OBJECTIVE: Using digitized M-mode and Doppler echocardiography, we evaluated left ventricular (LV) function in 54 patients (43 women and 11 men; mean age 50 years) suffering from active rheumatoid arthritis (RA) without obvious cardiovascular disease, and compared them with 54 age- and sex-matched normal subjects. RESULTS: No differences were found in LV end-diastolic diameter, systolic function and parietal thickness between the patients and controls. However, a significant reduction in various indexes of LV diastolic function was found, including E/A (ratio of early to late filling waves of mitral inflow Doppler) and the peak lengthening rate of the LV diameter (an index of LV relaxation evaluated by M-mode echocardiography). The former was correlated with patient age and was independent of disease duration, while the latter was more markedly correlated with disease duration than with patient age. CONCLUSION: The relationship between diastolic impairment and disease duration in active RA may open new perspectives in the study of RA-associated cardiovascular disease.
A knowledge-based consultation system capable of making multiple diagnoses in noninvasive cardiology is presented here. The system, expressly designed for a personal computer, is able to provide assistance in the diagnosis of 49 disease-entities. When tested in 52 retrospective cases (33 patients with one disease-entity, 17 with two, 2 with three), it made 39 correct diagnoses (75%). The average score of the actually present disease-entities was 94 +/- 3.2 (mean +/- standard error of the mean), significantly higher than the highest score obtained by the incorrect diagnoses (87.2 +/- 3.1) (p less than 0.001). The system was able to correctly perform 1 triple diagnosis and 9 double diagnoses. The results obtained, comparable with those of well-known expert systems (MYCIN, INTERNIST-1), demonstrate the reliability of a microcomputer-based expert system for medical diagnosis in cardiology.
Despite a low incidence of clinical manifestations, autopsy data suggest endocardial and myocardial disease in about 50% of patients with systemic lupus erythematosus. To investigate whether mitral valve prolapse can be considered a clinical manifestation of cardiac involvement in systemic lupus erythematosus, we carried out an echocardiographic study in 51 affected subjects and 102 normals matched for age and sex. Prevalence of mitral valve prolapse was 25% in patients with systemic lupus erythematosus and 9% in healthy controls with a statistically significant difference (p less than 0.01). Neither pericardial effusion nor prolonged (more than 12 months) treatment with corticosteroids were associated with higher prevalence of mitral valve prolapse. Libman-Sacks verrucae on the mitral valve apparatus as well as focal myocardial scars affecting the papillary muscles and adjacent myocardium could be responsible for the development of the valvular dysfunction. We suggest that mitral valve prolapse can be considered a manifestation of cardiac involvement in patients with systemic lupus erythematosus.

The research over 1023 patients examined in a metallurgical firm for prevalence of essential hypertension has proven that 144 subjects were hypertensive (5 secondary and 139 essential hypertension). 51 patients (35.4%) did not know to be hypertensive, 26 (18%) were without therapy and 57 (39.5%) were inadequately treated. 75 subjects with essential hypertension were treated once daily with Atenolol 100 mg. After a six months treatment 85.4% of patients reached a good control of blood pressure (lying diastolic blood pressure less than or equal to 95 mmHg). Atenolol was well tolerated, few collateral effects have been encountered (13.3%) but mainly the treatment did not cause weakness nor diminished the work capability of the subjects which instead had proven to happen with non selective beta-blocking drugs.
OBJECTIVES: Evidence has been obtained indicating that oxidation of low-density lipoproteins (LDL) plays a relevant role in the pathogenesis of atherosclerosis and it has been proposed that, due to the antigenic properties of oxidized LDL, the anti-oxLDL antibody titre could represent a useful index of in vivo LDL oxidation. METHODS: Sixty-nine control subjects and 64 patients scheduled for selective coronary revascularization were investigated before surgery. RESULTS: The coronary disease patients had a higher level of total plasma cholesterol, LDL cholesterol and triglycerides, and a lower level of HDL cholesterol. Plasma anti-oxLDL antibody titre was measured as the ratio of antibody binding to CuSO4-oxidised LDL versus native LDL. The antibody ratio was higher in coronary patients as compared with control subjects (1.56 +/- 0.5 vs 1.0 +/- 0.3, p < 0.01). A ratio higher than 1.34 (mean of controls +/- one standard deviation) was present in 60% of the coronary patients. Subclass analysis indicated that the presence of diabetes mellitus and hypercholesterolaemia (but not of hypertension, generalized arteriosclerosis, myocardial infarction and cigarette smoking) increased the anti-oxLDL antibody ratio to 1.72 +/- 0.4 and 1.68 +/- 0.3 respectively. CONCLUSION: The results obtained indicate that a) a high titre of anti-oxLDL antibodies is present in plasma of patients with coronary atherosclerosis, b) in these patients LDL oxidation takes place in vivo and probably plays a critical role in the development and progression of atherosclerosis.
