[Arteriosclerosis obliterans of the lower limbs. Epidemiological study of the risk factors].

Chronic arterial occlusive disease of lower limbs is recognized as a typical multifactorial disease, but the role of some risk factors is still debated, because contrasting results have been obtained in epidemiological studies. We investigated the prevalence of hypercolesterolemia, hypertriglyceridemia, hypertension, smoking habits and diabetes in a population of 172 patients with peripheral arterial disease admitted to the III and V Medical Division of the "Ospedale Regionale" of Parma in the period January 1979 - April 1983, and in a control group of 174 subjects comparable for age and sex, free of clinical symptoms or signs of atherosclerosis. Statistically significant (p less than 0,005) increases in the prevalence of hypercolesterolemia, hypertriglyceridemia, smoking and hypertension were found in vascular patients compared with controls, the relative risk for the various considered factors being respectively: 6.40; 4.72; 4.03; 3.16.

[Echopolycardiography in the detection of cardiac lesions in rheumatoid polyarthritis].

In order to verify the extent and the frequency of cardiac complications in rheumatoid arthritis (RA), the authors performed mechanophonocardiographic studies and simultaneous mono- and bi-dimensional echocardiography in 28 patients with RA, 2 men and 26 women aged between 38 and 65 (mean = 51) (history of the disease ranging from 1 to 37 years). One case showed an increase in the PEP:LVET ratio on the polycardiogram and 18 cases showed echocardiographic alterations (64.3%), in particular: pericardial effusion in 6 cases (21.4%), thickening of the epicardium and the pericardium in 4 cases (14.3%), alterations in the mitral valve (reduction in the protodiastolic closing velocity of the anterior edge of the large mitral valve) in 10 cases (35.7%) and thickening of the interventricular septum in 5 cases (17.9%). These examinations are therefore good indicators of the cardiac complications of rheumatoid disease and allow a group of patients to be identified who would not otherwise be detectable. These patients can be followed up subsequently, in the light of the positive correlation between echocardiographic alterations and the duration and severity of the disease. The authors conclude that simultaneous mono-and bi-dimensional echocardiography represents the optimal investigation for defining the nature, the severity and the frequency of rheumatoid cardiac abnormalities.

Intestinal glucose absorption in adrenalectomized rats: transport by isolated microvillous membrane vesicles.

D-glucose uptake by microvillous membrane vesicles, isolated from the intestine of the adrenalectomized, control (pair-fed) and normal rat, and incubated in 1 mM D-[U-14C]-glucose was measured. No qualitative difference in the time course of transport in the presence of an initial gradient of NaSCN or KSCN (100 mM, out greater than in) was found among the different groups. The small but significant decrease of 50% equilibration time (t 1/2) of adrenalectomized rat vesicles was aspecific, being the same as that of control, pair-fed (semistarved) animals. Thus adrenalectomy did not modify the transport properties of the rat luminal enterocyte membrane.

Effect of 4'-oxythiamine on thiamin transport and phosphorylation by everted jejunal sacs, and on thiamin uptake by rat isolated enterocytes.

Two preparations were used for the present investigation. Rat everted jejunal sacs were incubated at 37 degrees C for 1 h in Krebs-Henseleit buffer, pH 7.4, containing 0.2 microM [thiazole-2-14C] -thiamin, with or without 2 microM 4'-oxythiamine. Suspensions of rat isolated enterocytes were incubated at 37 degrees C for 15 min in Ringer-Krebs, pH 7.4, containing 10 mM d-glucose and 0.125 microM [thiazole-2-14C] -thiamin, with or without 1.25 or 12.50 microM 4'-oxythiamine. Radiometric methods were used for the determination of: labeled thiamin net transport, free and phosphorylated thiamin content in the sac walls, total thiamin content in the isolated enterocytes. 4'-oxythiamine, at a molecular ratio with thiamin of 10:1, did not modify labeled thiamin net transport and accumulation by everted jejunal sacs, but caused a slight, statistically insignificant, decrease of labeled phosphorylated thiamin content of the sac walls. 4'-oxythiamine, at a molecular ratio of 10:1 or 100:1, did not inhibit labeled thiamin uptake by isolated enterocytes. Therefore, 4'-oxythiamine, which is known to be unable to inhibit thiamin enzymatic phosphorylation, did not affect thiamin intestinal transport both by everted intestinal sacs and isolated enterocytes. The present results can be interpreted as a further evidence that intracellular thiamin phosphorylation is an important step in thiamin intestinal transport.

Intestinal thiamin transport in rats. Thiamin and thiamin phosphoester content in the tissue and serosal fluid of everted jejunal sacs.

Rat everted jejunal sacs were incubated at 37 degrees C for 15-60 min in Krebs-Henseleit buffer, pH 7.4, with or without (control experiments) 0.2 microM [thiazole-2-14C]-thiamin. The determination of thiamin and its phosphoesters in the sac wall and in serosal fluid was carried out by an electrophoretic micromethod. Irrespective of the presence of 14C-thiamin, the tissue content of endogenous thiamin pyro- and triphosphate decreased during the incubation, whilst that of thiamin-monophosphate remained relatively constant. The tissue content of free thiamin increased substantially only in control experiments. Endogenous free thiamin, together with a small amount of monophosphate, was found to enter the serosal fluid. The transfer of both compounds was greatly enhanced by the incubation with 14C-thiamin, when an efficient thiamin phosphorylation could be demonstrated. During incubation with 14C-thiamin, the concentration of 14C-thiamin-pyrophosphate and, to a lesser extent, that of free 14C-thiamin increased progressively in the tissue, while 14C- thiamin-monophosphate content remained almost unchanged. No 14C-thiamin-triphosphate was detected. There was a rapid increase in the tissue specific radioactivity of free thiamin and thiamin-monophosphate, which preceded the rise in the specific radioactivity of thiamin-pyrophosphate. The specific radioactivities of the former compounds in the serosal fluid reflected those observed in the intestinal tissue. These results are interpreted as evidence suggesting that the active transport of thiamin is efficient only when intracellular thiamin phosphorylation is operating.

[Practical use of the Multistat III centrifuge analyzer in an improved method for the determination of the activity of aspartate aminotransferase and alanine aminotransferase].

We applied three procedures for measuring aspartate aminotransferase and alanine aminotransferase activities to the Multistat III MCA. Assay conditions were almost exactly as prescribed by the Scandinavian Committee on Enzymes and the International Federation of Clinical Chemistry. Owing to the overall good performance of the analyzer and its loader module, results of 20 samples will be available in 10 minutes, including preincubation with pyridoxal phosphate. Details of procedures are given; there are no differences in activities of both aminotransferases determined with three methods, allowing the user to take a choice according to personal preference.

[Serological diagnosis of systemic connective tissue inflammation; a new immunofluorescence method for demonstrating antinuclear antibodies].

Two methods for the demonstration of antinuclear antibodies by means of indirect immunofluorescence were employed on 83 serum samples from 80 patients, with rat liver as the substrate. The first method (the classic ANA-test) uses cryostatic slices fixed in acetone; the second (ANA-Bp-test) is based on fixation of the substrate with Bouin's fluid and embedding in paraffin. The series was composed of 25 cases of lupus erythematosus (28 serum samples), 10 of rheumatoid arthritis, 1 of dermatomyositis, 8 of progressive systemic sclerosis, 4 of unclassifiable systemic connective tissues diseases, and 28 various internal and rheumatological forms. The undiluted sera that proved positive were subsequently tested for Ig typing and ANA titration. The ANA-Bp-test was found to be more sensitive, safer, more practical and less expensive than the ANA-test.
