Impaired clearance of cholephilic anions in Rotor syndrome.

The behaviour of some cholephilic anions has been studied in three male patients (two of whom are brothers) affected with Rotor syndrome. The unconjugated bilirubin clearance was markedly reduced with impairment of the plasma-liver transfer constant rate (uptake defect). BSP retention at 45 min. was elevated without late rise of the anion. Transport maximum ("Tm") of BSP was slightly reduced; relative hepatic storage capacity of BSP ("S") was significantly impaired. Plasmatic retention of 131-I-Rose Bengal was increased with a diminished hepatic uptake of the compound. The hepatic excretion of IDA-derivatives was impaired, but less for parabutyl-IDA than for diethyl-IDA. It is concluded that in Rotor syndrome there exists an alteration of uptake and storage of several cholephilic organic anions: the alteration is common not only to substances that undergo intracellular conjugation--such as bilirubin and BSP--but also to substances that are not metabolized--such as Rose Bengal and IDA--derivatives. Finally the authors suggest that the Rotor syndrome and so--called "Hepatic storage disease" are the same condition.

Functional study with 131I-rose bengal in chronic active hepatitis and nonactive cirrhosis using a three-compartment model.

The authors, using 131I-Rose Bengal, determined the fractional transfer constants of a three-compartmental model in 30 patients divided into two groups (chronic active hepatitis and nonactive cirrhosis) on a histological basis. Other functional indexes were also taken into consideration. In nonactive cirrhosis, the results show a greater impairment of Rose Bengal transfer from plasma to liver. In chronic active hepatitis K32 (fractional transfer coefficient from the second to the third compartment) appears significantly lower than in nonactive cirrhosis. No significant differences have been found for K12 and K03. The best differentiation between the two groups (consistent with histological data in 29 of 30 cases) was obtained by grouping K21, retention index (RI) and SGOT in the discrimination function: L=4.88 K21-0.016RI+0.0034SGOT. The ratio K12/K21 seems meaningful for diagnostic purposes and in the evaluation of the hepatocellular damage in single cases.

[Constitutional hyperbilirubinemia and mixed bilirubin. Clinical and functional study].

Three cases of non-haemolytic constitutional icterus with prevalently conjugated bilirubin have been studied: one case of Dubin-Johnson syndrome and two cases of Rotor's syndrome. Confirmation was obtained for the differences between the two syndromes from the physiopathological (by means of the BSF load test and determination of urinary excretion of porphyrin) and morphological (macroscopic, histological, ultrastructural) viewpoints. The following were carried out in all cases: a load curve with crystalline bilirubin, study of the course of the curve and application of compartmental analysis to the curve. Findings underlie the hypothesis that Rotor's syndrome contains an uptake defect and an accumulation change on the part of the liver cell of organic anions and that it is therefore possible to suggest some identity between Rotor's syndrome and the so-called "Hepatic storage disease".

Platelet-associated IgG in acute and chronic hepatic diseases.

In order to investigate the role of an immune mechanism in the pathogenesis of thrombocytopenia in hepatic patients, we measured platelet associated IgG (PAIgG) in 84 patients with various hepatic diseases. Increased PAIgG levels were found in 84% of patients with chronic active hepatitis (mean 21.32 +/- 8.45 fg/platelet) and in 75% of those with cirrhosis with hepatitis (mean 17.3 +/- 11.2 fg/platelet). In these patients there was no significant correlation between PAIgG and platelet count. Increased PAIgG amounts were also observed in some patients with inactive cirrhosis (18%). Normal PAIgG values were found in all patients with acute viral hepatitis and chronic persistent hepatitis. An immunologic mechanism may play a role in the pathogenesis of thrombocytopenia in hepatic patients. Furthermore, measurement of PAIgG may have a great usefulness in the differential diagnosis of chronic hepatic diseases.

A controlled study comparing cimetidine treatment to an intensive antacid regimen in the therapy of uncomplicated duodenal ulcer.

The authors report the results of a randomized study in which comparison was made between two different kinds of treatment in patients affected by uncomplicated duodenal ulcer endoscopically diagnosed. The first group was treated with 1 g of cimetidine per day, during a period of four weeks (200 mg three times a day and 400 mg at bedtime); the second with a liquid Al-Mg antacid compound, 210 ml/day 30 ml, 1 and 3 hr after meals and 30 ml before bedtime) for four weeks. Fity-one patients were studied, 27 treated with cimetidine, 24 with antacids. At the end of the four-week period, 21 patients (77.7%) in the cimetidine group and 18 patients (75%) in the antacid group were completely healed. Benign side effects were remarked in both types of treatment, none of which made it necessary to suspend treatment. No significant variation of the basal and peak acid output before and after each kind of treatment was observed, while a slight but significant increase in fasting serum gastrin concentration was noted after treatment in the antacid group.

[Contribution of laparoscopy-biopsy to the diagnosis of primary liver neoplasms].

After a review of the literature, personal observations of primary benign and malignant tumours of the liver, studied using laparoscopic biopsy are presented and the importance of this examination in the early diagnosis of primary hepatic tumours is stressed. This examination, particularly when associated with biopsy, is very useful since it reduces diagnosis time with little or no risk to the patient with a high percentage of positive results.

[GH secretion after L-dopa administration in hepatic cirrhosis].

A 500 mg L-Dopa administration in normal and cirrhotic subjects does not determinate a significant varation of plasma glucose and insulin level, while a peak plasma GH level in both cirrhotic and normal subjects occurred at 90' with a significantly greates values in cirrhotics. It is doubtful to affirm that a high plasma GH level is the only one factor which responsable of glucose intolerance in cirrhotic subjects, when there are many others factors contribute to it in synergic way. In order to elucidate the hypothesis on possible pathogenetic mechanism it is discussed some of our own experience and observations.

[Secretion of GH and diabetic retinopathy].

Recently it has been studied a possible pathogenetic role of GH in diabetic angiopathy. The purpose of the present study was to verify whether the diabetic retinopathy is associated or not with a high plasma GH level. In an attempt to answer this question we did the dosage of plasma GH after a oral glucose load (110 g), Insulin (0,1 U/Kg) i.v., and arginin i.v. (25 g/30'). The plasma GH measurements were approximately the same as in normal patients. The Authors underline the importance of GH in pathophysiology of diabetic angiopathy and report all recent literature on this argument.

[The association of diabetic disease with Friedreich's disease in members of the same family].

Friedreigh's disease is only rarely associated with diabetes mellitus and only few cases are to be found in the literature, especially as regards siblings. Three cases of this association in members of two different families are reported. Two of the patients died from cardiovascular complication; the third case is that of a 32-yr-old woman who was able to complete a pregnancy. The pathogenesis of the association is discussed and a number of theories are put forward. The most generally accepted genetic hypothesis makes it possible to explain under a common metabolic denominator neurological disturbances and those in other systems. The association is not a purely casual event but is perhaps dependent on either two different genes or on a single gene with pleitropic effect. Heart signs are frequent in Friedreich's disease and brief mention is made of these.

[Simple glaucoma and diabetic disease].

Blood sugar, blood insulin and the insulinogenic index after oral glucose were studied in 41 subjects with simple glaucoma. Clinical diabetes was present in 9.76% and asymptomatic diabetes in 21.95%. It is suggested that a high incidence of diabetes may be expected in simple glaucoma, probably as expressions of a common genetic disorder.
