempt to evaluate the efficacy of echography as a diagnostic tool has been made. A total of 52 patients (chronic pancreatitis (42); pancreatic cysts (three); and carcinoma of the pancreas (seven)) were studied and the results compared with those from other diagnostic techniques. In 65.7% of chronic pancreatitis patients, and in all cases of carcinoma of the pancreas, echography provided evidence of pancreatic abnormality but in no case could an unambiguous diagnosis of the disease be made. However, in all cases of pancreatic cyst, echography gave precise and unequivocal diagnostic information. There was good agreement between the echographic picture and surgical findings. Cholangiography and duodenography indicated duodenal and choledochal compression in a high proportion of cases in which echography revealed enlargement of the head of the pancreas. It is concluded that echography is a simple, safe, and valuable addition to the techniques available for studying the pancreas.

Investigation of exocrine pancreatic function by continuous infusion of caerulein and secretin in normal subjects and in chronic pancreatitis.

Exocrine pancreatic function has been evaluated in 24 controls and 29 patients with confirmed chronic pancreatitis by continuous infusion (90 min) of synthetic caerulein, 100 ng/kg/h, plus GIH secretin, 1 CU/kg/h. Mean secretory values of the controls were comparable to those obtained by others using maximal doses of secretin and CCK. Unlike the controls, patients suffering from chronic pancreatitis demonstrated a progressive reduction in secretion during infusion. Thus, the comparison of responses for the final 30-min period gave much clearer and more complete discrimination between normal and abnormal pancreatic function than the preceding 30-min period.

Immunization against human and swine-like influenza A: serological response to two inactivated vaccines of different formulation.

The immunizing capacity and the tolerability of two purified vaccines containing the inactivated A/New Jersey/76 ( x 53 recombinant) swine-like strain were studied. A trivalent vaccine (A/New Jersey/76-300 I.U.; A/Victoria 3/75-300 I.U.; B/Hong Kong 5/75-300 I.U.) was administered to 31 subjects over 50 years of age. No side-effects were observed and it caused a good HI antibody response to A/New Jersey/76, fairly good for B strain and modest for A/Victoria 3/75. A monovalent vaccine (A/New Jersey/76-400 I.U.) was administered to 32 subjects in the age range from 16 to 43 years. HI antibodies occurred in 84% of the subjects, devoid of antibodies before vaccination. Younger subjects (between 16-20 years) generally developed fairly low antibody titres; one among them (16-year-old) had a temperature rise within 48 h after vaccination. The Authors expect that the X 53 recombinant with the surface antigens of the A/New Jersey/76 strain may be employed for the preparation of the inactivated vaccine to be used in an emergency situation and discuss the composition of the vaccine more adapt for the present epidemiological situation.

[Therapeutic effect of a pharmacologic combination of choleretics and digestive enzymes in exocrine pancreatic insufficiency].

Phosphorylcholine and homocysteine have an important choleretic action and also potentiate exocrine pancreatic secretion by way of stimulation and a more effective preparation of the substrate against attack by lipolytic enzymes. The protection offered by pancreatic enzymes in the correction of the digestive insufficiency in the endoluminal stage is also known. An analysis was therefore made of the action of an association of phosphorylcholine homocysteine and digestive enzymes in cases of exocrine pancreatic insufficiency and primary or secondary malabsorption. This action was evaluated on the basis of the reduction of elimination of steatorrhoea. For this purpose, faecal lipids were determined with the method proposed by van de Kramer et al. The most significant results were obtained in cases where digestive insufficiency was most marked, where correction of the pancreatic enzyme deficiency and improved biliary function were particularly required.

[Eosinophilic gastroenteritis. Report of a clinical case].

An unusual case of eosinophilic gastroenteritis is described, together with its differential diagnosis from allergic enteropathy. Numerous biopsies and other instrumental examinations pointed to small intestine as the only site and eosinophilic enteritis as a suitable classification. Disappearance of the clinical symptoms was obtained with corticosteroid management in the space of 6 months.

Interactive system for automatic analysis of human R-banded metaphases.

A computer system analysing human metaphase images and finding banding patterns of R-banded chromosomes has been set up. This first paper describes the interactive procedure in selection of metaphase images, automatically digitized starting from microphotographic negatives.

Effect of glucagon on pure human exocrine pancretic secretion.

In this study the inhibitory effect of glucagon on pure exocrine pancreatic secretion obtained by direct drainage of the duct of Wirsung in man is demonstrated. In two experiments carried out during stimulation with secretin and CCK-PZ and in one experiment during secretin stimulation an inhibitory effect on pancreatic secretion was found. This effect was global, affecting both the total volume and the concentration and output of protein and the output of bicarbonate. The greater inhibition, however, occurred in the case of protein.
